The very rare case reported here is a periodontosis with hyperkeratosis palmo-plantaris. The patient is a girl, nine years of age. Her father and mother are first cousins. Shortly after birth, skin changes of hands and feet appeared: the skin in the area of palms and soles became coarse and scaling, some superficial grooves were noted. Otherwise the skin presents a normal aspect. The deciduous teeth erupted normally, but shortly afterwards the gums became inflamed and purulent; the teeth loosened and lost. Her permanent teeth erupted normally, but at her 6th years of life, she suffered again from inflamed and suppurating gums. This inflammation is still present.
The gingiva was a severe, almost generalized inflammation with deep pockets containing a purulent exudate. Diastemata could be fairly seen. The roentgenograms showed severe generalized alveolar bone resorption. Excepting these, she enjoys good health and has never suffered from special diseases.
The patient had a thorough examination, including local and systemic tests. No systemic factor could be found in this patient and almost all of the findings were in the normal range. Analysis of the systemic and local factors in this case could not offer a satisfactory explanation for the genesis.
