We report a case of hypoglycemia caused by multiple medications of CNS drugs including venlafaxine (VEN). The patient, a female in her twenties, was brought to the emergency room after taking an overdose of 10,800 mg valproic acid, 112 mg ramelteon, 140 mg olanzapine, 405 mg mirtazapine, 4 mg mecobalamin, and 1,050 mg VEN. The patient was diagnosed as having serotonin syndrome with myotonia, myoclonus, tremor, and agitation. On the second day after admission, her blood glucose level dropped to 59 mg/dL in the morning and glucose correction was performed, but her blood glucose level dropped to 78 mg/dL before lunch. She ate all her lunch, but her blood glucose level dropped to 79 mg/dL before the evening meal, and her glucose level was again corrected. The patient was discharged from the hospital on the third day without hypoglycemia.
