The purpose of this study was to analyze clinical outcomes and postoperative radiographic findings in patients treated with total shoulder arthroplasty (TSA) for glenohumeral osteoarthritis of the shoulder (OA). 14 patients treated with TSA for OA, whose average age was 71.4 years old, were the subjects of this study. Rheumatoid arthritis, avascular necrosis of the humeral head and cuff tear arthropathy were excluded in this study. Primary OA was observed in 11 patients and secondary OA in 3 patients. Humeral stems and glenoid components were fixed with bone cement in all of the patients. The average follow-up period was 32.5 months. Clinical outcomes were assessed using the JOA shoulder score, and radiographic findings such as radiolucent line were evaluated using the postoperative X-rays. The average JOA shoulder score in the 14 patients improved from 52.3 points preoperatively to 88.3 points postoperatively. Although the 11 patients with primary OA obtained 92.3 points of the average JOA shoulder score and satisfactory outcomes, the 3 patients with secondary OA obtained 73.7 points of the average JOA shoulder score and unsatisfactory outcomes postoperatively. Radiolucent line around the humeral stem was observed in one patient, and that around the glenoid component in 2 patients. Complications such as infection, fracture or neurovascular injuries were not observed at final follow-up. This study suggests that TSA for OA is a reliable procedure to obtain satisfactory outcomes. However, TSA may provide poor functional outcomes in some patients with secondary OA. Therefore, indication of TSA should be deliberated in patients with secondary OA.
