Design and Implementation by Hospital Pharmacists of a Pharmacotherapy Management Protocol for Outpatients with Type-2 Diabetes

Summary: In this study, hospital pharmacists (hereinafter referred to as pharmacists) working with a medical doctor designed and implemented a pharmacotherapy management protocol to improve treatment compliance in outpatients with type-2 diabetes. Furthermore, we evaluated the factors involved in the clinical efficacy of our protocol. From 96 outpatients undergoing pharmacotherapy treatment for type-2 diabetes, 31 patients who already had >7.5% glycohemoglobin A1c (hereinafter referred to as HbAlc) for more than six months participated in this study. Although the BMI of the participants was not reduced at six months from the start of our study, the number of medication types used by them as well as their HbA1c levels were significantly reduced. As for satisfaction level after treatment, 31 patients (100%) said they found their treatment was beneficial and wanted to use it in the future. Moreover, 30 patients (96.8%) responded that the right medications for them had been provided by the pharmacist. The study results suggest that an appropriate choice of medications based on pharmaceutical knowledge, with the support of a doctor, and guidance on medication adherence by pharmacists were beneficial to diabetes patients with poor glycemic control over the span of our study.
