    Objectives: To clarify the support provided to, and intentions of, municipalities in developing the Community-based Integrated Care System in Japan through the work of public health nurses (PHNs) in public health centers.

    Method: Semi-structured interviews were conducted with nine prefectural PHNs who had engaged in public health nurse activities related to the development of the Community-based Integrated Care System, and the results were analyzed qualitatively.

    Results: The categories summarized by the intentions of prefectural PHNs are support intended to consider support methods, such as “Go to grasp the circumstance of municipalities and related organizations in order to prepare materials for considering support methods that match the characteristics of municipalities” support intended to lighten the burden on municipalities, such as “to visualize the current situation of the region and share it with related organizations in order to create an environment where it is easy for municipalities to construct the Community-based Integrated Care System”, and support intended to foster the independence of municipalities, such as “encouraging related organizations to recognize municipalities as the actors involved in their efforts”, Support intended to make use of the position of public health centers, such as “to support municipalities by utilizing the functions of public health centers in order to fulfill a role that only public health centers can play”.

    Conclusion: Prefectural PHNs coordinated with multiple organizations, empowered municipalities, and demonstrated leadership. Prefectural PHNs consider measures from circumstances and data, and support visualizing and sharing data and their thoughts, to respond to the needs of multiple municipalities.

    Objective: This study aims to clarify what type of activities are effective for disaster mitigation under normal conditions, based on the experience of public health nurses(PHNs)at community comprehensive support centers(CCSC)during the Hokkaido Eastern Iburi earthquake.

    Methods: A semi-structured interview was conducted with 13 PHNs who had experience in activities before and after the disaster, and qualitative inductive analysis was performed on their “contents of normal activities that helped residents’ life and safety during the disaster”.

    Results: Some of the PHNs activities that were seen as effective for disaster mitigation during normal times include the following: 【Sensitize the community to provide disaster mitigation for the elderly】【Foster self-help and mutual assistance for disaster mitigation for the elderly】【Develop activities that link prevention of care for the elderly with disaster mitigation】【Introduce measures to keep the safety of the elderly who are at high risk when disaster strikes】【Provide local disaster measures based on support for the elderly】【Establish a system at the government-level to promote disaster measures】【Develop a community-based activity system to implement disaster measures】.

    Discussion: It is important for PHNs at CCSC to promote community building through self-help, mutual aid, and public assistance to ensure that the elderly can evacuate, and to discuss issues with the government to prevent disaster-related deaths and secondary damage among the elderly.
