    A survey by web questionnaire concerning actual conditions of areas, satisfactions of their spaciousness and preferable sizes of houses and living rooms was conducted.

    Background: We hypothesize that single coil lead system is less troublesome in a long-term period because of its simplicity. Afraid of elevated defibrillation threshold (DFT), we examined DFT in both single and dual coil in the same patients. Method: This study was a prospective, multicenter study. Twenty-eight ICD/CRTD with dual coil lead system patients were included (24 men, 4 women; mean age 67.3±9.0 years). We performed DFT test from 10J to 25J by 5J step-up protocol both single (RV to CAN) and dual coil (RV to SVC & CAN) system respectively. Nine patients were re-examined DFT test after 1 week. Result: The characteristics of the patients as follows; IHD: 11, HCM: 4, DCM: 3, Brugada syndrome: 2, idiopathic Vf: 2, others: 6. In total of 36 DFT test performed in 27 patients, there were no significant differences in single and dual group (11.7±3.4 vs. 10.7±3.0J, p=0.28). The same results were obtained after one week DFT test. Only one patient needed external defibrillation to terminate VF, but his DFT was 25J with dual coil system. All patients successfully defibrillated margin with greater than 10J. Conclusion: Single coil system needed a

    higher energy delivery than dual coil system. But it can be feasible especially for the patients with long life expectancy.

    A DEM numerical model is constructed for analysis of ultimate lateral resistance to piles in sand. The pile was modeled as a rigid body with two kinds of pile movements: rotational and translational movement, simulating free head and fixed head condition, respectively. In the numerical experiments, the lateral resistance, lateral pressure distribution along the pile and the displacement of pile at different depths can be measured and used to construct p-y curve. In addition, the soil-pile interaction can be examined in detail to reveal the extent of influence due to the movement of the pile. In general, the results of the numerical simulation were compared favorably with other semi-empirical solutions.

    Author carried out this study for clarifying topographical characteristics and verifying scale of natural phenomenon in the places indicated by the “Shizensaigaidenshohi” (monument of natural disaster). The followings are the major results.

    1) Clarification of topographic characteristics where a “Shizensaigaidenshouhi” stands or indicates.

    Most of the place where “Shizensaigaidenshouhi” (monument of natural disaster, simply referred to as “the monument” in this article) stands or indicates, locates not only in very low areas (e.g. flood plain), but also in a

    bit
    higher areas (e.g. natural levee). This is because the monument building was not reasoned by heavy rainfall or flood itself, but by the damage of lives or properties which exist in higher areas.

    2) Verification of scale of natural phenomenon which brought disaster where a “Shizensaigaidenshouhi” stands or indicates.

    Past flood depth inferred by the description of the subject monuments was slightly low or equal to that estimated by modern hazard map. In early and middle of the 20th century, many flood disaster happened and the relevant monuments were built thereafter, but in the late 20th century, the number of severe floods which recorded in the monuments decreased. However, frequency of heavy rainfall is increasing recently. This suggests flood disasters likely to be recorded in the monuments would occur again in the near future.

    These results show that location of a “Shizensaigaidenshouhi” stands or indicates prefers a

    higher areas as people lives more in such areas compered to lower land and also show that past flood depth inferred by the description of the subject monuments was compatible with that estimated by modern hazard map.

    Map symbol “Shizensaigaidenshohi” is recently introduced to the basic topographic maps, and the new topographic maps therefore provide people with new map reading experience, which might raise people's awareness of disaster prevention as existence of “Shizensaigaidenshohi” is deeply relating to topography.

  • An Operative Strategy based on Anatomic-Angiographic-surgical Correlation and using the Pars Triangularis as Intraoperative Landmark

      Objective : The authors present the anatomical and angiographical details that enable surgeons to quickly locate middle cerebral artery (MCA) aneurysms and to gain proximal control without unnecessary delay or premature rupture.
      Materials and Methods : The anatomical dissections were performed in 10 adult cadaveric heads from 1993 to 2011 at the Department of Neurological Surgery, University of Florida. The angiographic and the surgical data were derived from 93 MCA aneurysms operated on by Hung Tzu Wen (HTW) from 1996 to 2012 at the Hospital das Clínicas, University of São Paulo and Hospital Samaritano, Brazil.
      Results : MCA aneurysms arise most frequently from the M1 segment and less frequently from M2. From a practical viewpoint, the M1 extends from the carotid bifurcation to the MCA genu (on the basal surface of the cerebrum) with specific topographical relationships along the way, and the M2 extends from the MCA genu to the sylvian or “M” point (on the lateral surface), also with specific topographical relationships. The key for the angiographical analysis of an MCA aneurysm is to establish its topographical relationship to the genu of the MCA. If it is proximal to the genu, it is important to estimate its distance to the carotid bifurcation and to the MCA genu. If it is distal to the MCA genu, it is important to estimate its distance to the genu and to the sylvian point ( “M” point). Also, it is important to evaluate the direction of the dome of the aneurysm, as it indicates the structures to which the dome is attached. The key for locating an MCA aneurysm intraoperatively is the relationship between the MCA genu and the tip of the pars triangularis. The tip of the pars triangularis is a reliable intraoperative landmark (even when it is obscured by severe subarachnoid hemorrhage) and it is located just distal to the MCA genu and approximately 2 cm distal to the sharp transition between the basal and the lateral surfaces of the cerebrum. Once the pars triangularis is identified, the MCA genu can also be quickly estimated and identified, and thereby so will the aneurysm.
      Conclusion : The carotid bifurcation, genu of the MCA, and the “M” point on the AP view carotid angiography, and the sylvian triangle on the lateral projection constitute the cardinal landmarks for locating MCA aneurysms angiographically. Correlating the angiographic location of the aneurysm to the pars triangularis of the inferior frontal gyrus constitutes the key for then locating the MCA aneurysm intraoperatively.

    本研究は,寒冷地住宅の快適性を損なわずにエネルギー消費量を低減するため,ICTと日射エネルギーを効果的に活用する北方型住宅向けエネルギー管理システム(HEMS)の構築を目的としている。この中核技術が,在室者の周囲の温度と照明を調節する室内環境制御システムと,換気のために導入される低温の外気を蓄熱した日射エネルギーで温める換気昇温器を備えた太陽熱利用システムである。前者は居室全体の平均室温や照度を上げないことで無駄なエネルギー消費量を抑えることを狙いとし,後者は貯湯タンクの蓄放熱効率を上げることで太陽熱利用システムの総合効率を向上することを狙いとした。これらの実証に向けて,既設の北方型実験住宅に太陽光発電システムやHEMSコントローラとともに上記の中核技術を適用した。在室者の周囲の温度と照明の調節には,在室者を検知して採暖や照度を制御する専用のモジュールを組み込んだ。平成25年12月初めから平成26年3月末までの期間稼動させた結果,換気昇温器を備えた太陽熱利用システムの有効性が見い出され,システム全体として快適性の改善に向けた知見と灯油使用量の低減に向けた示唆が得られた。

    Objectives: To reveal the effect of age and other factors on perceived anxiety over radiation exposure among decontamination workers in Fukushima Prefecture, Japan. Methods: A survey questionnaire was sent to 1505 workers, with questions regarding age, presence of a written employment contract, previous residence, radiation passbook ownership, presence of close persons for consultation, knowledge of how to access public assistance, and a four-point scale of radiation-related anxiety (1= "Very much," 2= "Somewhat," 3= "A

    ," and 4= "None" ). The relationships between the degree of anxiety and variables were analyzed using the chi-square test and residual analysis. Results: In all, 512 participants responded to the questionnaire. The mean age of participants was 46.2 years (SD: 13.1, range: 18-77). Of them, 50, 233, 168, and 61 workers chose "Very much," "Somewhat," "A

    ," and "None," respectively, on the anxiety scale. Chi-square test showed that participants aged 61 years and over had higher degrees of anxiety (p<0.001). Ordinal logistic regression showed that the degree of anxiety increased if they did not have a written contract (p=0.042) or persons to consult (p=0.034) and if they routinely checked the dose rate (p=0.046). Conclusions: Decontamination workers who do not have a written contract or who are in socially isolated situations have greater anxiety over radiation exposure. Thus, it is important to both create supportive human relationships for consultation and enhance labor management in individual companies.

    In order to investigate effects of marble plate shape on the natural frequency and damping ratio, vibration analysis of the small base isolation system was done. The increase in the natural frequency can be suppressed by matching the radius of curvature of the concave spherical surface and the marble. Furthermore, mounting some rivets at the surface of marble bearing or boring the surface of marble bearing, the damping ratio of this system in all cases indicates a

    By electrolysis of water using new mixing equipment with low frequency vibratory fins, microbubbles of oxygen and hydrogen were produced for NaOH electrolyte. The effects of operating parameters of the mixing device, that is, vibrating frequency of fins, concentrations of electrolyte and electrolysis current on the average bubble diameter, bubble size distribution and gas holdup were investigated. This operation shows a new microbubble generation method.

    Portable radiography is available for the patient who is postoperative, severe condition and old. As they have weak immunity, it is important to prevent from hospital infection. Wrapping of 14×14 inch or 14×17 inch X-ray cassette by a plastic (polyethylene) bag a

    bigger than the cassette was proposed for infection prevention in portable radiography. How to wrap the cassette easily was devised using the sheath of a polyester bag cutting at the bottom. In radiography with the grid, the plastic bag fastens the X-ray grid to the cassette substantially without any other means. In addition, the wrapped cassette, or the cassette with grid covered by the foamed plastic sheet alleviates patient’s discomfort.

    Over soft soil area the geosynthetic-reinforced and pile-supported subgrade was often used to reduce the post-construction settlement. But after operation it was found that the settlement sometimes was bigger than required. For settlement evaluation the stress concentration ratio between pile and soil is very important and the arching effect is often considered to solve this problem. There are some methods to analyze the sharing of fill load, but the dynamic train loads were often ignored. In this paper the finite element was used to simulate the stress concentration ratio during fill stage construction and dynamic load which were compared with the in-situ test. The results during fill stage also were compared with EBGEO and BS8006. They showed that the reliability of current methods is related to some factors including fill height, the stiffness ratio of pile and soil. There is a critical filling height whether the dynamic load decrease arching effect or not. According to FE results, the critical height is 3m. When the filling height is smaller than 3m, the stress concentration ratio should be re-evaluated by the reduction factor.

    In the frame of the activity of JSME RC-268 committee, new bevel gear i.e. Invo-Planar bevel gear is developed, the tooth flank of which wheel is plane or whose transverse tooth profile is straight line. The cutting/grinding time of this bevel wheel can be more than 10 times shorter than the working time for conventional bevel gear wheel, when it is cut with 5-axis machine, because it cuts or grind the tooth flank through one path of tool movement. The finished tooth flank is far smoother than the curved tooth flank manufactured by many paths of cutting blades of tool. The production rate of the mating pinion with 5-axis machine is same as that of conventional bevel pinion. Some experiences in the production, quality of the product gears and performance survey of this new bevel gear is introduced to promote the discussion.
