    Two cases of Larsen's syndrome, that we have recently experienced, were reported.
    Case I, a male, aged 10 years, was born via an uncomplicated delivery.
    Case II, a female, aged 6 years.
    They had prominent forehead and wide spaced eye.
    They had bilateral hip, knee and elbow dislocations. In addition, they had foot deformity.
    Radiographic studies showed abnormalities.
    They had extra calcaneal ossification, short metacarpals, supernumerary carpal bones and anormalous segmentation of vertebrae.

    This case is a boy aged thirteen years, who sustained a slight injury to his left elbow while playing soft ball game. He visited our hospital with a complaint of his left elbow pain.
    Radiographs showed a picture often described as characteristic of congenital dislocation of the radial head. As a result of clinical and X-ray examinations this case was diagnosed as the congenital anterolateral dislocation of the radial head.
    The radial head was removed and the ulnar nerve was translocated anteriorly with satisfactory results.

  • 1. Isozyme Pattern of Liposarcoma

    Esterase isozymes were studied in the extracts of bone and soft parts tumors. The isozymes were extracted with 0.25M sucrose solution and separated on polyacrylamide gel according to Tomino et al. Esterase activity was detected in the polyacrylamide gels by the method using α-naphthyl acetate and hexazotized pararosaniline.
    In the tissue varieties examined. a composite total of 12 discrete esterase bands were discerned.
    The extracts of three specimens of lipomas revealed the same isozyme pattern as normal adipose tissue. In the extracts of three liposarcomas, the isozyme pattern showed greater heterogeneity.

    Five cases of benign chondroblastoma were presented in this report. Three cases had tibial involvement, talus and pubis was also affected in each one case. Four males and one female. The age range was 12 to 28 years.
    Histologically, tumors were consist of oval chondroblasts and giant cells with cartilage matrix, stained slightly with Alcian blue.
    The ultrastructures of chondroblast and cartilage matix were studied utilizing TEM and SEM. The well developed and sometimes dilated endoplamic reticulums and filaments in the cytoplasm were characteristic and also, protein-mucopolysaccharide was detected by PAM staining. By energy dispersive analysis of X-ray, apatite clusters with 0.5 to 0.8μ in diameter in tumor matrix showed a little different patterns of Ca, P and S from the epiphyseal cartilage matrix.

    During these 5 years we have experienced 5 cases of spinal cord neurinoma (2 men, 3 women). One was multiple and 4 were single. When classified by injured sites of the vertebrae, 2 were in the thoracic vertebrae, 2 in the lumbar ones and the rest (multiple case) was distributed in the changing part from the thoracic to lumbar vertebrae. Patients complained of lumbago, pain and numbness in the limb, and were diagnosed at the first medical examination as radicular ischias and spondylosis deformans. The period from the onset of symptoms to surgical treatment extended from 3 months to 12 years, though 4 cases were within 2 years. The other was of the multiple type, and as it was encountered by a traffic accident, its symptoms were complicated and to diagnose was very difficult. These may be the causes of spending a long time until surgical treatment.
    All the cases were treated by surgical total extirpation, and histologically diagnosed as neurinoma. Postoperative courses were uneventful.

    Needle biopsy was conducted in 18 cases of soft tissue tumors, and the success that biopsy made in soft tissue tumors was 94.4%.
    Especially the diagnostic correctness rate proved at 94.1%.
    Technique, instruments, clinical materials, localisation and successful results were discribed with figures and tables.
    The complications were little. Swelling occurred in only one case following biopsy.
    Advantages of needle biopsy are that (1) the method is simple and safty, (2) in the great majority of cases the method can be applied to out-patient and (3) the technique can be applied by non-specialists.

    Reticulum cell sarcoma is one of malignant lymphomas. The most common site of departure is lymphnodes, but it rarely starts in a bone site, which is referred to as reticulum cell sarcoma primary in bone.
    We have a case of the rare onset, which is the multiple bone lesion of extremities without lymphnode lesion. Roentgenologically, lt. radius, capitate, 3rd metacarpal of lt. hand, rt ulna, metacarpals of rt hand, distal end of lt. humerus, lt. fibula, metatarsals of both foot are extensively destroyed without periostal reaction and new bone formation. These lesions were surprisingly healed by 60Co irradiation, but multiple metastatic lessions have appeared in the subcutaneous tissue and lymphnodes.

    A case of intraosseous ganglion is reported. Forty-nine-year-old woman was first seen for gradual onset of pain in the dorsum of the left foot of two weeks' duration. The positive physical findings and slight swelling and tenderness at the dorsum of the left foot. Roentgenograms revealed a cyst-like lesion of the 3rd metatarsal.
    The margin is slightly sclerosing, and a periosteal reaction is seen in the cortial bone.
    All lavolatory data is within normal limits.
    During excision of the cyst, we found a jelly-like fluid in the cyst. Histological section showed mucinous degeneration of the connective tissue, normal bone, and no lining-epithelium.

    A boy aged 12 was admitted to our clinic on January 1976, complaining of pain in both knees. The roentogenographic examination revealed lytic lession of the cortex in the distal metaphysis of both femurs with excessive periosteal reaction. The histological findings did not show typical pattern of Histiocytosis X.
    Culture of curettage material revealed Staphylococcus. The patient did not response to antibiotics, but responsed dramatically to vincristine, so we clinically suspects Histiocytosis X rather than osteomyelitis.

    Bone cement (Palacos R) was used in the internal fixation and replacement of bone tumors. 2 cases of primaly malignant tumors of bone; malignant xanthofibroma (1) and multiple myeloma (1), 3 cases of metastatic bone tumors; uterus cancer (1) and Grawitz's tumors (2), and 3 cases of benign bone tumors and tumorous conditions; aneurysmal bone cyst (1) and giant cell tumors of bone (2). All patients were given relief of pain and got functional improvement.

    Alkaline phosphatase activity (ALP) as well as Ca and P levels were measured in sera of 22 patients with cerebral palsy (CP), administrated with anti-epileptic drugs for a considerably long term, and 71 CP patients without epilepsy.
    Serum ALP in CP patients with epilepsy was proved to be lower than the average value in 87.5% of male and 66.7% of female. Serum Ca levels were found to be within normal limits in all of cases. Serum P levels were proved to be within normal values in almost all of them.
    In CP patients without epilepsy, serum ALP was proved to be lower than the average value in 93.5% of male and 92.5% of female. Serum Ca levels as well as P levels were found to be within normal limits in all of the cases.

    We examined the serum chemical test of ALP, Ca, P, GOT, and GPT in 22 patients who were continuing to take the antiepileptic drug for a fairly long time and 71 patients who have no epilepsy in our hospital.
    As a result we found the decline of ALP in the CP children who have the epilepsy. The ALP value of the CP children who have no epilepsy was showed the fall of less than the average value in 27 male children and 37 female children.
    On the other hand we found the effective difference between the average of each age of normal children and the average which we examined in the CP children.
    Ca and P was within the normal limits.
    We recognized AP III in the ALP isozyme of the CP children.

    Serological examinations of ESR, CRP, RA-T, RAHA, ANF, LE-T and Au antigen were performed for 106 rheumatoid patients during two years, and the data was compared with clinical manifestation.
    CRP was correlated with the inflammatory changes of the arthritis rather than ESR.
    Positivity of RA-T was 82.8%.
    RAHA was correlated with Waaler-Rose reaction, but the titer had no relation with the clinical manifestation.
    Positivity of ANF was 40%. There was the tendency for ANF to be positive when the inflammation showed acute change in severity.
    Positivity of Au antigen was 6.7%.

    Isolated fractures of the greater multangular are rare.
    It occur in about 2 or 3 per cent of all fractures of the carpal bones. The fractures mechanism of trauma are almost evenly diviped between direct violence to the wrist and hand and falls on the hyperextended hand. We reported that the isolated fracture of greater was strongly compressed by the first, second metacarpal, small multangular and scaphoideus on radial deviation of the wrist.

    A 45-year-old female has broken the shaft and intertrochanter of her femur by labour accident, which were treated by a pin and an intramedullary nail, respectively. The intertrochanteric fracture healed with a slight varus deformity, Subcapital fracture of the femoral neck occurred 4 months thereafter, which was thought to be a fatigue fracture and was operated on by a screw and bone peg fixation. The fracture healed completely after one year.

    Although traumatic habitual dislocation of the peroneal tendons is statistically uncommon, its occurrence can be expected to increase in frequency to parallel the rapid growth of skiing as a participant sport. There are already forty two cases in Japan. We recently experienced one case, had good effect by modified Dr. Vries' method.

    Anterior tibial syndrome is uncommon, and only 16 cases have been recorded in the Japanease literatures.
    A 38 year-old man who suffered from this syndrome after baseball is reported.

    Six cases of epiphyseal separation took place by different causes has been presented. Four of them were occured by traumatic lesion, one by inflammation and in one case the cause was uncertain. Traumatic lesions were satisfactorily treated manipulatively or operatively. A case of separation of proximal femoral epiphyseal plate of the hip joint was found at 45 days after onset of the initial symptom of suppurative arthritis. On examination in slipped capital femoral epiphysis, the levels of 17-OHCS and 17-KS in urine were found decreased.

    Case 1. An eleven-year-old boy, got a fracture of the lower pole of right patella by direct blow while gymnastic exercise. The fracture was operated on.
    Case 11. A fourteen-year-old boy, got a avulsion fracture of left tibial tubercle while high jumping. The lesion was treated by open reduction and internal fixation.
    Case 111, A twelve-year-old boy, injured his left knee joint by inversion while running. His knee was immobililized in extension for two months, resulting active extension of the knee was lost. A malunited avulsion fracture of left tibial tubercle was found at our clinic, and the patellar tendon was reconstructed by Campbell's method.

    We had six patients who are received, 1st, 2nd and 3rd degree burns over 60% of the bodydsurface. Emergency and early treatments of burn shock by intravenous and oral administration were done at Red Cross Hospital, Kumamoto.
    Only one of them left our hospital on foot after 5 months of hospitalization and five patients died. We studied the fluid therapy of them.

    Twelve rupture of the achilles sendon were treated non-surgical.
    The method of the treatment was immobilization in a walking plaster boot cast applied with foot in gravity equinus position for eight weeks. After the cast removed, the patient used a 2.5cm, heel lift for four week and undertook active gastrocnemius strengthening exercise. The results of the treatment showed ten satisfactory, and one fair for old rupture.
    A rerupture occured two days after initial plaster immobilization and was subsequently operated.

    In a severe crush fracture of the calcaneus, part of the lateral cortex may be displaced to lie under the lateral malleorus.
    This may cause local pain and tenderness either by direct bony contact or by compression of the peroneal tendons.
    Previously the surgical treatment of the complication has been excision of the displaced bone. However, in this way a trouble of calcaneal width does not improve. Excision of the tip of the latcral malleorus by ISBLSTER (1974) is good procedure in this condition.
    In our cases, five of the six patients reported were completely relieved of lateral pain.

    Zur Diskussion gestellt wurde hier zunächst über das Behandlungsergebnis von der lateralen Humeruskondylenfraktur bei insgesammt 31 Patienten, die in meiner Privatklinik behandelt worden waren.
    In den Fällen, wobei die Frakturfragmente nicht über 1mm. verschoben waxen, erziehlte konservative Behandlung das gute Ergebnis.
    Bei einem Kinde hat es eine Pseudoarthrose gebildet, bei denen anfänglich keine Fragmentverschiebung zu erkennen war. Durch blutige Reposition konnte eine sichere Frakturheilung erreicht werden. Man sah jedoch in 2 Fällen die Frakturheilung in Cubitus varusstellung (nach längerer Beobachtungszeit von jeweils 1.5 und 4 Jahren).
    Es taucht bei der blutigen Reposition stets das Problem mit dem geeigneten Fixationsmittel auf. Der Fechtendraht nach Professor M. Yamamoto hat es logisch gelösst und er ist leicht verwendbar. Mein eigenes verbesserten Modell wurde bei der Gelegenheit vorgestellt.
    Es gab die Fälle, die nicht einfach von einer Transkondylenfraktur zu differenzieren liessen.
    Bei der medialen Kondylenfraktur ist es wegen ihrer äusserst Instabilität, kann eine exakte Fixation nur dann ermöglicht werden, wenn man einen wirkungsvollen Fixationsdraht verwendet.

    For the treatment of the acromio-clavicular joint dislocation we have over 50 conservative methods and over 30 operative methods.
    But only a few reports gave the long-term follow-up and comparative studies of each method.
    According to their reports, we have traced the very good results to the conservatively-treated cases (Kenny-Howards Harness) even in the IIIrd degree dislocation in the fresh cases and to Gurd's operative method in the old cases.

    When the dynamic reduction in the Pavlik Harness fails to occur, in the initial treatment of congenital dislocation of the hip, Overhead Traction is used usually. However, aseptic necrosis of the femoral head were reported in the cases of Overhead Traction, we avoided using Overhead Traction, and adopted reapplication of Pavlik Harness in these cases since five years ago.
    The results were as follows,
    Reduced
    1) reapplication of Pavlik Harness 6 Hips
    2) Manual Reduction after (1)** 18
    3) Open Reduction 3
    total 27 Hips
    Aseptic necrosis of the femoral head was none.
    **After four weeks from reapplication of Pavlik Harness, the adduction contracture of the affected hip was disappeared, and Manual Reduction, with the hip fully flexed and abducted, would be able without any force.

    Twenty-seven abnormal findings, which were observed at one year of age on CDH in 25 infancies treated with Pavlik's bandage, were followed up until 5 to 8 years of age. The abnormal findings were hypoplasia of the acetabular roof, lateralization and epiphysitis of the femoral head.
    The follow-up results of 27 hips showed anatomical healing in the 21 hips (77.8%), and the causes of unsatisfactory results were considered to mainly osteochondritis of the femoral head.

    For the purpose to find out C. D. H. 6791 newborn infants were examined during the year 1961-75.
    Among them, thirty-five were found to have positive click sign, and the number of girls was 3.6 times as many as that of boys. In the case of breech presentation C. D. H. were found in higher incidence.
    We have treated them with modified Pavlik-harness or diapers as soon as they were found and in more than 80% successful reduction was obtained.

    Twenty-four unreduced CDH treated primarily by Pavlik's bandage were experienced for the past 4 years. Our method of treatment is the classical Lorenz's method for one to 2 months followed by Pavlik's bandage for 3 to 4 months. While three hips were re-dislocated within two weeks after Lorenz's method, they could be all reduced by additional Lorenz's method. Two hips showed residual subluxation at the end of our treatment were successfully reduced by Lange's method.
    Fifteen hips (62.5 %) out of 24 CDH showed excellent and good short-term results.
    Our method of treatment is believed to be a reliable method for CDH unreduced by Pavlik's bandage.

    The author noticed a deformity of the upper femoral region which occurred after the closed treatment of congenital dislocation of the hip joint and attempted to elucidate this cause by clinical observation.
    Of 6 cases with this deformity, the process of its development was analysed by persuading the course of treatment and radiographic changes.
    It was found out that most of the cases had been treated before three months of age. They were immobilized in frog-leg plaster cast following closed reduction.
    On the radiographs all of them showed not only the involvement of epiphysis and physis of the femoral head, but also metaphysis.
    In comparison with normal cases, all of them showed the growing disturbance of the femoral neck.

    The muscle release operation has been performed on 17 patients of Perthes' disease, and the results were followed up for ten years in regard to clinical and radiological aspects on 10 cases. Following are the conclusions based on this study.
    1. No clinical complication has been observed except in one case in which joint pain and fatigability of the lower extremities were complained.
    2. Radiological examinations disclosed the increase of femoral neck-shaft angle, elongation of the femoral neck and deformity of the femoral head in some cases, ascribable to the release of the trochanter major.
    3. The trochanter major developed to the normal size in its new position.
    4. There were found deformity of the trochanter minor in a few cases as a probable result of the release of the iliopsoas muscle.
    5. It is still an open question whether this procedure is effective or ineffective as a treatment of Perthes' disease.

    Since 1974 we have performed total hip replacement proposed by Charnley and Müller on 16 patients or 20 joints including 11 cases of osteoarthrosis of the hip joint (15 joints), three cases of salvage operation follwing intracapsular fracture of the neck of femur and two cases of necrosis of the head of femur. The average age of the patients was 50 years, ranging from 35 to 67 years. The follow-up study for six to 23 months, one year on the average, revealed a marked improvement in 16 cases, from preoperative 50 points to postoperative 85 points according to the clinical evaluation standard recommended by the Third ad hoc Committee of Japan Orthopedic Society. The grades of satisfaction by patients themselves were also high, 94 per cent of them expressing Grade A and B. There was, however, one case of recurrent pain owing to loosening 22 months after the operation.
    This was fortunately corrected by a revisionary operation, but there remains a possibility that similar cases could be expected in future.
    Therefore, one should be cautious in reducing the age limit for this operation, even though the satisfaction grades of the operated patients were favorable.

    High-Density Polythylen (H. D. P.) artificial femoral head had been applied to 35 cases of the fracture of femur neck, but many of them had brought down unsuccess within 2 years after replacement operation. By revision operation of 6 cases of them, it was confirmed that unsuccess was caused of the wear of H. D. P. head. Histology showed that fine particles of the weared H. D. P. induce the foreign body inflammation granuloma in the capsule and in the neighbor bone tissue. Bone destruction comes first to proximal region of the femur and finally to the acetabulum.

    Case 1. 36 years old woman suddenly developed an “Aching” pain in the right shoulder about ten days. Then she noticed weakness of the shoulder. Her recovery was nearly complete within four months.
    Case 2. 25 years old man was affected rihgt shoulder as same as Case 1. But his recovery did not appeared after three months.
    Case 3. 63 years old man suddenly developed an “Aching” pain in the right thigh. The pain subsided within six days. Then he noticed weakness of the knee. Hypesthesia was present in the region of the femoral nerve. His recovery was almost within four months.

    Steindler flexorplasty at the elbow was performed on seven patients of plexus brachialis injury and one patient of arthogryposis multiplex congenita.
    Follow-up research after the operation extended from six months to seven years and six months (three years and one month on the average). Analysis of the results after the operation was made in the range of motion and muscle strength at the elbow, according to Mayer and Green's method.
    All patients of plexus brachialis injury had good results, but three of them showed the loss of supination of the forearm. The patient of arthrogryposis multiplex congenita had poor result, but he felt comfortable because he could bring his hand to his face.

    Because the peroneal nerve is present just beneath the skin especially near the lateral aspect of the knee joint, it is very susceptible to injury.
    Authors presented a case of peroneal nerve palsy maybe caused by compression of the fabella and reviewed the literatures reported untill last year.
    Case report:
    53 years old engeneer admitted in February 1975 with a diagnosis of lumbar disc lesion. He had complained of pain and numbness on the lateral aspect of the right leg and the dorsum of the foot. Since the symptomes increased from the full extension of his knee, he was forced to keep his knee flexed at 15-20 degrees.
    On the physical examination, (1) hypesthesia on the lateral aspect of the right leg and the dorsum of the foot, (2) weakness of extensor hallucis longus muscle, and (3) Tinel's sign over the peroneal nerve at the level of the lateral head of gastrocnemius were demonstrated. Plne X-p., laboratory data, lumbar myelogram and electromyogram were almost normal. On February 25, 1974, surgical exploration of the peroneal nerve was performed. It was compressed and stretched by the osseous mass (fabella) through the gastrocnemius in the full extension of the knee, but by the flexion its tension was decreased. The shape and size of removed fabella was normal and its articular surface was yellow and fibrillated.
    On next day of operation no pain was noted. Muscle weakness and hypesthesia disappeared within 3 weeks and 8 weeks after the operation.
    Untill last year 10 cases in Japan and only one case in U. S. A. had be reported. These cases reviewed with the authors case.

    Muscle activities and three dimentional photographic pictures at the same time.
    Each values are converted to digital values through adequate digitazing tools. By measuring photographic pictures, positions and velocities of hand movement are obtained. Electromyographic wave are put into computer through A/D convertor, processed by the method of ordinary digital processing of signals, give the necessary information of muscle activites.
    The relation between the joint position and role and activities of muscles in each stages of hand movement were made clear by spontaneous recording of electromyography and pictures.

  • Electrophysiological and Histological Studies

    In this paper, several kinds of stimulation were given to the dog bone marrow and response was analyzed electrophysiologically. Histological studies of the bone marrow were also carried out.
    Intraosseous pressure stimulation produced response on threshold of 100-130mmHg.
    Intramedullary administration of chemical pain substances produced responses at the following concentrations: potassium chloride, 10-3g/ml; acethylcholine chloride, 10-7g/ml; histamine dihydrochloride, 10-6g/ml; serotonin, 10-6g/ml; bradykinine, 10-8g/ml.
    Ligature of the femoral artery caused reduction of intramedullary temperature, and response was obtained at thermal reduction of 0.8°-1.0°C. Infusion of epirenamine hydrochloride (10-3g/ml) or norepirenamine hydrochloride (10-3g/ml) into the bone marrow induced the thermal fall, which produced response at 0.5-1.2°C.
    The medullary branch of tibial nerves used for leading, comprised unmyelinated and myelinated nerve fibers mixedly in a ratio of about 6.6:1, mostly included in the category of Aδ or C fibers. End structures of nerve fibers in the bone marrow were of the simple branching or non branching type, and lacked specific endings.

    Cliical and histopathologic features were remiewed and analized on twelve patients with bone and joint tuberculosis. The ages of the of the patients ranged from 19 to 73 years with an average of 46 years, Six patients were male and six were female. Sites were were as follows; radius; one, wrist joint; one, finger joint; one, major trochanter two, knee joint; three, tibia and wrist joint; 1, and ankle joint; three. Four petients had been affected from another tuberculous diseases (lung, kidney, lymph node, etc). All cases were finally diagnosed by histologic examination from excised specimens.
    Histologically, marked involution of tuberculous lesion was found in the specimens from the cases with treatment by chemotherapy. There were, however, small numbers of newly formed tubercles here and there in these specimens also.

    Recently, we think thoracic outlet syndrome so noteworthy on it's diagnosis and treatment. In the past five years, there have been 30 cases of thoracic outlet syndrome in own clinic.
    We have some problems on the diagnosis of thoracic outlet syndrome, for example, the objective finding, the compression test of the brachial plexus and subclavian vessels and angiography.
    We report our cases and describe these problems.
