We report a case of intraosseous lipomas arising in the mandible. A 68-year-old man was referred to our department for evaluation of a radiolucent lesion in the right side of the mandible. The face was symmetric. No swelling of cervical lymph nodes or hyposthesia of the right lower lip was noted. The oral mucous membrane was normal despite many residual roots, and no bone eminence was palpable in the right side of the mandible. Panoramic radiography and computed tomography revealed a unilocular radiolucent lesion measuring 30 x 20 mm at the border of the right side of the mandible. On magnetic resonance imaging, the lesion showed signal intensity as high as that of fat tissue on T1- and T2-weighted images, and the signal intensity was reduced by fat suppression on T2-weighted images. The lesion was diagnosed as an intraosseous lipoma in the mandible and was extirpated via a submandibular approach with the patient under general anesthesia. The surgical specimen was elastic soft and yellowish. Histologically, the lesion consisted of proliferation of lobulated mature adipocytes. Degeneration of adipocytes and fibrosis and calcification were partly observed within the lesion. Nuclear atypia suggesting malignancy was not noted. On the basis of these findings, the lesion was histologically diagnosed to be a lipoma. The postoperative course has been uneventful, with no evidence of recurrence 2 years 10 months after surgery.

症例は50歳，女性．左乳癌の精査中に胆嚢近傍の腫瘍性病変を指摘された．CT・MRIでは膵頭部の腹側に位置しており，膵臓との境界は明瞭であった．動脈相で濃染することから結腸間膜由来のGISTや神経原性腫瘍が疑われた．左乳癌はリンパ節転移を伴っており，術前化学療法の方針となったため，腹腔内腫瘍については慎重な画像的フォローを行った．半年間，腫瘍径に変化は認めなかった．腫瘍摘出術を施行し，パラガングリオーマと診断された．パラガングリオーマが膵頭部前面に発生するのは稀であり，また非機能性であったため症状も認めず，診断が困難であった1例を経験したので報告する．

症例は34歳女性。出産後発熱が持続し, 産褥熱の診断で抗生剤投与されたが改善せず当院に紹介された。当院入院時の腹部CT, MRIにて両側卵巣奇形腫を認めた。左卵巣奇形腫は緊満感欠如, 壁のたるみ・くぼみなどがみられ, 腹膜炎を示唆する腹水貯留・腹膜肥厚も認められた。これらの所見から卵巣奇形腫破裂およびこれに伴う腹膜炎と診断し, 緊急開腹術が行われた。術中所見で術前診断に一致する所見が確認された。両側卵巣奇形腫核出術と腹腔内洗浄が施行され, 軽快・退院した。典型的な画像所見を呈し, 術前診断が可能だった卵巣奇形腫破裂の1例を経験したので, 若干の文献的考察を加えて報告する。

症例は64歳，女性。自宅前で転倒，腹部を打撲し，腹痛を主訴に救急外来受診。腹部全体に圧痛，筋性防御を認めた。腹部造影CTでは，左上腹部から骨盤内に境界明瞭で，内部はやや不均一な濃度を示す径24cm程の腫瘤が描出された。腫瘤上部に内容液の被膜外漏出を疑う所見があり，外傷を契機とした成熟囊胞奇形腫破裂に伴う汎発性腹膜炎と診断し，緊急手術を施行した。開腹所見で巨大な右卵巣腫瘍を認め，腹腔内には脂肪成分に富む腫瘍内容物が流出しており， 術前診断通り外傷性右卵巣成熟囊胞奇形腫破裂であった。左卵巣にも4cm大の同様の腫瘍を認めたため，両側付属器切除術とドレナージを施行した。病理組織所見では，卵巣成熟囊胞奇形腫を背景として偶発的に微小な扁平上皮癌の病巣を認めた。軽快退院後は，婦人科で経過観察中である。術前診断が可能であった外傷性卵巣成熟囊胞奇形腫破裂の1例を経験したので，若干の文献的考察を加えて報告する。

症例は40歳，男性．検診の腹部超音波検査で肝に異常を指摘され，近医を受診した．CTで膵尾部腫瘍を認め当院に紹介となった．造影CTでは膵尾部と左副腎の間に辺縁主体に濃染する腫瘍を認めた．MRIではT2強調像で内部が高信号，辺縁が低信号，造影早期相で辺縁が濃染する所見を認めた．膵尾部と腫瘍の間に介在する組織ははっきりせず，膵原発であると判断した．超音波内視鏡では膵尾部，左副腎と離れて境界明瞭，類円形，内部不均一の低エコー腫瘤を認めた．EUS-FNAを施行し膵神経内分泌腫瘍の診断となり腹腔鏡下膵体尾部切除・脾合併切除術を施行した．病理検査ではparagangliomaの診断となった．膵神経内分泌腫瘍とparagangliomaの画像所見には類似した点があり鑑別が問題となる．また膵周囲に発生した腫瘤の場合，膵原発であるか膵外病変であるのかについて注意深い検討が求められる．

症例は 68 歳の女性。下腹部痛を訴え近医を受診し，急性腹症として当院紹介となった。腹部造影CT検査で右下腹部に類円形の嚢胞性病変を認め，内部に不均一な高吸収域を認めた。周囲臓器への浸潤は認めなかったが，腹腔内全体の腹水貯留を認めた。以上より，右卵巣腫瘍破裂に伴う腹膜炎と臨床診断し，緊急手術を施行した。右卵巣腫瘍の穿孔部からは脂肪と毛髪を混じた黄褐色の液体成分が漏出していた。右卵管と右卵巣動静脈を結紮切離し，右卵巣を摘出した。病理組織診断では，重層扁平上皮や多列線毛円柱上皮に裏打ちされた嚢胞状病変で，壁には脂腺，甲状腺組織を認めた。内腔には角化物と毛髪が充満していた。卵巣成熟嚢胞性奇形腫の診断であった。術後経過は良好で，術後第 8 病日に軽快退院した。

We have developed a speech-driven embodied entrainment characters with eyeball movement model, which consists of an eyeball delay movement model and a gaze withdrawal model. This characters can support the embodied interaction and communication in avatar mediated communication and demonstrate that the line-of-sight plays an important role in enhancing the embodied interaction and communication. In this paper, we propose a concept of eye-contact measurement method and develop an eye-contact measurement system using a dichroic mirror m face-to-face communication for enhancing the embodied interaction and communication. In this system, a dichroic mirror which has a function that transmits visible rays, and reflects infrared rays is arranged between two talkers as a virtual display, and the gaze points on the virtual display are estimated. When the estimated gaze points are present m the eye area of opposite talker, an eye-contact is established in face-to-face communication.

症例は71歳の女性で，肝機能障害の精査のため腹部超音波検査を施行したところ，膵頭部腹側に30 mm大の腫瘤を認め，増大傾向であった．造影CTで，腫瘍は副右結腸静脈へ浸潤が疑われ，横行結腸の辺縁動脈に接していた．また，他臓器と連続性がない多血性腫瘍であり，腸間膜由来の神経内分泌腫瘍（neuroendocrine tumor；以下，NETと略記）または消化管間質腫瘍を疑い，根治切除の方針とした．術中，網囊を開放し観察したところ，腫瘍は隔壁を伴い，横行結腸間膜との連続性は認めなかった．腹腔鏡下に腫瘍摘出を行い，腫瘍血管は右胃大網動静脈より分岐することを確認できたため，胃結腸間膜由来の腫瘍と診断した．病理組織診断ではchromogranin A，synaptophysin，CD56陽性，Ki67陽性率は1%であり，NET G1に相当する所見であった．腸間膜由来，なかでも胃結腸間膜由来のNETは極めてまれである．また，腹腔鏡下術の拡大視効果によって，腫瘍と周囲組織との連続性の有無を詳細に観察することができ，低侵襲な治療を施行できたため，報告する．

症例は13歳女児．左下腹部痛と頻回の嘔吐で発症翌日に前医を受診した．CTで骨盤内に囊胞性病変を指摘され，卵巣囊腫茎捻転を疑われ同日当院へ救急搬送となった．エコー・CT検査より左下腹部から膀胱頭側にかけて径10 cmの充実成分のない多房性囊胞性病変を認め，囊胞成分はecho freeで鏡面形成は認められなかった．腹水を少量認め，卵巣囊腫茎捻転を疑い同日緊急手術を施行した．術中所見で左卵管より突出した囊腫と360°反時計方向の左卵管捻転を認めた．左卵巣の血流障害は認められず，左傍卵管囊腫に伴う卵管捻転と診断し囊腫核出術を施行した．術後経過は良好であり特に合併症を認めず術後7日目に退院となった．今回我々は卵巣囊腫茎捻転疑いで緊急手術を行い，術中所見から傍卵管囊腫に伴う卵管捻転と診断した1例を経験した．

　術後肝動脈仮性動脈瘤はいったん発症した場合，出血を繰り返しその治療に難渋する場合も少なくない．今回，我々は胃癌に対する幽門側胃切除術後に固有肝動脈仮性動脈瘤を形成し，出血を繰り返した症例を経験した．緊急開腹術や血管内治療で出血のコントロールの後，IDC^TM（interlocking detachable coil，Boston Scientific，USA以下，IDCと略記）soft typeを用いて直接仮性動脈瘤を塞栓することで最終的に止血を得た．最近肝動脈仮性動脈瘤に対してcovered stentを用いて止血を行った報告などがみられるが，血管の分岐部付近などでは依然困難なことが多い．今回左右肝動脈分岐部直前に発生した術後固有肝動脈仮性動脈瘤に対しIDCを用いて安全に止血しえた1例を経験したので若干の文献的考察を加え報告する．

The aim of this study is to propose the estimation method for size of useful field of view. In order to examine parameters related to eye movement effective for estimating useful field of view in driving, we measured eye movements and blinks during simulated driving. In the experiment, head and eye movements were measured at the same time to calculate the fixation point along global coordinate. The results shows following: (1) The experimental system was developed to measure the useful field of view in Head-free Condition. It was shown that the bigger the mental workload becomes, the smaller the useful field of view is. (2) The estimated equation shows that the relationship between the useful field of view and the parameters of eye movement was proposed.

Recently, several estimation methods of 3D gazing points have been proposed. Many of these are measured by pupil and corneal reflection with infrared light. This conventional method required special measuring devices for estimating 3D gazing points. We previously developed an eye-gaze detection system by image analysis under natural light. This detection system uses an eye-gaze detection method based on the image analysis for horizontal and vertical eye movements. In addition, this detection system consists of a personal computer and single home video camera. In this paper, we present the new eye-gaze detection method for depth direction. We set the new depth parameter by obtained eye-gaze characteristics both eyes. Also, we proposed a depth direction identification method using it. As a result, depth eye-gaze detection became possible by our horizontal eye-gaze detection using eye images with offline process. This detection system employs input images including both eye areas by single home video camera under natural light. Similarly, horizontal and depth eye-gaze detection became possible by combining with our horizontal direction identification method.

我々は様々な術式に積極的に単孔式腹腔鏡手術（TANKO）を適応してきた．卵巣を含まない孤立性卵管捻転の小児例は極めて稀であり診断に苦慮することも多いが，TANKOが診断及び治療に有用であった小児卵管捻転症例を経験したので報告する．症例は12歳女児．前日から続く腹痛と嘔気を主訴に前医受診し，CTで卵巣腫瘍茎捻転を疑われ当科へ救急搬送された．MRIで右卵管捻転が疑われて緊急手術を施行した．腹腔鏡下に観察したところ，右卵管が720度捻転して黒色に変色しており，右卵管水腫の捻転と診断した．すでに壊死に陥っているものと判断し，TANKOで右卵管摘出術を施行した．術後2日目に軽快退院した．小児卵管捻転に対するTANKOは安全かつ低侵襲で整容性にも優れており，診断及び治療にも有用であった．

Portability of an eye tracking system encourages us to develop a technique for estimating 3D point-of-regard. Unlike conventional methods, which estimate the position in the 2D image coordinates of the mounted camera, such a technique can represent richer gaze information of the human moving in the larger area. In this paper, we propose a method for estimating the 3D point-of-regard and a visualization technique of scanpath under natural head movements for the head-mounted device. We employ visual SLAM technique to estimate head pose and extract environmental information. Even in cases where the head moves dynamically, the proposed method could obtain 3D point-of-regard. Additionally, multiple scanpaths are appropriately overlaid on the scene camera image.

In this study, we conducted an experiment aimed at developing an effective method for measuring the useful field of view when driving while maintaining free visual (eye and head free) conditions with completely unobstructed head and eye movements. In the experiment, the mental workload was changed in a stepwise manner in order to test the hypothesis that the useful field of view becomes narrower as the mental workload (MWL) increases, as well as to verify the accuracy of the proposed method. The results confirmed the validity and accuracy of the proposed method for measuring the useful field of view under completely unobstructed visual conditions.

In this study, we propose a method for generating response utterances which take into account contexts and topics of the dialog by complementing omitted words such as subjects in the input utterances of dialog systems. In order to complement omitted words in the input utterances, an automatic anaphora resolution based on the centering theory is performed. To achieve highly accurate anaphora resolution, we also performed spoken-to-written style conversion based on sequence-to-sequence model using LSTM as a preprocessing. The results of evaluation experiments using NUCC, the Nagoya University Conversation Corpus showed that our proposed complementation method works robustly against errors in spoken-to-written style conversion.
