There are three basic columns of procedure in Oriental medicine.
1) To reveal the meridian phenomenon clearly, 2) To ev aluate the results of Oriental medicine objectively, 3) To find the most suitable methods to treate difficult cases by Oriental medicine.
In Japan, the Welfare Ministry selects some difficult cases (over 50 diseases) as Specified Diseases and national funds are provided by the Welfare Ministry for the study and research of these diseases. Also patients who suffer from these difficult diseases will be helped economically for their treatment.
In 1974, our hospital changed its name from the First National Hospital in Tokyo to the National Medical Center Hospital mainly because it has its own institute to study clinical problems from the view points of basic science and also it can study the mechanism and pathoohysiology treatment methods of these difficult cases.
According to this plan, we started to study the treatment of these difficult cases by Oriental medicine 5 years ago and nearly 120 cases were treated already in my clinic. The general rules for treatment are as follows:
1) The diagnosis and results of the treatment a r e confirmed by specialists,
2) The patients may be treated basically as in-patients and the r esults will be studied after one course of treatment which is usually three months, and
3) The treatment consists of general treatment, local treatme nt and specific treatment according to the rules of Oriental medicine and also Western medicine is used together with Oriental methods, if necessary.
We examined the results of the treatment in my clinic by questionaire and 587 answers were received from 1314 patients. Among them 38 belonged to the difficult cases. Among these-difficult cases at the end of one course of our treatment, no one indicated very good results; that is to say nearly cured (A group). However 8% answered 'A' after some treatment in other hospitals.
The remarkably improve d group (B group) was 11% when they left my hospital, and 18% after further treatment. The kinds of treatment after leaving my hospital were acupuncture, 28%; medicament therapy, 28% (including 2 % by herb medicine); and no treatment, 38%.63% of the patients hoped to continue acupuncture treatment even after leaving my hospital. From these facts, it can be said that acupuncture and Orie ntal medicine is useful for the treatment of difficult cases especially to decrease the complaints caused by the disturbances of the autonomic nerves.
