Metastatic Seeding along Percutaneous Transhepatic Biliary Drainage Tracts which Occured 4 Years and 9 Months after Surgery

We report a case of metastatic seeding along the catheter tract after percutaneous transhepatic biliary drainage (PTBD). A 77-year-old Japanese woman was admitted due to the presence of a hard mass in the right lateral abdominal wall. The patient had a past medical history significant for bile duct carcinoma which was treated by a radical pancreatoduodenectomy with preoperative PTBD 4 years and 9 months before. On physical examination, the hard mass was located on the right frank at the entry site of the previous PTBD. She underwent a exicision of the tumor and a fistulectomy. A histologic examination revealed adenocarcinoma which was compatible with metastasis from the primary bile duct tumor. A review of the world literature found 21 other patients with such complication. In a review of these patients, all nineteen which described detail were underwent the manipulation of the tumor by inserting the catheter through the tumor or biopsy of the cancer. Their postrecurrence 1-and 3-year survival rates were 40 and 32%.In conclusion, metastatic seeding along the catheter tract is an unusual complication, but almost all such cases result in a poor prognosis. For prevention such complications, the frequent exchange of catheter or insertion through the tumor should be avoided in such patients in whom a possible curative resection is considered.
