      Introduction:Although numerous procedures have been used to correct cleft palate, buccal musculomucosal flaps have only been applied in limited cases such as wide cleft. In this study, morphological analysis of the soft palate was performed in cleft palate patients.
      Methods:Eighteen patients whose soft palate was closed with buccal musculomucosal flaps were included. Lateral cephalograms at rest, occlusion and during/a/phonation were used to evaluate the length and position of the soft palate. Results were compared with values for the non-cleft control group.
      Results:The length of the soft palate in the cleft palate patients was significantly longer than that in the control group. Sufficient palate elevation, as measured by angle and length, was achieved, and the values were not significantly different between the operated and control groups. The position of velopharyngeal closure was lower in the cleft palate patients than in the normal controls.
      Discussion:The musculomucosal flap is a useful option for the closure of wide cleft. The characteristics of soft palate morphology indicative for this procedure are longer length and equivalent velopharyngeal function to normal children.

      The purpose of this study was to retrospectively evaluate the records of patients who were treated at the orthodontic clinic in order to improve jaw deformity and malocclusion that occurred after maxillofacial fracture due to trauma. There were 11 patients(6 males and 5 females)who ranged in age from 9 to 44 years, with an average of 26.5 years. The most frequent causes of trauma were traffic injuries in 9, followed by sports injuries in 2. Of these patients, 8 had a fractured mandible, 7 had a fractured maxilla, and 4 had a fractured maxilla and mandible. The treatments performed at the orthodontic clinic included comprehensive orthodontic treatment for 4, surgical orthodontic treatment with two-jaw surgery and preparation of surgical splints for 3, and manual removal of incomplete dislocation of teeth, limited orthodontic treatment, and mandibular setback surgery for one patient each. Furthermore, after orthodontic treatment, there were many patients who required additional residual surgery, and the maximum was 11 times. In order to prevent jaw deformity and malocclusion after maxillofacial fractures due to trauma, involvement of an orthodontist to reduce maxillofacial fractures is desirable.

      Calcium phosphate bone cement(CPC)has been successfully used to reconstruct small bone defects in the field of orthopedics and neurosurgery. This product contains calcium phosphate powder, and it can facilitate the modeling and reconstruction of deformities when used as a paste.
      Surgical treatment of facial fractures usually requires weeks for satisfactory reconstruction. In some cases, surgical correction cannot be carried out after severe facial fractures with bone dislocation that caused facial contour deformity or enophthalmos. Here, we report two cases of severe facial deformity due to prolonged orbital and zygomatic fractures. The region was approached infraorbitally, and CPC was applied to correct infraorbital and malar depression as an onlay implant.
      In our experience, this material can be applied not only for reconstruction of bone defects, but also for correction of facial contour deformity due to prolonged orbital and zygomatic fractures.

      It is well known that malignant neoplasms, particularly basal cell carcinoma, can arise within nevus sebaceous lesions. However, it is rare to observe such transformation in a short period of time. Here, we report a case of nevus sebaceous that developed malignancy during pre-operative observation. A 44-year-old Japanese woman with a congenital skin lesion on the left temporal region presented to our hospital for treatment. The lesion was diagnosed as nevus sebaceous from clinical findings, and surgical resection was scheduled. At the time of the first visit, there was no evidence of malignancy. However, 4 months after the first visit, a glossy, round and black nodule(2 mm in diameter)was found within the lesion. Subsequently, surgery was performed. Postsurgical pathological examination revealed malignant basal cells in the hyperplastic sebaceous glands, and the diagnosis was basal cell carcinoma in nevus sebaceous. Although it has been reported that most basal cell carcinomas grow slowly, this case suggests that once malignant transformation occurs in nevus sebaceous, the growth rate is much faster than previously believed. Therefore, careful observation is necessary when treating patients with nevus sebaceous.
