      Purpose: Cleft lip patients have characteristic features due to congenital anomalies. Such features become notably worse if they are not treated adequately. We value early and staged treatment according to patient growth so that specific adverse features do not occur.
      Methods: We describe the treatment planning in three parts: (1) For the upper lip, the rotation-advancement method (Millard) and a small triangle flap are used in primary repair; (2) For the red lip, a boot flap is secondarily used for tissue deficiency or depressed deformity of the free border; (3) For the nose, an incision along the piriform margin and tagging up sutures are employed in primary repair. Reverse-U incision rhinoplasty is secondarily performed before a patient enters primary school.
      Results: Cases involving a long follow-up period (more than 15 years) were investigated. Our staged treatments according to patient growth showed good results.
      Discussion: The first operation for primary cleft lip repair is considered to be the most important, and surgeries during the growth period should be minimized.
      Conclusion: Staged repair for a unilateral cleft lip is considered to lead to favorable growth and good results.

      The Abbe flap is commonly used for lip reconstruction. However, while it is effective for horizontal constrictions, it does not adequately release vertical constrictions.
      The fleur-de-lis flap, described by Millard in 1964, is an Abbe flap with a horizontal vermillion flap, which can release not only horizontal but also vertical contracture of the upper lip. The original method is limited in size because the horizontal vermillion flap is elevated from the vermillion anterior to the inferior labial artery. In the present case, we elevated a horizontal mucosal flap from the oral vestibule posterior to the inferior labial artery to achieve large contracture release.
      The patient was a 70-year-old male who had a complete upper lip defect after resection of an upper lip adenoid cystic carcinoma. His upper lip was reconstructed with a buccal advancement flap; however, salivation was observed due to upper lip entropion. Elevation of the mucosal flap was performed in two sessions. The mucosal flap of the lower lip opposite to the pedicle was transferred to the upper lip with the Abbe flap in the first operation. The mucosal flap on the pedicle side was elevated at the time of flap separation and implanted after the contracture was released in the same manner.
      Postoperatively, the patient's salivation symptom was improved. We consider this flap to be one of the useful methods to release vertical contracture of the upper lip.

      Treatment for a dentigerous cyst involves cyst resection and removal of the embedded tooth, or marsupialization, to guide the embedded tooth to its original position. Here, we report the case of a 7-year-old boy with multiple impacted anterior teeth associated with impacted supernumerary teeth and a dentigerous cyst. Under general anesthesia, an impacted supernumerary tooth was extracted, and marsupialization was performed for the dentigerous cyst. At five months postoperatively, the maxillary left lateral incisor erupted into the oral cavity. At two years and eight months postoperatively, the maxillary left lateral central incisor erupted into the oral cavity. Orthodontic treatment was then initiated and brackets were worn for 18 months. At the time of bracket removal, panoramic radiographic images showed root resorption of the maxillary left central incisor. Gingival recession of the maxillary left lateral central incisor was also observed. However, root formation of the maxillary left lateral incisor and canine, which were compressed due to the cyst at the time of the initial examination, was unaffected. For young patients with multiple impacted teeth, as in this reported case, performing marsupialization while considering the burden on the patient was shown to be effective.

      Squamoid eccrine ductal carcinoma (SEDC) is a subtype of squamous cell carcinoma. It is a malignant tumor composed of squamous cell carcinoma and adenocarcinoma. The first three reported cases of SEDC were described in 1997 by Wong et al. Superficial diagnostic biopsies often lead to the misdiagnosis of SEDC as cutaneous cell carcinoma. Herein, we report a rare case of SEDC with repeated local recurrence following resection that was resolved with drug therapy. In our patient, SEDC was unresectable after the fifth recurrence; however, the tumor disappeared after combination treatment with chemotherapy and an immune checkpoint inhibitor. Herein, we present the case along with relevant literature.
