Isolated simple fracture of the coracoid process is seen infrequently. Recently we had a case of the injury. A house wife slipped while carrying a one old child and slipped down a flight of six steps. She arrived at her right shoulder with the child uninjured in her arms. She was suffering pain in the right shoulder. She was treated by internal fixiation and her shoulder with good movement was free from pain. We reported that adequate roentgenograms, including an axillary view, were an essential part of routine shoulder evaluation.

Four cases of facial bone fracture with functional disturbance were treated by means of orthopaedical procedure in our clinic.
Case 1: He had combined type of the Le Fort 2, 3 and vertical type with eye ball rupture. He was treated by intermaxillary wiring, reduction of fracture, application of eye ball prosthesis and repair of scar.
Case 2: He had zygoma fracture and disturbance of articulation without disturbance of occular movement. The reduction of fracture to improve of facial deformity and of disturbance of articulation was perfomed, and good result was obtained.
Case 3, 4: They had blow-out fracture. One of them showed good clinical course by operative procedure, other showed no good result inspite of similar procedure of the former. Although he had re-operation, the limitation of up-ward movement of eye ball was remained.

One hundred and seventy patients of fracture and fracture-dislocation of the dorso-lumbar spine were treated in our hospital during the last twelve years. This report is based on the observation of 83 cases, including 43 cases treated immediately by functional method. All cases were followed by an average of 7.3 years after injury. The effectiveness of treatment was investigated by subjective evaluations of patients and their physical examinations. 60.5 per cent of cases treated by functional method were classified as excellent or good, whereas only 37.5 per cent of cases treated by other methods gained similar results as above. 75.9 per cent of all patients were reinstated in their former position and 62.5 per cent of them were in the position of so called physical labor.

Although the term “desmoid” has been linked with musculo-aponeurotic tumors arising in the anterior abdominal wall. Nichols in 1923, however, was the first to recognize and define extra-abdominal desmoid tumors.
Case report; A two-year-old boy was admitted to our hospital with a tumor in the left upper arm. The tumor was excised: hitological examination showed poorly cellular collagenous fibrous tissue with some muscle fibers. The tumor recurred three times. Each histological examination showed well differentiated fibroblasts with abundant collagen. The etiology, treatment and prognosis were discussed.

This follow up study was carried out on 55 cases of giant cell tumor of bone which had been treating in this hospital from 1953 to 1975. From this study we noticed that the sex, age of onset, size of afflicted area on radiography and grade of pathohistology are not so important factors which influence recurrence.
On the other hand, The type of treatment is the most important factor. Resection of lesion with bonegraft is the best method on the point of recurrence. But after this surgical procedure, the disturbances of joint function frequently remain because the almost giant cell tumors of bone ocurr near the joints.
In this point, Curretage of lesion with bone graft is favorable method for us.

During past four years, twenty one surgical treatments were carried out on thirteen patients with aseptic necrosis of femoral head; drilling & bone nailing 1, intertrochanteric rotational osteotomy 7, cup arthroplasty 1, Moore's hip prosthesis 8, cup & socket arthroplasty 3 and total hip replacement 1.
13 patients who were followed up for more than six months after operation were clinically evaluated according to criteria of coxarthrosis of Jap. Orthop. Ass.
The patients who received intertrochanteric osteotomy showed good results in all cases. This method should be indicated as first choice of operation to early stadium with localized necrotic area. The results of hip endoprosthesis were also excellent for pain, mobility, gait and ADL. This operation was done in cases of moderate or advanced stadium in which the acetabular change was not observed.
Cup & socket arthroplasty and total hip replacement appear to be indicated to the case of severe stadium which shows narrowing of joint space and acetabular arthrotic change.

3 cases out of 50 cases began to notice pain on the operated hip one year after operation. It was confirmed that the pain was caused by the wear of the artificial femoral head made with high-density polyethylen by the revision operation. The total hip replacements were performed for the second surgery.
Since then, we have been using the new type of artificial femoral head, and the surgery was performed to 16 cases of the fracture of the femoral neck.

After this perfusion, continual suction of hematoma was performed for about 2 days.
Antibiotic concentration in the blood sucked out from the operated wound was very high at least five hours after the end of the perfusion.
So, it is considered that the bacteriostatic or bacteriocydal effect of this procedure is remarkable.

Excellent results are reported of 7 cases (8 hips) treated with shelf operation and of 4 cases (4 hips) treated with Chiari' pelvic osteotomy.
Chiari' operation is superior to shelf operation in the following reasons;
1) wideness and better stability of the new roof.
2) rapidity of postoperative physiotherapy; patient can begin weight-bearing ambulation in 3 weeks after operation.
Chiari' operation has a good indication in the following cases;
1) severe dysplasia
2) gliding subluxation
3) dysplasia with severe osteoarthritis.
Shelf operation has also a good indication in dysplasia without subluxation and in bilateral dysplasia.

Most of them suffered from secondary osteoarthritis due to CDH. This is to report the follow-up study of 46 patients who were able to be assessed directly (30 patients) or by mail (16 patients). There are 7 males (7 hips) and 39 females (43 hips) and average length of follow-up time is 5 years and 8 months.
11 cases (85%) out of 15 patients on which osteotomy had been done improved excellently. There is no difference in the clinical results between over and under 5 years after operation.
Shelf operation and shelf operation with osteotomy were performed in several cases and all these cases showed satisfactory results.
Arthrodesis and muscle release operation were better to remove pain but some-cases were unsatisfactory in ADL.

For the past twenty years, in our clinic acetabuloplasty and femoral osteotomy were the most common procedures for the treatment of osteoarthritis of the hip. But recently operation combined of the above two is being carried out most frequently. The cases treated by these three methods were followed up and some problems on indication and operative techniques were discussed in this paper.
It is concluded that for the cases having acetabular dysplasia and deformity of the femoral neck, the above combined operation is necessary. In the technical viewpoint, it is important to perform accurately designed osteotomy of the femur and not to make too solid bony shelf in order to avoid too strong pressure upon the femoral head.

Herbert Mayo, in 1833, was the first to describe the condition now known as Pilonidal cyst. The term “pilonidal” derived from the Latin, meaning hair, nest, was first applied to sinuses in the sacral region by Hodges in 1880.
There are many case-reports of pilonidal cyst in Europe and America, however the report is rare in Japan.
We have experienced two cases of pilonidal cyst, and have found the hair looking like the hair of the head.

Anterior sacral meningocele is very rare congenital anomaly, besonders in our country there is no case report.
We experienced this anomaly by chance. A man aged fifty nine years attended the urological department with a history of haematuria.
Roentgenography showed that the sacrum was to be deformed and hypoplastic. Rectal examination showed that a large smooth rounded mass was lying behind the rectum. The tone of sphincter was found to be normal and there was no hypaesthesia of saddle area and lower extremities. We diagnosed anterior sacral meningocele by myelography. He was operated because of renal cell carcinoma of the left kidney. After nephrectomy, he died due to massive necrosis of intestine.

Impules and frequency responses of the antero-posterior and lateral sways in human standing posture were obtained by applying two dimensional autoregressive processes to the both sways.
The impulse response of antero-posterior direction due to an impulse-like lateral sway and that of lateral direction due to an impulse-like antero-posterior sway depicted an oscillatory time-pattern with a decay in its amplitude. The former, however, was far more prominent than the latter in its amplitude and duration. A dominant peak around 7-10Hz was observed in the frequency response (frequency pattern of the impulse response) of the former, whereas the peak was very minute in the latter.

We report our experience with 9 patients in which spinal deformities were treated by means of halo-pelvic traction and Harrington instrumentation, There were 4 patients with kyphosis due to neurofibromatosis, Klippel-Feil's disease with spondilitis tuberculosa, Morquio's disease and laminectomy of the cervical region, 4 patients with idiopathic scoliosis and a patient with dislocation of the atlanto-axial joint and os odonteudium.
In the kyphotic curve, it is necessary for the relief of myelopathy that spinal osteotomy and subvertrebrectomy for decompression of the spinal cord have been made at first. After maximal reduction of the deformity by halo traction anterior strut bone graft and posterior spinal fusion was performed. In severe and rigid scoliotic curvature wedge osteotomy was made in several segments at the apex of the curve.

Perdurography is considered to be the best way for the measurement of the thickening of the ligamentum flavum in the cervical region. 119 cases of peridurography were observed for this purpose. The more the disc space is narrow and or posterior spur is large in plain X-ray of cervical spine, the thicker the ligamentum flavum is in peridurography. At the extension position of cervical spine, the inward bulgings of ligamentum flavum are strikingly observed, at the neutral position they are mild but they are significantly reduced at the flexion position.

Ventral horn and dorsal horn symptomes of cervical myelopathy were studied using 19 selected cases out of the operated cervical osteochondrosis. Those cases were 8 of impared filling of root pouch and 11 of myelopathy showing single intervertebral block in myelogram. There are no muscle atrophies, clumsiness of fine finger movement and gauntlet sensory changes in the cases of impared filling of root pouch. Muscl atrophies and clumsiness of fine finger movement seen in myelopathy result from ventral horn cell lesion, and sensory changes are considered to be a lesion of dorsal horn or fasciculus cuneatus. Also one case of myelopathy type I (our classification) will be demonstrated.

This is to report a comparative study between water soluble contrast medium (Dimer X) and oily contrast medium (Myodil) in 34 cases of lumbar discogenic diseases.
1) Dimer-X was superior in 10 cases and inferior in 3 cases compared to Myodil.
2) There were 2 cases of false negative findings in Dimer-X and 3 cases in Myodil.
3) No changes in cerebrospinal fluid was found after each myelography except some increase of the number of cells.
4) Slight complications occurred by both Dimer-X and Myodil. Dimer-X had less root symptomes but no difference was observed in the occurrences of irritation signs of meninges such as headache, fever and nausea. Any other sever complications were not found.
5) Dimer-X is considered to be the first choice. If any doubt, frequent myelography by Dimer-X or Myodil will be necessary.
