    The purpose of this study was to delineate the process of bone invasion by oral carcinoma. An experimental model of oral carcinoma invading jawbone was established by transplanting mouse NR-S1 tumor or human-cultured NB-1 tumor to the tooth extraction socket. In the early stage after tumor transplantation, the tumor grew in the extraction socket and subsequently filled the socket and extended outward to the neighboring regions. Bone resorption and collagen production status, as well as HSP47 expression, were investigated in the process of the tumor invasion. Osteoclasts appeared increasingly on the surface of the corresponding bone during the early stage and then decreased in number as bone resorption with tumor invasion progressed. Enlarged osseous lacunae, as well as Haversian and Volkmann's canals, were present in the bone near the tumor, and tumor cells were also detected in the enlarged osseous spaces. Expression of HSP47 was positive for the carcinoma cells infiltrating into the bone tissue, but not for the counterparts in the mass. In summary, cells may invade bone through the osteolytic spaces and necessitate ECM around them for their nutritional supply to proliferate.

    Patients suffering from jaw cysts treated in the department of Oral and Maxillofacial Surgery, Academic Hospital of the Free University of Brussels during the period 1982 -1997 were studied. The pathological reports of these cases were analyzed with regard to age, sex, anatomical distribution and diagnosis. About 40% of jaw cysts occurred in patients aged 20 to 49 years, and the male to female ratio was 2:1. Of the 203 cases, 121 (60%) were located in the maxilla, the remaining 82 (40%) in the mandible. Radicular cysts were by far the most common type of cysts (78%) followed by dentigerous cysts (11%), odontogenic keratocysts (7%) and fissural cysts (4%).

    Carcinomas of the oral cavity sometimes arise as multiple, independent lesions and this observation has led to the concept of field cancerization. Howerever, the mechanism of multiple occurrences of oral squamous cell carcinoma (OSCC) is still unknown. Thirty-one cases of synchronous and/or metachronous multiple OSCCs were selected from biopsy and operation materials of the Dental Hospital, Tokyo Medical and Dental University. Histopathogenetic study and investigation were carried out. The clonality of the tumor tissues was analyzed by immunohistochemistry for Ki-67, Bcl-2, p53, cyclinB1 and cyclinD1. Histologic observation showed two patterns of occurrence of OSCC: de novo type and premalignancy-associated type. Twenty-one patients with multiple OSCCs were classified into 3 groups. Group I was de novo type, Group II was mixed type and Group III was premalignacy-associated type. Seventeen of 21 patients (80.9%) revealed p53-positivity in the epithelium apart from the tumor. Fifty-two of 69 tumors (75.4%) showed a different expression rate between Cyclin B1 and Cyclin D1. This data suggests that those patients are at high risk of developing second primary tumors. It also suggests the possibility of polyclonal development of the multiple OSCCs.

    Oral capillary hemangioma (OCH) is a benign hamartomatous lesion that exhibits a proliferation of capillary endothelial cells and abnormal capillary formation. Its occurrence is relatively common in the oral cavity, yet the pathogenesis is not well understood. To elucidate the mechanism of OCH development, we examined the expression of vascular endothelial growth factor (VEGF) and basic fibroblast growth factor (bFGF), as well as their receptors (VEGF-R2, FGF-R), in OCH of 45 patients using an immunohistochemical staining method. More than 50% of the endothelial cells and the stromal cells of OCH showed significantly elevated expression of VEGF or/and bFGF, accompanied with an increase in the expression of their receptors. The increased number of capillary vessels in OCH was positively correlated with the expression level of these angiogenic factors. Our data indicate that the pathogenesis of OCH is closely associated with overexpression of angiogenic factors, which may result in autocrine or paracrine stimulation of endothelial cells and their precursors.

    The cases of three tetanus patients who visited our department with lockjaw as the chief complaint are presented. While the incidence of tetanus has decreased markedly in Japan, the disease remains highly lethal. Trismus is one of the initial symptoms associated with tetanus, and some patients with this symptom present to dental clinics or oral surgery departments. Since tetanus requires prompt treatment, dentists and oral surgeons must be aware of the clinical symptoms and pathology of this disease.

    Epulis, a discrete sessile or pedunculated mass of fibrous tissue on the gingiva caused by chronic inflammation, is a common lesion. However, other lesions may occasionally exhibit exophytic growth and they must be differentiated from epulis. A 38-year-old Japanese female noticed a pedunculated soft mass on the alveolar ridge of the right maxillary third molar shortly after extraction of the tooth. The mass, which had periods of exacerbation and quiescence, was excised under a tentative diagnosis of epulis. Because histopathology revealed edematous, inflamed antral mucosa with a covering of ciliated columnar epithelium, a final diagnosis of herniation of antral polyp through an oro-antral fistula was established. Although a rare occurrence, herniation of antral polyp should be included in the differential diagnosis of exophytic lesions on the maxillary molar alveolar ridge if there is a history of dental extraction at the concerned area.
