    Purpose : We tried to make an ECMO circuit with a bio-pump for newborn respiratory failure for the purpose of preventing the hemorrhagic tendency during low perfusion ECMO. Methods : We used ten puppies whose weights ranged from 3 to 5 kg. General anesthesia was maintained with hypoventilation and V-V ECMO was started. Systemic heparinization was also maintained by monitoring activated coagulation time (ACT : 200-250 s) during whole procedure. Five puppies underwent low perfusion ECMO (50 ml/kg/min) with the bio-pump (group BP), the other five with a roller-pump (group RP), for eleven hours to examine the effect on O_2 transfer, CO_2 removal, hemolysis, consumption of platelets and fibrinogen, liver function, and renal function. Results : There were no significant differences of these parameters between groups BP and RP. Conclusions : It was considered that there was no merit so far to use bio-pumps for newborn ECMO circuit. Clinical application of the ECMO with bio-pump may not be warranted yet.

    Purpose : The purpose of this study was to determine the effect of total parenteral nutrition (TPN) and erental nutrition (EN) on the septic status of the rat by measuring the serum endotoxin, β-_D-glucane and cytokines and analyzing histological changes of the liver and the lung. Methods : Sprague-Dawley male rats, each weighing about 200 g, were randomly divided into four groups. Group I (control group) was fed a rat chow diet for 5 days. In group II (PO+LPS group), rats were fed a rat chow diet for 5 days, then received an intraperitoneal injection of endotoxin (Escherichia coli LPS) 2 mg/kg body weight. In group III (TPN+LPS group), rats received a solution of 20% glucose, 3.3% amino acids and 0.5% fat intravenously as TPN for 5 days followed by intraperitoneal LPS injection. In group IV (EN+LPS group), rats received same TPN solution through the gastrostomy for 5 days followed by intraperitoneal LPS injection. Rats were killed 3 hours after injections of LPS. We measured serum levels of endotoxin, β-_D-glucane and cytokines (TNF-α, IL-1β, IL-6, IL-8, IFN-γ). Pathological change in the liver and the lung were evaluated with HE-staining and immunohistochemical staining. Results : In septic groups (group II, III and IV), serum endotoxin levels were increased. However, β-_D-glucane level was increased only in group III. The serum cytokines levels were increased in all septic groups. In group III, the liver showed remarkable dilatation of the sinusoid, ballooning of the hepatocyte and bleeding. The lung showed edema and bleeding. Conclusions : These results suggest that the TPN+LPS group might develop multiple organ failure (MOF) in septic status. On the other hand, the EN+LPS group showed minimal change in the liver and the lung, which suggested that EN might suppress the development of MOF in the septic status.

    Purpose : To evaluate the most standard inguinal herniorrhaphy based on the experience in a single institute. Methods : Inguinal herniorrhaphy in infants and children was carried out in 8148 patients at our department between May 1964 and December 1999. Results : Of these, 2600 cases were operated on by Ferguson's technique, 3865 cases by Lucas-Championniere's technique and 1671 cases by Mitchell Banks' technique, decade by decade. Twelve cases were operated on by other techniques. Recurrence occurred in 1 by Ferguson's, 5 by Lucas-Championniere's and 3 by Mitchell Banks' technique. Each recurrence rate was under 0.2%. There was no significant difference in recurrence rate among these procedures. The causes of recurrence were considered as the destruction of the neck of the sac while dissecting the hernia sac, or inadequate ligation of the neck. The necessary operating time for each procedure group was evaluated for a random sampling of 100 cases (50 males and 50 females) who were operated on by the author. Mean time was as follows : in Ferguson's group, 23.6 minutes in males and 16.4 minutes in females ; in Lucas-Championniere's group, 20.8 minutes in males and 14.5 minutes in females ; and in Mitchell Banks group, 11.5 minutes in males and 8.8 minutes in females. Conclusions : The Mitchell Banks technique is the simplest and safest procedure in which excision of the sac and high ligation are performed without opening the inguinal canal.

    Purpose : To assess the long-term outcome of biliary atresia patients treated in our institution by the Kasai procedure from 1967 to 1998. Materials and Methods : A total of 88 patients with biliary atresia were treated by the Kasai procedure from 1967 to 1998 at Niigata University. The study period was divided arbitrarily into three periods, 1967 to 1978 (period A, n=31) ; 1979 to 1988 (period B, n=32) ; 1989 to 1998 (period C, n=25). Results : Jaundice disappeared in 47% (n=41) of all patients, and 28% (n=25) survived. In patients treated during period C, jaundice disappeared in 80% (n=20), and 60% (n=15) survived as a result of refinement of operative procedures and improvement in postoperative medical therapy. The overall 10-year cumulative survival rate was 40% and 15-year cumulative survival rate was 35%, improving to 63% and 50% respectively in patients showing disappearance of jaundice at least once after surgery. Poor prognosis was evident in patients who had undergone repeat-Kasai operation. There was no difference in the incidence of postoperative cholangitis between any period or the types of bile drainage procedure performed. On the other hand, mortality due to cholangitis improved during period C. Of the 25 patients who have survived, 6 patients are problem free, and the rest have postoperative complications. Conclusion : Outcome of biliary atresia treated by the Kasai procedure has improved over the past 10 years. Some patients deteriorated despite initial disappearance of jaundice due to postoperative complications in the long-term. As the length of follow-up increases, clinical assessment should be performed regularly and must include comprehensive analysis.

    Purpose : Although the prognostic factors for hepatoblastoma have been investigated from several perspectives, no satisfactory markers for predicting the outcome of patients with hepatoblastoma have been found. The purpose of this study was to determine the value of DNA ploidy and labelling for Ki-67 and p53 for the prognosis of patients with hepatoblastoma. Material and methods : Twelve patients with hepatoblastoma who underwent complete resection of tumors between 1965 and 1998 without preoperative multimodal chemotherapy were the subjects for the study. Histopathological specimens were obtained from paraffin-embedded tissue. DNA ploidy was classified using a DNA index based on measuring DNA contents using Hedley's method. Labelling for Ki-67 and p53 was determined using immunohistochemical staining with monoclonal antibodies to Ki-67 and p53. Grade of staining (GS) was positive (GS≧6.0%), or negative (GS<6.0%). Results : 1. The survival rate of patients with DNA diploidy (DD) was better than that of patients with non-DD (DNA aneuploid or DNA heterogeneity), but this difference was not statistically significant (p=0.390). 2. The survival rate of patients in the negative index group was better than that of patients in the positive index group for both Ki-67 and p53 staining (p=0.022, 0.002, respectively). 3. A relationship between being in the positive index group and the incidence of postoperative pulmonary metastases was found for both Ki-67 and p53 staining (p=0.091, 0.020, respectively). Conclusions : Ki-67 and p53 may be useful as markers for the prognosis of patients with hepatoblastoma.

    Purpose : Meckel's diverticulum (MD) is the most common developmental abnormality of the gastrointestinal tract. We compared symptomatic cases with asymptomatic ones according to clinical and pathologic features, and assessed whether asymptomatic MD should be resected or not. Methods : Thirty five patients have had MD resected at our institute. Of the 35 cases, ten were symptomatic, and 25 were asymptomatic. The 25 asymptomatic cases were diagnosed incidentally during laparotomy. All ten symptomatic cases had typical presentations. All cases (both symptomatic and asymptomatic) were classified into two groups according to the presence of the ectopic gastric mucosa. The two groups were compared with respect to the gross appearance of the MD. Results : In the ten symptomatic cases, eight had rectal bleeding, one had peritonitis due to perforation of the diverticulum, and one had bowel obstruction. In seven cases which had chronic gastritis associated with MD, Helicobacter pylori organisums could not be identified. Ectopic gastric mucosa was observed in all cases with rectal bleeding, and was considered to play an important role in bleeding. MD with ectopic gastric mucosa is also likely to be folded over into the mesentery, and be attached to the mesodiverticular band. Conclusions : Although the chance of complications from MD found incidentally is difficult to assess, there is a possibility of primary gastrointestinal neoplasms developing in MD and they should be resected because of this in all cases.

    A rare case of ileo-ileo colic intussuseption caused by an ileal heterotopic pancreas in a 14-month-old boy is reported. The boy was referred to our clinic after failed hydrostatic reduction. On admission, ultrasonography revealed a target sign in the upper left quadrant of the abdomen. Under general anesthesia, laparoscopic reduction of the intussusception was performed. After laparoscopic reduction of ileoileal intussusception, a whitish subserosal mass, measuring 1 cm in diameter, was noted approximately 30 cm from the ileocecal valve. It appeared to be the cause of the intussusception and was suspected to be heterotopic pancreas from its gross appearance. Thus, ileal resection to include the mass was performed through a minilaparotomy by extending the umbilical incision. Histology of the mass revealed heterotopic pancreatic tissue with acini, endocrine islet cells, and pancreatic ducts (Heinrich type I).

    The recent advancement of ultrasonographic and MRI examinations can provide more accurate diagnosis of renal diseases in utero. Out of 60 antenatally diagnosed cases, we had 10 renal cystic lesions, which consisted of 7 hydronephrosis and 3 polycystic kidney. We report 2 cases of genetic polycystic kidney disease. One is a case with autosomal dominant polycystic kidney disease (Case 1), and the other with autosomal recessive polycystic kidney disease (Case 2). The differential diagnosis was performed from the various kinds of polycystic diseases and cystic variants of pediatric renal tumors such as Wilms' tumor, glomerulocystic kidney disease and mesoblastic nephroma. To make the diagnosis, a positive family record and various sizes of cysts shown by echograms in utero were useful in case 1. On the other hand, in case 2, both enlarged kidneys with small protrusions to the renal capsulae shown by the in utero MRI and small cysts shown by echograms after the birth were useful.

    We performed total urogenital mobilization in a 9-month-old girl with persistent cloaca. After the rectum was separated from the common channel (urogenital sinus) through a posterior sagittal incision, both the urethra and the vagina were mobilized together as a single unit, and orifices were created on the vertibule. The appearance of external genitalia was almost normal with the well-created perineal body. One year after this procedure, there was no evidence of urinary incontinence, vaginal stricture or other complications.

    A 14-year-old girl was transferred to our hospital because of primary ileus. She had been treated for osteosarcoma of the fourth rib for 1 year. She had no history of abdominal surgery. An abdominal X-ray film and a CT scan revealed the dilated small bowel with air-fluid level. An emergency laparotomy was performed revealing a white, shiny mass 7 cm oral from the terminal ileum. Decortication improved the small bowel obstruction. Operative diagnosis was abdominal cocoon. Histopathological study confirmed fibrous tissue without any other malignancy or inflammation. Postoperative course was uneventful. Abdominal cocoon is a rare entity in abdominal surgeries. Emergency operation is warranted in the case of primary ileus-abdominal cocoon-in the pediatric population.

    An 11-year-old girl has been noted with prolapse of the rectal mucosa during defecation since the age of five. The prolapse disappeared spontaneously after defecation. On rectal examination, the mass, 5 cm in diameter, was palpated on the posterolateral wall of the rectum with an elastic consistency and it was covered with normal rectal mucosa. A barium study of the rectum showed a round defect in the posterior wall of the rectum. The mass, containing white and mucinous fluid, was resected through a transanal approach. Histological study revealed that the cyst was a rectal duplication with heterotopic cylindric ciliated epithelium and zones of cuboidal epithelium. A rare case of intestinal duplication with a ciliated epithelium is reported.
