Anthracycline (AC) induced cardiotoxicity is a dose limiting factor in anti-cancer chemotherapy including AC.It has been reported that AC induced cardiotoxicity increases when the cumulative dose exceeds 500 mg/m² (calculated as doxorubicin).In view of this,we investigated accumulation rates in 59 patients with hematological malignancies,and estimated the time taken for it to reach 500 mg/m² for the 3 major regimens of high-CHOP,CHOP and VAD,to Compare the differences between predicted values calculated from the initial dose of AC for each regimen.The relationship between the cumulative dose of AC and cardiotoxicity for several chemotherapies including AC was also investigated.The accumulation rate of AC was 1.37 (0.17-3.14)mg/m²/day and the cumulative dose of 500 mg/m2 would be reached 170,231 and 495 days after starting high-CHOP,CHOP and VAD,respectively,which was 10-27% longer than the times predicted on the basis of the initial dose of AC for each regimen.A positive correlation was found between electrocardiographic QTc and cumulative dose of AC (r=0.448,p<0.05).As two of five patients in whom the cumulative dose reached 500 mg/m² of AC developed cardiomyopathy,AC cardiotoxicity should be intensively monitored 170-495 days after starting the chemotherapies such as high-CHOP,CHOP and VAD,when the cumulative dose is approaching 500 mg/m².

実験用ネコの飼育室内で, ネコとともに飼育されていた生後4カ月の子イヌ1頭が, 1979年11月に突然嘔吐と血様下痢および著明な白血球減少をきたして.発症後3日の経過で死亡した.病理組織学的には, 小腸から大腸に及ぶ広範な粘膜上皮の壊死および剥離と出血が著しく, また骨髄の細胞数の激減と脾臓の炉胞の萎縮が顕著であった.いっぽう, 腎臓からウイルスが分離された.本ウイルスはネコ肺線維芽細胞に大型の核内封入体を形成し, DNA型で, 熱およびエーテルに耐性であり, 4℃ で豚赤血球を凝集した.電顕観察では直径約20nmの球形粒子であった.またネコ汎白血球減少症ウイルスとの間に血清中和試験で交差が認められた.以上の所見から本ウイルスはパルボウイルスと同定された.同室内で飼育されていた他のイヌおよびネコは, 本ウイルスに対し, いずれも高いHI抗体価を示した.健康なイヌおよびネコへの接種では, いずれも軽度の白血球減少のみが認められたに過ぎなかった.

Pirarubicin (THP) を含む化学療法を施行した非ホジキンリンパ腫患者に対し, 心プールシンチグラフィー, ¹²³I-metaiodobenzylguanidine (MIBG) 心筋 single photon emission computed tomography (SPECT), ¹²³I-beta-metyl iodophenyl pentadecanoic acid (BMIPP) 心筋SPECT, ホルター心電図を施行し, THPの心毒性の早期検出を行った. 対象はTHP-COPBLM療法を施行した未治療非ホジキンリンパ腫26例である. THP-COPBLM療法でTHPは40mg/m²を21日毎に投与した. THPの総投与量は平均240mg/m² (40～400mg/m²) であった. 1) MIBGより得られた washout rate (WR) はTHP総投与量と相関を認め, 心交感神経障害の指標となると考えられた. 2) 左室駆出率 (LVEF) は, THP総投与量と負の相関を認め, 360mg/m²で4例中2例に60%以下に低下した症例を認めた. 3) THP総投与量はBMIPPより得られた extent score, severity score と正の相関を認めた. 4) WRは, 心室性期外収縮の頻度と正の相関を認めた.
THPは動物実験では doxorubicin (DXR) に比し心毒性が少ないと思われているが, 今回の高齢者の検討ではTHP総投与量約360mg/m²で心毒性が出現する. よって, DXRと同様に左心機能の低下が認められるような重篤な心筋障害が起こらないうちに早期検出することが大切であると考えられた.

2週齢の哺育牛の背部を中心に体表各所に紫斑形成・出血が多発したため, 鑑定殺された.皮膚には針尖大の出血を中心に紫斑が形成され, 内部諸臓器にも, 漿膜面を中心に出血斑が汎発していた.血液検査では, 血餅の退縮不全と, 塗抹像での血小板数の減少が顕著にみられた.病理組織学的には, 出血は皮下織, 消化管粘膜下織から漿膜面におよび, 骨髄では巨核球の軽度の増数を認めたが, 遊離の血小板はほとんど認められなかった.
発症牛の血漿を健康牛からの血球浮遊液と混合した場合, あるいは発症牛の血清をウサギに静脈内注射した場合には, いずれにおいても遊離血小板数の減少が見られた.とくに, 前者では血小板の凝集が認められ, 血漿中に血小板凝集因子の存在が示唆された.
以上の所見から, 本症例は血小板減少性紫斑病と診断された.

This paper presents decision method of policy reusing ratio of transfer learning in reinforcement learning based on gradient descent method. In recent years, learning robot system has been discussed for the actual applications. To reduce the learning time, transfer learning framework is proposed and the method is knowledge reusing mechanism. In particular, effectiveness of transfer evaluation method such as a transfer surface is proposed in reinforcement learning and adjustment method of transferring ratio is also proposed. However, decision of value of transferring ratio is depends on human intuition and experience. In this paper, automatic transferring ratio estimation method is proposed based on gradient descent method with random initial value and statistics in multiple estimation trials, further evaluation of proposed method in two different transfer surface.

For the realization of a wearable artificial kidney, the filtrationadsorption method with continuous low filtrate flow is suitable. For a filter for this use, a plasma separator is advantageous as regards the miniaturization of the filtration equipment. A parallel flow type plasma separator was designed based upon the relationship between filtrate flux and shear rate. To miniaturize the device, the blood cistern was laminated and in vivo experiments with dogs were performed. Filtrate flow rate was lower than the value of the design specification. From this result, some problems to be considered in more practical design were examined.

子宮の摘出が黄体の寿命を延長させることは,60年前に初めて報告されて以来,種々の動物で検討されて来た。しかしヤギにおいては,妊娠黄体に及ぼすその影響が検討されているに過ぎない。今回著者らは,排卵後間もないシバヤギに子宮摘出手術を施して,その後の末梢血中P濃度を測定することにより,この処置が性周期中の黄体に及ぼす影響を検討した。
シバヤギは表1に示す3頭を用い,キシラジン,フローセン麻酔下で図1~3に示す方法で子宮摘出術を施し,術後約50日にわたって末梢血中P濃度をRIAにて測定した。その結果,手術時に出血体あるいは黄体の存在を確認した2頭においては,図4,5に示す通り,少なくとも50日の長期間にわたって,P濃度が0.7ng/ml~3.9ng/mlと日によって大きく変動はするものの,P分泌は持続していることが認められ,その間に発情の回帰は認められなかった。このことからシバヤギにおいても他の家畜と同様に,黄体退行因子の産生場所は子宮である可能性が示唆された。一方子宮摘出時に黄体が存在しなかった1頭においては,図6に示す通り処置後46日間P濃度は0.5ng/ml前後の低値を維持し,黄体のないことと一致した。

犬の自然発症のDICに近い病態をつくることを目的として, 実験犬にエソドトキシンの長時間点滴静注を行った. そして次のような血液凝固学的ならびに病理学的検査結果を得た.
1) 血小板数はエンドトキシソ注射開始後に急激な減少を示し, その後も低値を維持した.
2) フィブリノーゲン量はエンドトキシン注射で減少し, 点滴開始後6～7時間で投与前値の約半分に減少した.
3) プロトロンビン時間および活性部分トロンポプラスチン時間はいずれも延長した.
4) フィブリン分解産物は犬の15頭すべてが注射開始2時間後に上昇し, そのうち13頭は40μg/ml以上を示した.
5) 病理学的検査では, 腸管全体に著明な出血がみられ, 肝臓, 脾臓, 腎臓のうっ血が認められた.組織学的には, 肝細胞の変性, 出血, および類洞内のフィブリン様物質が認められた.
以上の結果から, 犬のこれらの所見はヒトのDICの診断基準によく合致し, また犬の自然発症に近いDICモデルであろうと考える.

A filtration-adsorption method with continuous low filtrate flow has been found suitable for the realization of a wearable artificial kidney. This artificial kidney system operates in two stages, a filtration stage and a filtrate treatment stage. (Fig. 1)
In the filtration stage, a plasma separator appears to be superior to an ultrafilter from the viewpoint of miniaturization. In the filtrate treatment stage, this system consists of ultrafilter, immobilized urease, cation exchanger, activated charcoal and electrodialyzer.
Each component necessary for the constraction of the total system was manufactured and its fundamental properties were examined. Subsequently, the total system constructed from these components was tested in vitro to verify its operational capability.

Renal failure induced by cyclosporin A(CyA) in hematopoietic stem cell transplantation recipients is frequently observed as a major side effect. However, it is difficult to compare the frequency or severity among the reports because of differences in the method of administering CyA. In addition, there is little analysis of risk factors influencing the development of renal failure. Therefore, in this study, we tried to analyze risk factors influencing the development of renal failure induced by CyA in hematopoietic stem cell transplantation recipients at Chiba University Hospital using patient records. Among 50 patients in our study, 26 (52%) developed renal failure (grade 2 <), and serum creatinine reached maximum 32.7 days after the initiation of CyA administration on average. The mean blood CyA concentration was not correlated with the variance of serum creatinine (r = 0.12). In univariate analysis, the frequency of renal failure was significantly higher in females than that in males (P = 0.02). In addition, female and myeloablative conditioning were shown to be significant risk factors by multivariate analysis. These findings suggest that it is necessary to observe changes in clinical examination data such as BUN, uric acid, and serum creatinine more carefully because the risk of renal failure increases in patients with such risk factors.

This paper describes an automatic transfer rate adjustment method for transfer learning in reinforcement learning. Recently, learning robot systems such as reinforcement learning and transfer learning have been discussed. However, learning robots using transfer learning have a state of over-fitting that cannot behave correctly in an environment different from the environment in which they have been trained. The transfer rate and the transfer surface are proposed to avoid this over-fitting. The optimal transfer rate can be automatically estimated by using the transfer surface and the optimal transfer rate estimation method, however there is a problem that it is not possible to distinguish between two states, “Without solution” and “Solution with a width”. In this paper, we aim to prevent over-fitting by using a transfer surface, estimate the effective transfer rate in consideration of the problems in existing research, and discuss a method for automatically adjusting the transfer rate.

本研究の目的は、企業と消費者の円滑な環境コミュニケーションを実現させるために、送り手が作成する環境広告の傾向と、受け手が選好する環境広告の傾向を探り、両者の比較から環境コミュニケーションツールとしての環境広告のあり方を提案することである。まずは、「環境広告コンクール」応募作品および審査通過の現状分析から環境広告の送り手の意識の現状を探った。送り手が作成する環境広告の傾向と、審査員の評価との差異について指摘した、次に、架空の環境広告を用いたコンジョイント分析を用いて、環境広告の受け手の選好について探った結果、環境広告の選好は人によって違いがみられ、選好にはその人の環境意識が影響を与えているということが明らかになった。それらの結果から、環境広告の設計において、[1]環境意識の違いによる選好にあわせた環境広告、[2]実績データを提示する環境広告、[3]革新と適応がバランスした環境広告、について提案した。
