A fifty-nine year old male, farmer, was admitted with the complaint of anemia in March 1965. Physical examinations revealed no jaundice, hepatomegaly or lymphadenopathy. Peripheral blood: normochromic anemia with leukoerythroblastosis. Bone marrow aspirations: dry tap. Urine: albuminuria without Bence Jones protein. Blood chemistry: total protein 10.9 gm/100 ml with 49.2 per cent of gamma globulin. Ferrokinetics: PIT 27.0 mg/day and RCIU 63.2 per cent; radioactivity was high over the liver and low over the spleen and bone marrow. Needle biopsy of the bone marrow: myelofibrosis. Surgical biopsy of the bone marrow: myeloma with myelofibrosis. Immunoelectrophoresis of the serum: γA myeloma. Roentgenogram of the bones: destructive changes. No effect of blood transfusions nor prednisolone. The patient died of serum hepatitis on September 22, 1965.
