 Human errors often have great effects on quality, safety and efficiency in healthcare: e. g., giving a medication to the wrong patient, wrong- side surgery, overlooking patient’s allergies, not connecting the monitoring system when the patient is moved, or filling prescriptions with the wrong drug. Although many healthcare organizations have challenged to reduce the errors and succeeded, most of these activities depend on the try- and- error approach without understanding human error characteristics.
 This paper discussed some typical misunderstandings that may be obstacles to the activities for reducing human errors, and then explained how to promote the activities.
