     We sent a questionnaire to all 76 branches of the Japan Ostomy Association (JOA) to inquire about problems regarding lectures for ostomates and the ostomy visitor system. We received replies from 59 branches (77.6%). All branches offer lectures for ostomates and 23 branches (30.2%) hold lectures more than 5 times per year. Problems of lectures for ostomates include a lack of operational expenses and qualified lecturers. In addition,lectures concerning spiritual care were requested. 17 branches (29%) sponsor lectures for training ostomy visitors and 28 branches (47%) have been requested to send ostomy visitors. Although the ostomy visitor system is thought to be helpful for improving the QOL of ostomates,it has not been popular in Japan. Healthcare professionals have to recognize the importance of the ostomy visitor system and make a concerted effort to implement this system across the country.

     We experienced a case of a patient with colostomy who was referred a dermatologist due to serious peristomal skin trouble and diagnosed of bullous pemphigoid. While oral steroid was administered to him and local ointment therapy was done to the erosion of the peristomal skin four weeks,non-adhesive appliance was used to the colostomy. As the erosion in the peristomal skin tended toward epithelization,the appliance was switched from non-adhesive system to adhesive system.
     From this experience we learned that when we take care of a patient with colostomy diagnosed of bullous pemphigoid,we need adequate evaluation and treatment by a dermatologist. And it is important to avoid adhesive appliance for blisters and to switch from non-adhesive system to adhesive system after epithelization.
