    OBJECTIVE: The purpose of this study was to clarify families' thoughts regarding surrogate decision-making, nursing support and nurses' thoughts regarding nursing support in critical care.
    METHODS: Data were collected using participant observations, reviews of medical records, and semi-structured interviews. We qualitatively analyzed the data using a modified grounded theory approach.
    RESULTS: The analysis revealed 8 factors related to the 7 families: decisions influenced by critical care unit circumstances, decisions from the patient's point of view, the evaluation of one's own decisions, acceptance and anxiety of the future, information seeking, requesting patient and familial support, difficulty recognizing nursing support, and the nurse representing the salvation for the family. In addition, 8 factors related to the 10 nurses were identified: constant working demands, consideration of lifesaving and the sanctity of life, familial relations according to the needs of the family and patient, the circumference is adjusted, getting close to families, collaboration with others of the same opinion, difficulties in living one's own life, and the necessity of developing one's own intellect and sensitivity.
    CONCLUSION: We need to ensure that consistent nursing support for surrogate decision-making by family members is provided, regardless of the place or situation ( e.g., critical care) .
