    First, I would like to express my deepest gratitude for the heartfelt messages to organizers, editors and readers on behalf of the International Biophilia Rehabilitation Academy. It is my great pleasure to publish the new journal BIOPHILIA.

    The journal BIOPHILIA is an academic journal of the International Biophilia Rehabilitation Academy as a merger journal of the proceedings of the International Biophilia Rehabilitation Conference and the refereed original papers, reviews and reports. The publication aims at establishing the sustainable super-aged society by restructuring the rehabilitation medicine. The other purpose is to divide the previous Biophilia Rehabilitation Journal into two, one in Japanese and the other in English. It has been published with mixing the Japanese and English papers until now by the Biophilia Rehabilitation Academy of Japan. It now will be possible for our worldwide readers who are reading and/or investigating our journal not to become disappointed by finding an article they are interested in reading is only written in Japanese.

    We welcome and seek original articles, which are useful for furthering the improvement in welfare of human beings who have gained longevity, are aiming to establish a sustainable, super-aged society and are helping to restructure rehabilitation medicine. Positive participation by one and all having interest in this field is welcomed. The types of articles being sought are illustrated on the following page.

    Holding a workshop in your country is another way to participate and has been received with very positive results in every country held thus far. The photographs on the following page are from some of the workshops we’ve held. Moreover, the proceedings of a workshop will be also published by this journal. Now we all rejoice together for publication of the journal BIOPHILIA enabling fulfillment of our shared purpose.

    The journal BIOPHILIA is a new scientific journal, which is about to be fully lunched by the International Biophilia Rehabilitation Academy, IBRA in short. The journal’s credo may be described by “A healthier future through research and innovation“. It is designed as a medium for review articles, original papers, and proceedings in the broad field of rehabilitation medicine. We will aim to highlight the most relevant issues in the diagnosis, treatment and management of the whole spectrum of disabilities related to diseases as well as to those developing due to the old age in the process of ‘normal’ physiological aging. The aim will be achieved by presenting the latest developments in basic and clinical research and in technical progress concerning the rehabilitative aids.

    It is my great pleasure to extend the preface of this new journal “BIOPHILIA” on behalf of Okayama University.
    I am to learn that the journal “BIOPHILIA” has the purpose which is the English publishing to provide the doctors and researchers all over the world with the newest knowledge of the aging society sustainable.

    In Japan it is always very difficult to act with turning one's eyes to the world because Japan is surrounded by the sea all around and our lives complete enough at home. It has not been necessary to meet the strangers within the boundary since the long historic isolation in Edo area. But I am intended to open my university to the world, to cope with another member sophisticatedly to understand each other and to send the result of creative knowledge to the world to interchange the eminent scholars and students.

    Now, there is the experience that I participated in the conference named “IBRC 2009, held in Cuba by the International Biophilia Rehabilitation Academy. It was very meaningful for us to exchange and introduce the experiences within the field in Japan and Cuba mutually through the presentations of the research and opinions about the functional recovery of elderly disabled persons.

    It was very substantial contents as an international activity of the academy which had the headquarters in Japan.

    It is not necessary to say the importance of the refereed paper. And it is expected to open the consciousness of the Japanese people to the world widely through offering the lecture proceedings to the researcher in the world not only through WEB but also through the national search engine “CiNii” and else that the new journal permitted to do.I wish the new journal and the International Biophilia Rehabilitation Academy much more successful years on first publication.

    I herein assert my satisfaction regarding the issue of the new journal "BIOPHILIA". Considering, on one hand, the fact that I have exactly the two medical specialties - i.e. Physical & Rehabilitation Medicine and respectively, Gerontology & Geriatrics - on which relies the focus the of the International Biophilia Rehabilitation Academy (IBRA) scientific endeavors - and on the other hand, my academic position, obviously I can not but to be happy about the born of a new related scientific magazine.

    The word itself suggests the true nature of this organization: BIOPHILIA: the love of nature and all living things. In accordance with this name, this organization activates to improve the life of the handicapped people in Japan and around the world, giving them hope and a real contribution to improving their quality of life. The journal "BIOPHILIA" is an academic journal of the International Biophilia Rehabilitation Academy that is aiming at the restructuring rehabilitation medicine.

    It is a great pleasure and honor to invite you to participate in the 10th International Biophilia Rehabilitation Conference (10th IBRC), which will be held in Chieti, Italy, on October 15, 2013. I, Toshiyuki Tanaka, of the Faculty of Science and Technology at Keio University will chair this conference. The 10th IBRC will be held in collaboration with Keio University, a long-established private university, which was founded by Yukichi Fukuzawa in Japan in 1858. Today, Keio University is held in high report in Japan. The University's involvement in the Conference highlights the importance of holding conference.

    You are invited by the founder and director to realize "the revolution of hope" with people all over the world in order to build up a sustainable, super-aged society by overcoming much difficulty named “the aging crisis.” It is important to clarify the present problems, to show the way to solve it by encouraging and introducing our research, and to promote the enforcement, both domestic and abroad on the basis of the facts. It is necessary to establish the new civilization not to assume the increase in elderly person a negative factor, meaning the integration of the elderly population toward the productive society. This invitation describes this endeavor in the pursuit of "the revolution of hope" in the hopes of forming ranks with you.

    The social security system was originally established in the feudal period of Tokugawa for helping low-income groups, followed by the pension system for the defense groups prepared by the Meiji government. This system was developed for the government officer and later for the private sector in the 1960s. During the early stage of the modern period, thanks to rapid economic growth, no financial problem was found fortunately. This situation has completely reversed recently due to several reasons; stagnation in economic growth, growth of elder groups, lack of proper financial strategy, organizational problems in the enforcement of the social system, and uneasiness among the nation. This paper was prepared to show a brief outline of the recent history of the social system in Japan and proposes some alternatives to be taken by the government.

    Motivative exercise was introduced into the Geriatric Health Services Facilities and the evaluation of both motivative exercise and passive exercise were compared by functional Near-InfraRed Spectroscopy (fNIRS) in 11 subjects in March, 2010. Moreover, the randomized controlled trial (RCT) in two groups: 13 subjects with motivative exercise and 15 subjects without it for three months. In the brain fNIRS, motivative exercise for both knee extension and plantar flexion showed a significantly more activation than passive exercise (p<0.05). Though the effect on the lower limb ROM was seen in two cases, there was not any significant difference between the motivative exercises introduced and not introduced groups by RCT showing improvement within muscular force and contracture, lower limb ROM, MMT, Barthel Index, Hasegawa screening test and the Brunnstrom stage.

    In the chronic stages of aging or illness, less staffs are employed in both medical and care faculties in Japan than acute stages. This situation might have the serious risk of many patients returning to bedridden states easily. To solve this problem, we have recognized "The Motivative Exercise" as being very useful. In Motivative Exercise, we use simple tools instead of therapists' hands to have them exercise their legs by themselves. We could prove the effectiveness of this rehabilitation method by f MRI to observe brain activities.

    An objective rating of walk ability is desired for efficient rehabilitation enforcement. We developed a measuring device in which reciprocating movement of cross direction and rocking movement based on the heel parts as a pivot are available based on US patent. Experimental trials using the device for the measurement of muscle function were done in volunteers. In a comparative study of their walk ability in normal daily living, it was confirmed that the angular speed in rocking movement had a close relation with the gait evaluation. It is expected to judge the walking ability automatically with using the device developed in the future by the study of a larger cohort of subjects.
