A survey on nutritional status of vitamin B_1 in adults of two villages with insufficient medical care in Shimane Prefecture was carried out in 1978, 1979 and 1980. The results of the survey were as follows: (1) Thiamin level and transketolase activity in blood were very low and the thiamin pyrophosphate effect was high. (2) Urinary thiamin excretion (24 hours urine and urine of a single excretion) was also very low. Urinary thiamin contents were good correlation with γ=0.84 between 24 hours urine and urine of a single excretion. (3) The thiamin intake analyzed with their foods was, on the average, 0.53 mg/day and that calculated from hearing nutritional survey was 0.69 mg/day. In these areas, thiamin intake seems to be insufficient. (4) In a consequence of intake of thiamin fortified rice (1.5 mg/day), blood thiamin level, transketolase activity and the urinary thiamin excretion were increased, and pyrophosphate effect was decreased. An improvement of vitamin B_1 nutritional status was observed.
