Fractures of femur after Hip arthroplasty or Knee arthroplasty are well described, whereas fracture of the humerus after arthroplasty is relatively rare. We present a case of humerus fracture after hemiarthroplasty treated with cerclage wire and locking plate. The patient was a 74-year-old female. Her chief complaint was right, upper arm pain. 11 years ago, she had her right proximal humerus broken in a traffic accident. She underwent hemiarthroplasty of the right shoulder at that time. This time, she fell down and had her right humerus fractured again. 3 parts fracture was seen distal to the edge of prosthesis. Osteosynthesis was done. The proximal fragment was fixed with cerclage wires and the distal fragment was fixed with locking screws. The patient had a functional brace attached. Callus was identified 3 months after operation. This time, we used cerclage wire and locking plate. The usual cable plate is too large to fix a broken humerus. But the locking plate which we used is narrower and thinner than the usual cable plate and in this system, cerlage wire can be fixed to the plate. So the strength of fixation increases and the possibility of metarosis decreases. We think this system is useful for periprosthetic fracture of the humerus.
