Micro arteriovenous fistula (AVF) is one of the causes of intractable ulcers of the lower extremities. We report two cases of micro AVF with skin ulcers who underwent transcatheter arterial embolization (TAE). Case 1 is a man in his 40s with a left leg varicose vein and a bleeding skin ulcer after a stripping and venous sclerotherapy. Although the ulcer tended to shrink during the postoperative course, angiography was performed because bleeding was observed again. A micro AVF was diagnosed based on abnormal vascular stain around the ulcer and early venous filling, and the posterior tibiofibular artery branch was embolized with coil. Case 2 is a man in his 50s who underwent angiography due to repeated skin ulcers and bleeding on both lower extremities, with frequent bleeding from the left lower extremity ulcer. A micro AVF was diagnosed based on abnormal vascular stain around the ulcer and early venous filling, and the anterior tibial artery branch was embolized with microsphere and coil. In both cases, although the ulcers persisted, we were able to obtain disappearance of bleeding and a tendency for the ulcers to shrink.
TAE is useful as a control for bleeding in micro AVF of the lower leg complicated with intractable bleeding skin ulcers.
