This article is a study of practical application on our previous paper entitled “Organizational Factor Analysis for Human Error.” We have obtained a simplified method, through uniting two evaluation lists, called “Situation of concerned team” and “Relational team characteristics,” into one list, and excluding and/or arranging conceptual redundancy among the evaluation items. In addition, we eliminated items which are distinguished as non・applicable based on four case-studies. For an inspection of the methodological validity, some cases were applied by ourselves and other disinterested parties, substantially similar results were obtained, except for various descriptions.
