Ischiorectal Abscess in a Child Resulting From Ingested Fish Bone : Case Report

The case involved to 12-year-old girl with West syndrome who had had anal discomfort complicated by a high-grade fever for several days. Soon after, a large perianal abscess developed and the patient was referred to our department. A pelvic CT scan revealed a gas density area and a high density, needle-like foreign body in an ischiorectal abscess. The abscess was urgently extirpated and drained A fish bone 2 cm in length, was found in the abscess cavity. A bacterial culture of pus with Batteroides fragilis and Escherichia coli was growing. After draining the abscess, therapy with antibiotics rapidly improved her general condition. Reports of periproctal abscesses due to ingested fish bone are occasionally found in adult cases, and this condition most commonly occurs in males between 40 and 60 years old However child cases are rare. Diagnosis of a foreign body in the anus is easily made on simple digital rectal examination at the first symptom of acute anal pain In a case such as one with a mentally retarded child with vague symptoms, delayed diagnosis will result in deeper penetration of the fish bone and abscess formation. For preoperative diagnosis, a CT scan should be particularly helpful for this condition.

We report a case of pyriform sinus fistula with another dermoid cyst in the median neck. A 3-mo-old boy was presented with a subcutaneous abscess in the median neck. A plain radiography showed an abnormal air-containing mass on the left side of the neck Esophagogram. CT and MRI showed pyriform sinus fistula in the region. After reducing the subcutaneous abscess, he was discharged. At 7 mo of age, a complete excision of the pyriform sinus fistula and excision of the dermoid cyst were performed MRI was useful for the management of pyriform sinus fistula, allowing complete imaging of the fistula.

We report the case of a 2-mo-old formerly 31-week premature baby boy presented with intestinal perforation after manual reduction of an incarcerated hernia. When the infant was transferred to our hospital 12 h after hernia reduction, his general condition was deteriorating and hemorrhaging via the rectum was evident X-ray imaging revealed intestinal obstruction, and blood examimation data suggested severe inflammation. During lapalotomy, a small deal perforation with peritonitis was found. An ileostomy was made to allow extenonzation of the perforated site. The stoma was taken down on the 42nd postoperative day. We presumed that the gangrenous bowel of the incarcerated inguinal hernia could be reduced. Therefore close observation is required even after successful reduction of such incarcerated hernias seen in formerly premature infants.
