Hypothesis: Massive rotator cuff tear often occurs at the upper side of the humeral head, and studies have been done on this. However, the number of reports of humeral head anterior and posterior translation is low. The purpose of this study was to evaluate the anterior and posterior translation in the shoulder joint at internal and external rotation in massive rotator cuff tear.
Methods: We divided 13 shoulders in 13 patients, who underwent Modified Debeyre-Patte procedure for massive rotator cuff tear, into 3 groups: 1) supraspinatus and superior half of the subscapularis tear (SSC group); 3 shoulders, 2) supraspinatus tear(SSP group); 7 shoulders, and 3) supraspinatus and superior half of the infraspinatus tear (ISP group); 3 shoulders. We investigated the relationship of the tendon tear size and the deviation of the humeral head.
Results: SSC group led to significant anterior translation (7.9 ± 3.5 mm) in internal rotation. ISP group led to significant posterior translation (-4.9 ± 3.2 mm) in external rotation.
Conclusion: With regard to the torn tendon size and the deviation of humeral head, this report was revealed that subscpularis tear reinforces the unstable anterior side and infraspinatus tear reinforces the unstable anterior side of humeral head.
