Abstract
This paper discusses a method for preventing serious failures, which is based on analyzing incidents and related to the CAR process of the CMMI. Serious failures occur in the systems being actually used by users. Therefore, the potential causes of serious failures are hidden not only in the planning and development stages of the system, but also in the whole activity of its operation and maintenance. Moreover, the progress of information system technology and the development of organizational maturity are changing them continuously. To cope with this problem, we instituted a serious failure prevention method to our whole organization as an SI vender. We utilized "Why Why Analysis" to collect information relevant to the prevention of serious failures from incident information. Here, we attached a great deal of importance to collecting the information that can be widely utilized throughout our organization, in addition to the recurrence prevention of each event, while developing our method of analyzing incidents. Through these activities, we found that incidents can be utilized for prevention, because the causes occurred in clear patterns.
