Patients with cleft lip and palate are known to have high caries prevalence due to the presence of alveolar cleft and dental anomaly. In this study, we investigated caries in patients with bilateral or unilateral alveolar cleft in Hokkaido. 48 patients aged 4 and 5 years with bilateral or unilateral alveo lar cleft undergoing initial examination at Orthodontic department of Hokkaido University Hospital from January 2014 to December 2018 were investigated decayed and filled deciduous tooth（d, f）for each tooth using panoramic X-ray, oral photo, and medical records in the initial examination, and the dental caries prevalence rate, the average number of df per person, df rate, and the caries incidence rate of each anterior deciduous teeth in cleft and noncleft side were calculated. The dental caries prevalence rate at 4 and 5 years was 75.0％ and 59.4％ respectively. The average number of df per person was 1.8 and 2.7 and the df rate was 9.0％ and 13.9％, respectively. All values are higher than that of The 2016 Survey of Dental Diseases results in Japan. The df rate of the anterior deciduous teeth on the cleft side at ages 4 and 5 was 25.9％ and 24.3％, and that of other deciduous teeth was 5.4％ and 11.6％, respectively. It was indicated that the alveolar cleft increased the caries risks.
