    Clinical symptoms before and after permanent artificial cardiac pacing in 71 patients (35 male and 36 female; averaged age 65.1 years old) with bradyarrhythmia were described. These patients were diagnosed and classified into the following three groups: (1) A-V block (36 cases), (2) Sick sinus syndrome (31 cases), and (3) Atrial fibrillation with slow ventricular response (4 cases). The clinical symptoms shown before pacing were Adams-Stokes attack (66.7%), the limitation of physical activity (22.2%), heart failure (19.4%), palpitation (19.4%), vertigo (16.7%), anginal pain (13.9%) and dyspnea (8.3%).
    After the implantation of the pacemaker, Adams-Stokes attack disappeared in all the cases. The limitation of physical activity was improved in younger patients, but such improvement was slightly depressed in the older patients. General symptoms caused by heart failure were also disappeared except in the patients with valvular heart disease. Palpitation was observed more frequently in sick sinus syndrome than in A-V block; it was difficult to eliminate this by pacing. Anginal pain, dyspnea and vertigo were disappeared by pacing.

    酢酸ビニル・メタノール・水系の常温における溶解度曲線および液液平衡値を測定し,酢酸ビニルを30%以上含む酢酸ビニル・メタノール混合液は水を抽剤とした連続抽出によってメタノールを含まない酢酸ビニルを収得し得ることを推定し, かつ充填塔,の回転円弧塔によって酢酸ビニル・メタノール混合液および少量のアセトアルデヒド,酢酸メチルを含有する酢酸ビニル・メタノール混合液を抽料とし,水を抽剤として抽出実験を行ない確認した。なお液液平衡を平田法,Othmer法によって検討した結果,いずれもほぼよい直線関係を示すことを知った。

    Hemodynamic observation prior to pacemaker implantation and generator replacement were performed to obtain the details of cardiac function and to adjust the pacing rate in response to the function.
    The pacing rate was determined with reference to maximal cardiac output and lowest intracardiac pressure.
    To observe the rate-output curve, cardiac output was measured by the thermodilution method and the intracardiac pressure was measured with a catheter to 40 paced patients.
    The rate-output curves tended toward a flat tracing and the right atrial pressure and pulmonary arterial end-diastolic pressure did not elavate. During long-term pacing, no congestive heart failure or pulmonary hypertension occured.

    In this paper the relationship between the clinical effect of permanent pacing and the cause of death is discussed.
    Of 63 patients with implanted pacemakers, 8 died. The causes of death were cerebellar hemorrhage, sudden death, unknown cause, senility, pneumonia, cerebral embolism, myocardial infarction and mesenteric embolization. The mortality of our paced patients was 12.7 per cent. Post-operative mortality (within 30 days of surgery) was 1.6 per cent (one case). However, the cause of this death was unrelated to the pacemaker (myocardial infarction).
    When the patients who have died are compared to the living patients, with relation to the average age at the time of initial pacing, we find that the former group was older than the latter. This may be due to natural history.
    Electrocardiographic findings reveal that mortality in the supraventricular arrhythmic group was slightly higher than in the atrio-ventricular block group. In the supraventricular arrhythmic group, systemic embolization as the cause of death was remarkable. This may be due to the irregularity and/or stasis of the blood flow in the atria.
    Hemodynamic improvement with pacing was quite obvious not only i n the living, but also i n the patients who had died.
    After pacing, no Adams-Stokes attacks or death due to congestive heart failure ever occured.
    Although patients derive hemodynamic improvement from pacing, it is no guarantee that they will not die from causes unrelated to the pacemaker. Pacing, therefore, is not always effective in prolonging life.

    The growth of Hiochi-bacteria in SI-media containing antibiotics were investigated. All of the Hiochi-bacteria tested were able to grow in the presence of 20μg/ml of aminoglycoside and polypeptide antibiotics. Both hetero-fermenters of hiochi-lactobacilli and true hiochi-bacilli were able to grow in the presence of tetracycline more than 20μg/ml, but none of the homo-fermenters of the same groups could grow at 5μg/ml of tetracycline.

    The shell side heat transfer and pressure drop to water flowing counter were experimentally investigated on the basis of the overall heat transfer coefficient. The investigation was intended to identify ways to get higher performance for the cooler in a BWR nuclear power plant. The following three conclusions were reached in the study. (1) From estimated performance of the heat exchanger using the overall heat transfer coefficient based on the outside area of the tube K0, performance of this heat exchanger was enhanced 92% as compared with the measured performance of the conventional segmental heat exchanger. Assuming that the fouling factor is Rf=8.6×10-2 m2K/kW, the former was enhanced about 23%. (2) The tube side pressure drop ΔPt=20 kPa and the shell side pressure drop ΔPs=70 kPa were obtained, and they were within the allowable value ΔPa=80 kPa. The shell side pressure drop of the standard spacer could be decreased 20% as compared with that of the low pressure drop spacer. (3) The enhancement constant of heat transfer of the low pressure drop spacer with thin plate type supports was about 1.2 times as large as that of the standard spacer. The heat exchanger with the low pressure drop spacer was about 1.6 times more compact than that using the standard spacer.

    Multiprogrammable pulse generatorの2機種(Programalith 221, 223, 82個, Cyber Lith 253, 53個)計135個を使用した。それらのコード分類は, VVIM 116個, AAIM 11個, DVIM 8個である。VVIMでは, rate 63回, output 16回, pulse width 18回, sensitivity 4回, hysteresis 7回のprogramが実行された。AAIMでは, rate 6回, pulse width 6回, sensitivity 8回, refractory period 1回が実行され, pulse width, sensitivityの頻度が高いのが特徴である。 DVI Mでは, rate 2回, sensitivity 1回が実行された。 Rate, refractory periodの仕様は, ほぼ満足しうるが, output, pulse width, sensitivityには拡張を必要とする部分がある。hysteresisについては, 120 msecで十分と考えられる。

    Recently, the noise caused by the refrigerant fluid is spotlighted due to the demand of low noise home and office air conditioners. Especially, reduction of refrigerant fluid noise and vibration noise generated from throttle in the expansion valve is becoming important. Therefore in this study it is intended to resolve fluctuating phenomena of fluid, which may be the cause of noise, by flow visualization and measurement of fluid pressure and temperature at throttle valves, focusing on two-phase flow of refrigerant fluid. A test equipment suitable for flow visualization of R410a refrigerant was developed by epoch making design and manufacturing method. Visualization was realized by laser beam irradiated on the visualization equipment and by high-speed video camera. Test conditions are set of combinations of 4 different conditions of compressor revolution and 4 different conditions of valve opening of expansion valve. As results of the following conclusions are drawn. (1)A visualization technique of throttle of expansion valve has been developed by manufacturing visualization equipment, which is most suitable to the test. (2)It has been confirmed that refrigerants is liquid and two-phase condition in the upstream of throttle, where a needle is inserted and that refrigerant fluid flow in two-phase in the downstream of throttle.

    The pacing rate must be controlled on the basis of the clinical condition. The programmable pacemaker is available for this purpose.
    Eighty-five patients in our Surgery have undergone implantation of 119 programmable pacemakers (Cordis Omni-Stanicor 162C, D, 190A: 105 generators, Edwards Prolith 215: 4 generators, Intermedics InterLith-RP: 10 generators).
    All programming systems have been fully programmed on the basis of hemodynamic examination, clinical condition, or for the reduction of complaints induced by artificial pacing (e. g. palpitation) and for the purpose of carrying out some clinical examination.
    No programming trouble or interference has been experienced.

    We founded the Intractable Disease Care Center in Shinshu University Hospital in June 2009. By the end of May 2013, we had conducted 304 house-call medical visits responding to the requests of 108 patients with intractable neurological diseases. We have counseled them on their problems and provided medical information to primary care doctors and supporters such as home-care nurses, public health nurses or care managers. We also have arranged hospitalization for respite care. This multidisciplinary medical care has helped patients and their families to understand the disease, resulting in a reduction of anxiety and promoting cooperation among supporters. In arranging for the respite care, it is important to facilitate cooperation between hospitals and public administration based on the status of home care in the local region. To further facilitate the home care, we need to develop a new system to share continually the patientʼs information among medical institutions and supporters, using information and communication technology.

    人工呼吸器等の医療機器は高機能・小型化され在宅利用が進んでいる.しかし,機器のアラーム発生時には迅速な対応が求められることがあるため,医療関係者や介護者家族には少なからぬ心的負担が生じている.また,災害時等に備えて代替機器,バッテリー,酸素の確保が必要である.そこで,在宅医療機器に発生したアラーム情報をネットワーク経由で介護者家族,訪問看護ステーション,医療機器管理会社や医療機関に伝送することにより可及的速やかに事態の収束を目的に,医療機器のシリアルまたはUSBポートから出力されるアラームや機器情報をデバイスゲートウェイを介して高セキュリテイー(SSL)下に置かれた専用クラウドサーバーに伝送し,多地点のPCやモバイル端末へ配信を可能とするシステムを開発した.一方,機器の外部出力情報フォーマットの公開や標準化,異常発生時のリスクマネジメントやアラーム疲労への対策も必要である.

    Recently, there has been an increase in demand for low-noise air-conditioners. The reduction of noise originating from the expansion valve in an air-conditioner is particularly important, since the propagation of noise along the piping system leads to problems for overall low-noise operation. In order to design a low-noise operating valve, it is necessary to understand the flow characteristics of the refrigerant passing through the throttle of the valve. By acquiring knowledge from the visualization of two-phase flow in an old throttle design, we manufactured an expansion valve with a new throttle design that aims for a reduction in noise generation. We compared the performances of the old and new throttle designs by monitoring the refrigerant flow conditions, refrigerant pressures and temperatures, as well as the acoustic noise generation. Flow visualization of the new throttle clearly indicates liquid or bubbly flow upstream; there is no slug flow. These results show a direct contrast to those for the old throttle design, where slug flow can occur upstream under certain operating conditions. There is also a marked reduction in the noise power spectra in the high frequency domain, which is the one that poses particular problems for the human ear.

    Since 1974, 159 programmable pulse generators (53 rate only PPG, 106 rate and output PPG) and 98 multiprogrammable pulse generators have been implanted in 189 patients with symptomatic bradyarrhythmia, ranging in age from 9 to 90 (mean age 61.2).
    The rate programs have been performed for the purpose of hemodynamic improvement, clinical condition, reduction of the complaints induced by artificial pacing, and clinical examination.
    The hysteresis programmability was available for the maintenance of A-V synchrony.
    The hysteresis interval should be programmed over the rate of 55 per minute, because if the A-V synchrony was mainteined, the rate-output curve shaped the plateau over 55 PPM in our investigations

    Rehabilitation problems in fifty-one patients with cardiac pacemakers were discussed.
    Adams-Stokes attacks, observed in fourty-one cases (80.3 per cent), were completely eliminated after pacing.
    Occupational rehabilitation was carried out in twelve cases (55.0 per cent). Housewives and schoolgirls completely recovered.
    The rehabilitation ratio in various types of conduction and rhythm disturbances were; AV block, 50.0 per cent; sick sinus symdrome, 38.0 per cent; atrial fibrillation with bradycardia, 25.0 per cent.
    The physical performance of unemployed paced patients was markedly improved in thirteen cases (65.0 per cent).
    The most significant factors inhibiting the rehabilitation of paced patients were age, and unfavorable social and/or family conditions.

    粗粒材料の過剰間隙水圧の発生特性および動的せん断強度特性を明らかにするため, 礫材質の異なる3種の粗粒労務について, 相対密度, 拘束圧の試験条件を変化させ, 繰返しおよび単調載荷ねじり単純せん断試験, 繰返しおよび単調載荷三軸試験を実施した. その結果, 粗粒材料の過剰間隙水圧は, 密な砂のケースと同様に, 初期の繰返しせん断で急激に上昇すること, 密に締固めた粗粒材料の最大過剰間隙氷圧比は1にまで上昇せず, 有効応力は0まで低下しないことがわかった. さらに, 過剰間隙水圧の発生量と余裕間隙比の関係を明らかにし, 粗粒材料のダイレイタンシー特性と過剰間隙水圧の発生特性, そして動的せん断強度の関係を示した.

    カチオン交換樹脂を触媒として用いる酢酸メチルの加水分解反応の速度は,従来希薄水溶液の場合について研究され,原料濃度の1次に比例する速度式で表わされることが報告されてきた。著者は比較的濃厚な水溶液(濃度9~50mol%)の場合について検討したところ,1次式ではなく次の2次可逆反応速度式がよく適用されることを認めた。

    つぎに触媒を充填した固定層をつくり,流速0.016~0.59cm/secの範囲で固液系の流通式反応を行なったところ,上記速度式に押し出し流れを仮定して積分した積分型反応速度式によって実験結果をよく整理できることが認められた。
    さらに本反応の平衡定数は40~60℃において温度にほとんど影響されず,およそ0.20であることが求められた。
