    In recent years, unmanned aerial vehicles (UAVs) have been utilized in various fields due to their relatively low cost and technological advancement. While the use of drones is increasing. The number of accidents involving UAVs is also on the rise, with 71 accidents reported to the Ministry of Land, Infrastructure, Transport and Tourism (MLIT) in the fiscal year 2020, doubling to 139 in FY2021. One of the reasons for the increase in the number of accidents involving UAVs is the lack of effective countermeasures based on past accident cases and their insightful studies. This study attempted to create an incident reporting format for drones by employing the methodology used in the healthcare industry for incident reporting. It involved the classification of accident causes and the setting of incident levels necessary for conducting risk assessments.
