After the intensive care, the number of children who need medical cares and equipments have been increasing. These children have various difficulties to establish linkages to primary health services, specialty health services, and related services, e.g., nurture, education, welfare and social services. For these children, hospitals and facilities have been the only places in their lives. Regarding the use of medical services, attention has been focused on the home care medicine, which has been mainly targeted at the elderly. In the near future, it is expected that these children will also benefit from a community-based integrated care system and a community based inclusive society.
