    年齢25~70歳で術前の不同視の程度が4.1D~7.1Dの,近視性不同視の症例4例4眼に
    エキシマレーザー
    によるPRK(photorefractive keratectomy)を施行し,術後の視機能を検討した.屈折は,術後6ヵ月時で平均4.3D遠視化し,狙いの矯正度数に対する達成率は平均109%で,不同視の程度は,平均5.8Dから1.5Dに改善した.視力は,矯正視力は殆ど変化なかったが,裸眼視力は大幅に改善した.コントラスト感度は,角膜上皮下混濁がピークの術後1~3ヵ月には術前のレベルより低下するが,角膜上皮下混濁がほぼ消失する術後6ヵ月には術前のレベルに回復した.コントラスト感度が低下しても,矯正視力は低下しなかった.
    エキシマレーザー
    によるPRKは,不同視の改善に有用であり,今後が期待されると思われるが,臨床導入の前に視機能への影響を十分に検討していく必要があると思われる.

    The purpose of this study was to investigate the short-term biological effects of Excimer Laser. The surface of the skin in the rat was irradiated with KrF Excimer Laser at wave length of 248nm for 30 seconds at pulse repetition rate of 10Hz and pulse width of 15nsec.
    Energy density was 3.1mJ/cm2, 47.6mJ/cm2, 160.0mJ/cm2, 576.9mJ/cm2 or 1714.2mJ/cm2.The irradiated skin tissue was removed of various times between 1 hour and seven days after rradiation and with H-E stain for pathological examination. Necrosis was observed in epidermis irradiated at 47.6mJ/cm2 and in dermis irradiated at 576.9mJ/cm2, and defect of the muscular tunic was observed in tissue irradiated at 1714. 2mJ/cm2. These results indicate that depth of skin defect is related to energy density and that necrosis occurs at energy density ranging from 3.1mJ/cm2 to 47.6mJ/cm2. Satisfactory wound was observed after irradiation at each energy density.

    We have been studied to establish the requirement for the coronary laser endoscope with animal experiment using goats' coronary arteries by developing various coronary laser endoscopes. The purpose of this study was to develop coronary laser endoscope in which the tip had the bending function.
    In this study, the laser endoscope with the diameter of 1.7mm which is thought to be applicable to the coronary artery, in which the tip had two directional bending function, was developed. The endoscope was evaluated with goat's carotid artery which was partially occluded with organized thrombus made by the interposition of Gore-Tex artificial graft.
    The endoscopic view was very improved with two directional bending function of the tip, and excimer laser could be scanned in line to destruct the thrombus.
    More than three directional bending function of the tip is required to improve the scanning ability of laser and is the most important next subject.

    The research for the endoscopic coronary laser angioplasty system using excimer laser and a very thin diameter endoscope has been performed since 1987 by us. To date, the laser endoscopic system, which is able to approach into the coronary artery percutaneously, could be developed. However in-vivo study showed that there remained the following problems; 1) it was impossible to make the laser aiming under the moving visual field caused by the beating of the heart, 2) it was impossible to perform the operation successfully within a few minutes of manipulation inside coronary because of the ischemie of the heart.
    In order to improve these problems, we tried to examine the possibility of the procedure in the arrested heart by using the circulatory assistance.
    The PCPS system was used for the circulatory assistance. The endoscopic coronary laser angioplasty system consist of the long-pulse excimer laser (308nm wave length, 8Ons pulse duration) and the ultra-thin laser endoscope (1.5mm diameter) for the coronary artery was used for the experiment. The endoscope had the 4-directional bending function at the tip.
    Four goats with about 50kg weight were used. When the heart was beating, it was impossible to aim the target with the laser. However, when the heart was arrested under the circulatory assistance, the target could be aimed successfully with laser. The arrested heart was begun to beat again with the DC shock after the examination. In the experiment of the endoscopic coronary laser angioplasty, the heart was arrested immediately after the insertion of the skin tissue into the coronary artery by the acute myocardial infarction. Under the circulatory assistance, the excimer laser did not failed to be aimed to destroy the obstruction. However, the length of the obstruction was too long to remove the obstruction completely. The safety and reliable endoscopic coronary laser angioplasty would be possible, if the safety cardiac arrest condition was obtained in future.

    The KrF excimer laser is a high power pulsed laser oscillating in the ultraviolet field and able to destroy or amputate molecules of tissue without thermal damage.
    In order to evaluate the effects of KrF excimer laser irradiation on human mandibular bone, we examined by visual, pathological and SEM observation.
    The laser irradiation on human mandibular bone resected by oral and maxillofacial surgery had a pulse energy: 65mJ, spot size: 0.80×0.50mm2, energy density: 16J/cm2, water volume: 6.5ml, assist gas (N2): 301/min, frequency: 80Hz, incision speed: 5, 10, 15mm/min.
    Visual observation: the incisions were sharp without thermal damage, and the depth of the inicions ranged from 0.68 to 2.53mm according to the incision speed.
    Pathological observation: the inicisions were almost sharp and there was almost no thermal damage layer.
    SEM observation: the incision section was slightly wavy, but the incision surface was almost clear at 20 magnification. Crystals formed at the incision surface at all incision speeds, and there was no melting of bone tissue at 5000 magnification.

    Experimental results of fiberoptical delivery and a reaction on atherosclerotic tissue using a long pulse excimer laser are described. In order to improve the maximum energy delivered through optical fiber, a long pulse excimer laser which had the pulse duration of 100nsec was examined. The quartz fibers with both 300 and 400μm core diameter were used for the laser energy delivery. As a result, the maximum energies transmitted through optical fiber were 30mJ/pulse with 400μm core fiber and 16mJ/pulse with 300μm core fiber. And the average energies transmitted constantly were 20mJ/pulse and 10mJ/pulse with 400μm core fiber and 300μm core fiber, respectively. The energies were about 3 times lager than these using a conventional short pulse excimer laser. The laser energy delivered through the fiber was irradiated on the sample of atherosclerotic aorta of a rabbit in saline to investigate the tissue reaction on atheroma. The result shows that the fiberoptically delivered energy of long pulse excimer laser was enough for the destruction of atherosclerotic tissue.

    We have been developing coronary laser angioplasty system with excimer laser. And the coronary laser endoscopic system has been developed to realize visual coronary laser angioplasty.
    In order to design the coronary laser endoscopic system, various kinds of endoscope were developed experimentally and evaluated with animal experiment using goat to establish the requirement for the coronary laser endoscope.
    The requirement for coronary laser endoscope was clarified to be as follows:
    1, Diameter: 1.5-1.8mm max (goat, 50kg)
    2, Flexibility of the tip: flexibility is required and the combination of guide wire is useful for the successful coronary approach with endoscope.
    3, Stiffness of the body: stiffness is required for the better handling.
    4, Image fiber: 2000 pixel is possible. Light guide: good
    5, Bending function of the tip: 4 direction (at least 1 direction)
    6, Cannel: 1 channel for the guide wire and laser fiber passage.
    In present time, the bending function of the tip to 4 direction is not developed yet, and the technique of bending mechanism is being studied.

    目的:Photorefractive keratectomy(PRK)術後15 年以上の長期経過観察が可能であった症例に対し,角膜形状解析装置により角膜前面屈折力の変化について検討し,PRK の術後角膜形状の安定性について評価した。  方法:対象は,PRK を実施し15 年以上経過観察が可能であった7 例12 眼として,角膜形状解析を行った。測定は角膜形状解析装置TMS にて実施し,検討項目はsimulated keratometry(SimK),differential map(DM),surface regularityindex(SRI),surface asymmetry index(SAI),corneal eccentricity index(CEI)の5 項目とした。各項目を,術前,手術施行から0.5,1,5,10,15 年目時点で測定している角膜形状データから各検討項目を抽出し評価した。  結果:SimK では術後1 年と比べ,その後15 年までsteep 傾向への緩やかだが有意な変化がみられた(p<0.05)。DM では,術後1 年以内の変化量と5 年目以降の変化量を比較,5 年目以降の変化は有意に少なくなっていた(p<0.05)。SRI・SAI では術後1 年まで変動し不安定であるが,術後1 年以降は術後15 年まで安定を示した。CEI では手術により角膜形状はoblate を示したが,それ以降術後15 年まで,わずかずつプラス方向への変化がみられるが有意差はなかった。  結論:術後早期では変動があるものの1 年以降は安定的であり,5 年目以降の変化は非常に軽微な変化であった。このわずかな変化では角膜中央部はsteep,角膜全体はprolate 傾向の変化を示した。不正乱視の15 年間の変化は正常範囲であり,PRK 術後においても安定的であった。

    Recently, lasers operating at shorter and shorter wavelengths have becomeavailable. The excimer laser emits pulses of ultraviolet wavelengths.The KrF excimer laserhas high incision efficiency, and can cut hard tissue without thermal damage. An incisionwas made on the mandibular bone of a pig by laser under the following conditions. Frequency: 60Hz, pulse energy: 32mJ, pulse width: 15ns, beam spot size: 1.5×0.4mm2, energy density: 5.3J/cm2, irradiation time: 5min, volume of distilled water: 5l/min, andvolume of assist gas: 30l/min. After irradiation, visual, pathological and SEM observations was carried out. There were almost no thermal damage layers. The surface of boneincision was comparatively sharp. But, SEM finding showed the melting of tissue at the surface of bone.

    The endoscopic coronary laser angioplasty system using excimer laser thin diameter endoscope has been developing.
    In order to improve the transmitted energy through the laser fiber, long pulse excimer laser was used for the experiment. This laser could oscillate 308nm wavelength (XeCl) with maximum I 50m.1 per pulse energy and 100ns pulse duration.
    In order to improve the bending function at the tip of the endoscope. the endoscope, which had 4 directional bending function at the tip and had one channel for the laser fiber, was developed and evaluated using organized thrombus inside the goat's carotid artery which was made beforehand by the interposition of artificial graft with I cm length.
    Using this long pulse excimer laser, the transmitted laser energy was improved to be maximum 30mJ/pulse and average 15mJ/pulse using the optical fiber for ultra violet laser transmission with 400μm core diameter. The energy was enough to destruct the atheroma with non-contact irradiation.
    The developed endoscope had 1.5mm diameter, 1.7mm diameter at the tip and 150cm inserting length. Th endoscope had 4 directional bending function with 10-20° for each direction. The laser could be scanned to organized thrombus to destructed through this endoscope. This endoscope improved the scanning ability of laser irradiation. As the endoscope had small diameter enough to attach the coronary artery, it is possible to perform the experiment with coronary artery for the next step.

    A pulsed ultraviolet excimer laser has been investigated as a cutting-ablating tool for bone surgery. The special feature of the KrF excimer laser is that it cuts bone tissue without thermal damage. In our earlier report, we determined the best frequency for healing of femur wound. In the present study, femurs were examined over a long period. The laser was used at pulse energy: 50mJ, beam spot: 0.015cm2, pulse width: 15ns, peak power: 3.3MW, energy density: 33.3J/cm2, incision speed: 4mm/min, irradiation time: 30s, and frequency: 20Hz. The femurs were examined histopathologically just after the operation, and at 1, 2, 4, 8, 12, and 48 weeks after the operation.
    In the case of femurs examined at the postoperative 48th week, there was remaining the rmal damage around the cortical bone, and the new bone did not connect with the cortical bone.

    We studied scattering of the probe beam at ablation plume on KrF excimer laser (λ=248nm) escharectomy to monitor ablation condition. The transient change of the probe-scattering light of He-Ne laser (λ=633nm) from the ablation plume was detected. We measured the gradient of the rise slope (Gr Slope) and the e-holding decay time of the probe-scattering waveform. Gr Slope at high fluence (5J/cm2) irradiation was two times sharper than that of near threshold fluence (1J/cm2). The e-holding decay time was 1.2 times longer at 10 shots. Gr Slope and e-holding decay time showed to have relation to the ejection velocity and the diffusion time of the ablation plume respectively. We evaluated performances of this reflectometry by comparing with the time-resolved photography. The reflectometry may be the useful information from the ablated surface to maintain the optimum control of the irradiation condition without remaining hard char grains.

    We carried out a study on the possible application of the excimer laser in laser angioplasty. In the first experiment, the ablative effects of the excimer laser at the wavelengths of 248 nm (KrF) and 308 nm (XeCl) on the pig myocardium were examined in vitro at an air-tissue interface. Very clear cuts were observed on the histological examination. There was no evidence of thermal damage. In the second experiment, the excimer laser was coupled to a 400 μm quartz optical fiber. Laser angioplasty was performed in 4 totally occluded femoral arteries in mongrel dogs using the excimer laser (XeCl: 308 nm). The arteries were successfully recanalized in all 4 arteries using 2.5-6 mJ/pulse, 20-30 pps and 6000 shots.

    Recently, lasers operating at shorter and shorter wavelengths have become available. The excimer laser emits pulses of ultraviolet irradiation. The KrF excimer laser has high incision efficiency, and can cut hard tissue without thermal damage.
    Use of a laser in the oral region is dangerous due to the close proximity between the operative field and the tracheal tube. Evaluation of the influence of laser irradiation on tracheal tubes is therefore necessary.
    The authors assessed the effects of KrF excimer laser irradiation on Portex®, Spiral tube® and Diffencer® tracheal tubes under ordinary atmospheric conditions, and gas flow rates of 3l/min for both O2 and N2O. The laser was used at energy densities of 40 and 60 J/cm2, frequencies of 20 and 40Hz, a beam spot of 0.015 cm2, a pulse width of 15 ns, and a peak power of 2.7 MW.
    The tubes was perforated by laser irradiation under ordinary atmospheric conditions, but they did not ignite. The time required for perforation of the tubes became shorter as the energy density and frequency increased. The perforation time under the flow of O2 3l/min and N2 O 3l/min was comparable to that under ordinaly atmospheric conditions. Spiral tube® ignited almost simultaneously to the time of perforation at energy densities of 40 and 60J/cm2 and a frequency of 40Hz.

    は条件を適切に設定すれば, 中枢神経における組織の部分切除, 除去などに, 応用可能であると考えられる。
