Here we report the first case transported urgently from islands to our hospital by rooftop heliport at night. An 82-year-old woman suffered from fever was already suspected pyogenic liver abscess by CT images and treated with intravenous antibiotics. She was combined with heart failure and her general state took a sharp turn for the worse. Emergency transportation was performed by helicopter and she was admitted to coronary care unit at first. After her cardiac state got stable, ultrasonography-guided percutaneous drainage was performed and her clinical condition gradually improved. Nine weeks after onset, she was discharged without sequelae.
