Complete prevention of bleeding and avoidance of complications are prerequisite in surgery for unruptured aneurysms. We report a difficult case with 2 internal carotid aneurysms at the anterior choroidal artery and at the carotid bifurcation. We stress the importance of preserving the perforating arteries and multiple-clip-adjustment.
