This final report synthesizes a prevention‑oriented framework for reliability and safety across the product lifecycle. It proposes a “ design-operation circulation ” that links TEM-FM‑driven design reviews, condition monitoring of stress and degradation, predictive maintenance, an incident database, updates of standards / certifications, and feedback to field practice. Cross‑cutting insights are organized as three front‑line activities—A: rapid and appropriate response, B: prevention of recurrence via PDCA and standardization, and C: upstream prevention by prediction—supported by organization, systems, and society. Prevention integrates deduction (functions, mechanisms, stresses,
failure mechanisms) with induction (tests and case learning) under seven predictive viewpoints, especially TEM-FM backcasting from top events. Implementation guidelines formalize A→B→C linkages, add TEM-FM‑tagged data interpretation, and institutionalize multilayer protection. Extending the framework, quality misconduct is treated as organizational failure modes leading to social top events; practical QMS checklists, CQO leadership, and KPI/quarterly circulation reviewsembed sustained prevention..
