    Diabetes is characterized by a high incidence of age-related microvascular and atherosclerotic vascular dysfunction along with other complications. HbA1c is a useful marker that can be monitored to reduce the risk of vascular disorders in diabetic individuals. In diabetes treatment, blood glucose should be controlled according to degree of independence and level of living environment of patients. Recently, our country has promoted community-based integrated care system in which it is desirable to share information on a patient’s medication with home health care providers for the purpose of continuous medication therapy after discharge. In providing the information, use of a document, such as a medication management summary, is considered to be important. In this study, we examined two cases in which ward pharmacists provided discharge support to elderly diabetic patients. The support may lead to appropriate medication guidance and management of medication after discharge, which may result in achieving good blood glucose control.
