    An 18 year-old female visited our clinic with aesthetic complaints due to cleft lip and palate on the right side complicated with partial anodontia. Clinical and cephalometric examinations revealed severe orofacial deformities such as undergrowth of the maxilla and overclosure of the mandible. The purpose of the treatment was aesthetic improvement, however, it was determined that orthognathic surgery alone could not provide a satisfactory prognosis.
    In order to determine the optimal mandibular position, occlusal plane, and aesthetics prior to orthognathic surgery, a diagnostic upper denture was made. The newly-established intermaxillary relationships were evaluated clinically, cephalometrically, and electromyographically.. Aesthetic improvement was obtained by the team approach and the goal of orthognathic surgery became more predictable by the prosthetic approach.

    Secondary bone grafting into alveolar clefts has become a very popular procedure, though ideal results are not always obtained. In 76 patients treated alveolar bone graft from 1975 to 1988, data were obtained by studying 64 alveolar clefts of 51 patients. In order to evaluate the prognostic factors of this procedure, the relationships of clinical factors and prognosis of alveolar cleft after bone grafting were evaluated by univariate and multiple logistic regression analyses.
    The results were as follows:
    1. Bone bridge formation w as found in 51 alveolar clefts (79.7% ).
    2. The factors correlated with bone bridge formation were the age at the time of bone grafting, pre-and postoperative orthodontic treatment, and kinds of bone materials for univariate analysis.
    As for multiple logistic regression analysis, the age at the time of bone grafting, kinds of bone materials and kinds of closure techniques by soft tissue flaps were found to be independent cofactors.
    3. The fac tors correlated with acceptable alveolar bone level were the age at the time of bone grafting, the eruption stage of the canine on the cleft side and the cleft width. As for multiple logistic regression analysis, only the cleft width was found to be an independent cofactor.

    The purpose of this study was to clarify the efficiency of the treatment for hypemasal speech using CPAP therapy. Ten cleft palate patients who underwent surgery, three female and seven males, aged from 10 to 21 years, were selected as the subjects. The subjects had normal articulation but mild to moderate hypernasality.. Each subject was instructed to perform speech drills six days a week according to a protocol, which included a daily program in volving change of nasal air pressure and speech tasks. Induced air pressure was incrementally increased on a week by week basis for 8 weeks. Evaluation of improvement in velopharyngeal closure was carried out at 3,6, and 12 months after CPAP therapy as well as prior to it.
    For four of the ten subjects, the nasalance score after the CPAP therapy was significantly smaller (p<0.05; t-test) than that before the therapy. The results suggested that the CPAP therapy could reduce mild hypernasality.

    Investigations into the case of a 19-year-old male with unoperated complete bilateral cleft lip and palate were carried out. The patient's maxillofacial morphology was analyzed clinically and cephalometrically. The findings of the investigation were reported. From examination of the dental casts, it was found that the premaxilla was protruded, and the length of maxillary arch was longer but its width narrower than those in noncleft patients.
    Both the ANB angle and the angle of convexity reflected the relative maxillary prot rusion, but the SNA angle was within the normal range cephalometrically. The SNB angle showed that the mandible was retruded. The gonial angle was more obtuse and the mandibular plane was steeper. Maxillary central incisors were rotated and protruded preoperatively and slightly retruded postoperatively. The results suggested that a patient with bilateral cleft lip and palate has potential for normal maxillofacial development, except for the width of the maxillary arch which will be narrower than that in noncleft patients. Therefore, it was thought that performing early orthopedics and adequate orthodontic procedures is necessary to secure good maxillary arch form.

    In order to obtain information on the effective cleft palate team approach, malocclusions and speech state just prior to the commencement of orthodontic treatme nt were retrospectively examined using 61 cleft lip and palate patients who received orthodontic treatment from 1988 to 1995 at the University of Tokyo Hospital. The materials included clinical records on speech and orthodontic treatment, and dental casts. The results were as follows:
    1. Th e most frequent age at the commencement of orthodontic treatment was 9 years, followed by 8 years.
    2. Cross-bite was found most frequently in the regions adjacent to the alveolar cleft, many of the involved teeth were maxillary incisors and canines.
    3. The lingual arch and multi-bracket appliances were the most common devices in cases with unilateral cleft lip and palate, and quad helix appliance and palatal expansion plate were employed in bilateral cases.
    4. Speech examination wa s performed on 59 of 61 patients, and 47 of them had normal speech. There were no abnormal findings in 19 patients and speech was improved in 28patients by speech therapy. As for the other 12 patients,8 were undergoing treatment for defective articulation, and hypernasality was found in 4.
    5. The most frequent defective articulation among the patients was palatalized articulation, followed by glottal stop.
