To evaluate the effects and problems by carrying out the tentative plan of the Discharge Support Action Program in the joint workshop of nurse, rehabilitation and welfare.
Method:The Discharge Support Action Program has been carried out twice. The program was regarded as a series of a session: a lesson, care study and reflection, and a debrief meeting. The analyzed data was about “achievement level of the workshop goal”, “learning of the seminar”, “self-evaluation acquiring essential abilities toward discharge support”. It was self-evaluated by participants of the discharge support seminar engaging in nurse, rehabilitation and welfare.
Result:In the “achievement level of workshop goal”, the participants answered five items as positive. The “learning of the seminar” was classified as all categories such as basics of discharge support and 14 categories like “the sustained life and seamless care”. Also, in the “self-evaluation of acquiring essential abilities toward discharge support”, the positive answers “fairly true” were shown in sub-entries such as “understanding family assessment method in discharge support”, before and after the workshop.
Discussion:Regardless of occupation, the result showed that it is significant to respect the subjects, support families, and cooperate with workers from various occupations facing each other. The problem to be solved is to refine the contents and methods.
