A 81-year-old woman complaining of epigastric pain was admitted to our hospital with diagnosis of severe acute pancreatitis. Esophagogastroduodenoscopy was performed for the further examination because of recurrence of pancreatitis, and it revealed a lobular protruding lesion in the duodenal papilla. Histopathological examination performed by biopsy revealed adenocarcinoma. As a result of consultation with surgeons, we decided to perform the curative surgery after inflammation improved. We placed the stent in the main pancreatic duct for the purpose of preventing the recurrence of the pancreatitis. Subtotal Stomach-Preserving Pancreaticoduodenectomy (SSPPD) was performed in 6.5 months after the onset of pancreatitis.
Papillary lesions are relatively rare as a cause of pancreatitis, but it seems to be necessary to keep in mind, especially when repeating pancreatitis. The pancreatic duct stent is considered to be an effective device for preventing recurrence of pancreatitis during the waiting period until curative surgery.
