To clarify the optimal treatment of peristomal pyoderma gangrenosum(PG),the medical records of fourteen patients with peristomal PG were analyzed in terms of the clinical diagnosis, operative procedures, time until diagnosis of PG,initial dose of prednisolone, period required for healing of PG,local treatment, patients’ complaints, and so on.In conclusion,at the onset of peristomal PG, early diagnosis and systemic administration of prednisoline proved to be necessary. In topical management, gentie skin care, topical treatment using cadexomer-iodine and prednisolon ointment, application of hydrocolloid dressing, drainage by cutting of skin barrie, and early change of one-piece stoma bag proved to be effective.

We report a case of a 77-year-old man with senile dementia who came to care for his stoma by himself independently during a lomg hospitalization. After undergoing an abdominoperineal resection of rectum and colostomy, wound infection delayed his recovery and this prolonged his hospitalization generally,it is difficult to perform stoma care without any help from others for a patient with senile dementia;it is more common to do so with the assistance of others.ln this care,we made it possible for him to do it by himself independently by developing his ability and supporting him intensively and patiently. In addition,we think it is a great benefit for him to have reconstituted his life cycle with stoma care as its center during his long-term hospitalization.

Bathing is an integral part of Japanese life, and in Japan there are numerous facilities that provide bathing as a from of recreation. However, many patients with stoma believe that they are unable to use bathing facilities, and information concerning the use of these facilities by such patients is scarce.
The author has carried out a questionnaire survey of 260 bathing facilities concerning the access of patients with stoma.
The response rate was 32.6%(85 facilities),but the survey shows that 58 out of the 85 replaying facilities(68%)are available for patients with stoma.
The facilities requested that such patients wear pouches, properly dispose of evcretory substances, and used pouches.
While informing patients with stoma about conditions of bathing facilities and their requirements,it is equally important that self-care guidance of such patients be conducted. The provision of proper information to bathing is likewise needed.
