Two junior high school girls who had swellingsof the skin over the process spinosus oflumbar vertebra were reported. Besides, one ofthem had knuckle pads of the fingers.
Histopathological study of the lesion revealedthe prolifiration of collagen fiber and mucopoly saccharidein the dermis and subcutaneous fattissue.
In 1982, on the mass examination of the skindisorders of 4838 school students between theage of 4 to 15, we found that about 3.9% boysand 7.6% girls had pigmented round plaqueson the same region. The incidense of themwere highest among the junior high school girls (11.2%).
We think the causative factors were anatomicalcharactaristic of the lumbar region, rapidbody groth and hard school bench.
And we think these lesions are essencially notso different from secondary knuckle pads.

Microbial flora and candidial flora in the lesonsof eczematous dermatitis before and after topicalapplication of corticosteroids were investigated. After topical application of Topsym cream, Staphylococcus aureus decreased in number fromthe lesions of eczematous dermatitis. The decreasingtendency of Staphylococcus aureus wasobserved in both oozing and dry lesions ofeczematous dermatitis. This result was same asin topical application of betamethasone valeratewith gentamicin. Clinical effects of Topsymcream were excellent. From the results obtained, it was consideredthat Topsym cream is applicable on both oozingand dry lesions of eczematous dermatitis inwhich Staphylococcus aureus is cultured.

The skin safety of a new moisturizing cream, H. B. cream, supplied by the Maruho Co., Ltd. Osaka, was investigated with patch test andphotopatch test in 30 normal volunteers and 40 patients with contact dermatitis. H. B. creamcontains polysulfated mucopolysaccharides, whichare known to possess an excellent moisturizingeffect, parabens, thymol and a compoundperfume.
The following materials were tested: A) H. B. cream, B) H. B. cream without any perfume, C) H. B. cream without the main component, mucopolysaccharides, D) vehicle alone, E) 2% of the compound perfume in white petrolatumand F) 5% of the compound perfume in whitepetrolatum.
Open test of the 6 materials in 30 volunteersrevealed that they contain no substance whichinduces any non-allergic contact urticaria. Photopatchtest in 70 subjects showed no significantreactions. It means that the materials wereconfirmed to elicit neither of photourticarial, phototoxic nor photoallergic reactions. However, 4 out of 40 patients with contact dermatitisinduced allergic reactions, two probably fromthymol (isopropyl metacresol), and the remainingtwo from the compound perfume. The resultsof patch test in 70 subjects showed that theskin irritant index of H. B. cream was 6.7 innormal volunteers group and 2.6 in the patientsgroup, excluding the allergic reactions. Thus, the H. B. cream is considered to be a very safetopical cream, except for the allergic reactions.

We reported here a case of multiple lymphadenitistuberculosa seen in the patient of lungcancer (squamous cell carcinoma) which had beenthought to be in remission due to chemotherapy. Cultured bacillus was proved to be Mycobacteriumtuberculosis and to be resistant to almostall tubercolostats (isoniazid, paraaminosalicylate, streptomycin, kanamycin, rifampicin, ethambutol, cycloserine, ethionamide, capreomycin andenviomycin). The patient was dead because of DIC after central nervous system involvementand autopsy could not be done. As T-cell functionabnormality was estimated in the backgroundin this patient, we described the result of lymphocytetrans formation with PWM (pokeweedmytogen), PHA (phytohemagglutinin), PPD (purifiedprotein derivative of tuberculin) and TAP (tuberculin associated peptide) and about ourexperience of usage of transfer factor.

Four cases presenting anaphylactic reactionsincluding urticaria, dyspnea, shock and so ondue to topical application of Hibitane
®werereported. Intradermal test was carried out todetermine the presence of allergic siensitivity tochlorhexidine gluconate which is contained inthe agent and showed positive reactions in allof them. Open patch test was also done in oneof them in order to determine if application ofthe agent on the healthy skin may cause anaphylaxisand resulted in the same responses ata concentration of 1% Hibitane
®50 minutesafter topical application. There were reports in which the agent hadproduced severe adverse reactions, though theywere not common. Because Hibitane
® was appliedat concentrations of 0.2% or more in allcases presented severe anaphylactic reactions, the agent was recommended to be used atconcentrations less than 0.2% when topicallyapplied.

Three patients of pustular psoriasis (von Zumbusch) were treated with high-doses ofintravenous pepsin-treated human immunoglobulin. It was effective in two patients and wasnot in one patient. The effective cases wereadministered when skin manifestations and othersymptoms had been improving. However, theineffective case was administered when theyhad been worsening. It seems that high-doseintraveneous gammagloulins should be administeredwhen skin manifestations and other symptomswere improving.

In a six-year period (1977-1982), 2779 patients in the department of plastic surgerywere patch tested with 0.1% chlorhexidinegluconate and 0.1% benzalkonium chloridewhich are widely used as local antiseptics, andpositive reactions were obtained in 10.3% tochlorhexidine, 12.6% to benzalkonium, and 6.8% to both chlorhexidine and benzalkoniumrespectively.
In one of these patients, the reaction tochlorhexidine was strong positive, but in othersweak reactions were observed and most of thereactions have been considered irritant.
It was suggested that chlorhexidine gluconatewas a safe remedy for prevention of skin infections.

Allergic contact dermatitis from a topicalophthalmic medication has rarely been reported. Pupil is dilated by mydriatic as a rule at Ophthalmic Clinic, so we have performed patch testusing mydriatics mainly, and antibiotics, too. In seven patients who complained abnormalreaction to eye medications, two showed positivepatch test reactions to phenylephrine hydrochloride. However, in prospective patch test, therehas been only one of 115 patients with cataract, who shows positive reaction to Mydrin-P, and hehas proved to show positive to the ingredient, benzalkonium chloride.

Skin irritancy of topical agents was studiedby 48 hours closed patch test. First of all, 69topical agents were patch tested to 241 patientswith dermatoses and evaluated according to the Japanese standards. From the results, the reactionpatterns of the irritancy of topical agentswere divided into 6 classes according to the degrees of the strongest reactions and time coursechanges of the reactions. The characteristics ofthe irritancy of topical agents seem to beclarified to some extent by our classification of reaction patterns. Secondly, 25 topical agentsand their vehicles offered by some pharmaceuticalcompanies were patch tested to 33 patientswith dermatoses to study skin irritancy andinteractions between active ingredients and theirvehicles. The results showed that the to picalagents including high concentrations of propyleneglycol reacted stronger at 72 reading than 48reading. The cause was assumed that irritantreactions of the vehicles became stronger at 72 reading, while the active ingredients or corticosteroidswere rapidly absorbed percutaneously.

6600 patients with skin diseases had visitedour clinic and 133 patients out of them sufferedfrom topical medicaments contact dermatitis. Causative remedies were anti-fungous (37%) corticosteroidals (16.1%), eye lotions (8.8%). acne ointments, stupes, disinfectants and so onin accordance with the numbers of cases.
The ways with which patients had taken theremedies causing contact dermatitis were doctor'srecipes (53.3%), over the counters (37.6%) and souvenirs (4.5%).
The patch test positive rate with ingredientsof topical medicaments were as follow.
Hydrogenated lanolin 37.5%, lanolin alcohol 25.0%, chlorhexidine 33.3% and neomycin 20.0%.

From September 1979 to September 1982 atotal of 67 patients were patch tested under thediagnosis of contact dermatitis from topicalpreparations at the Department of Dermatology, Sakai Municipal Hospital, Osaka, Japan. 46 Patients showed positive reactions to topicalpreparations, 4 patients showed irritant reactionsand 17 patients showed negative reactions. 144 cases suffered from contact dermatitiswith daily necessaries and household commoditiesvisited our clinic in the past three years.
We had more patients in spring and summerthan in fall and winter. We thought the high frequency in spring andsummer was due to the allergic reaction withdecorative accessories.
The patient's age group had 2 peaks of under 10 years and between twenties and forties.Most of the male patients were included in aage group of under 10years. But female patientswere distributed in various age groups.
The causative materials of contact dermatitiswere as follow; clothings in 51 cases, householdchemicals in 41 cases, decorative accessories in38 cases, and others in 18 cases 4 cases wereinduced by overlapped materials. In male casesclothings was the first cause and in femalecases household chemicals was the first. Contactdermatitis with clothings was thought to beinduced by primary irritant reaction and decorativeaccessories by allergic reaction.
In most cases household chemicals causedhand dermatitis including keratodermia tylodespalmaris progressiva.

A 60-year-old man, first visit on June 8, 1980, was given intramuscular injection ofribostamycin (Vistamycin (R)) for otitis media.Then he had erythematous eruptions on thesun-exposed areas. Patch test and intracutaneoustest were carried out with several kinds ofaminoglycosides.
Patch tests showed positive reactions withribostamycin, streptomycin, kanamycin, amikacinand fradiomycin.
Intracutaneous tests showed positive reactionswith ribostamycin and kanamycin.
Drug eruptions due to aminoglycosides are notcommon, especially due to ribostamycin is veryrare. In the Japanese literature, there havebeen six reports (seven cases) on this subject.

7 cases with melanosis faciei feminae visited our clinic from December 1981 to November 1982. 5 cases out of the 7 cases were patch tested with the cosmetics which they had used and 137 allergens which were thought to be contained in these cosmetics.
2 cases out of patch tested 5 cases showed positive reactions to cosmetics and both of 2 cases reacted to tar dyes R-219 and PAN. Benzyl salicylate was reacted, too. But Irg CF
3 was not reacted in any case.
In the cases which were caused by the chemicals except cosmetics, we should have coresearch with labor hygienists and social doctors.

The following score was given to each grade of skin response in patch tests, respectively: 0 to -, 0.5 to ±, 1.0 to +, 2.0 to ++, 3.0 to+++ and 4.0 to ++++. The sum of total scores in a cosmetic group or preparation was divided by the number of the test subjects to be expressed in percent. This value is termed the “Skin Irritant Index” of the test material. Various commercial cosmetic products provided by patients with cosmetic dermatitis were patch-tested and photopatch-tested since 1975. This paper refers to the comparison of skin safety in the cosmetic products between the years 1975-1976 and 1981-1982.
The skin irritant index of cosmetic products was found to decrease by one fourth in 1981-1982 than in 1975-1976. The incidence of allergic reactions of make-up preparations was found to decrease markedly in 1981-1982 than in 1975-1976, but that of basic cosmetics to elevate slightly in 1981-1982 than in 1975-1976.
In addition, cosmetic products made by the 6 representative companies was found to be much safer than ones made by the other cosmetic companies.

An attempt to clarify the allergenicity of a compound perfume “Fleuri 981SA, Lot No.31987. 7802”, made by the Firmenich Co., Switzerland, was undertaken. Because it was ranked as the 12th worst substance out of the whole registered standard allergens and as the 5th worst one out of the fragrance materials at our clinic from patch test results in 1981. The incidence of positive reactions of the Fleuri 981 SA in patch test was 10/225 (4.4%) in 1981. This lot of compound perfumes is said to be added three other fragrance materials, that is, hydroxycitronellal, oil of ylang ylang and geraniol. Fleuri 981SA without any additional fragrance material just showed a positive ratio of 1/162 (0.6%) in simultaneous patch tests.
Thus, the incidence of positive reactions in patch tests was compared between 23 patients sensitive to Fleuri 981SA with additional perfumes and generally tested population about the 3 fragrance materials and benzyl salicylate as a control. It was found that the frequency of positive reactions to hydroxycitronellal and oil of ylang ylang was significantly higher in the former than in the latter.
The results obtained suggest that Fleuri 981SA will accelerate a percutaneous absorption of hydroxycitronellal and oil of ylang ylang as a vehicle, showing much higher sensitivity than the original base perfume.

The incidence of positive reactions to 31 fragrance materials in patchtests from January to August in 1982 was reported, in addition to the summarized data from 1974 to 1981. The worst 7 materials from the results obtained in 1982 are the following: a compound fragrance L 8.9% (11/124), perfumes mixture recommended by the ICDRG 8.3% (25/300), isoeugenol 8.0% (2/25), oakmoss 7.1% (3/42), benzyl salicylate 6.5% (13/200), jasmin absolute 6.0% (12/199) and another compound fragrance M 5.2% (6/115). It is noticed that the incidence of positive reaction with oil of ylang ylang dropped marke dly from 6.0% (83/1, 368) to 1.5% (3/198).
Four representative cases with contact dermatitis from fragrance materials were also reported.

The perfumes mixture recommended by the ICDRG in 1975 contained 2% of cinnamic alcohol, cinnamic aldehyde, eugenol and isoeugenol in each, and 4% of hydroxycitronellal. Cummulative data of patch test results at our clinic showed that the frequency of positive allergic reactions is 71/1, 401 (5.1%) to hydroxycitronellal, 51/1, 108 (4.3%) to cinnamic aldehyde, 52/1, 309 (4.0%) to cinnamic alcohol, 18/1, 047 (1.7%) to eugenol and 9/152 (5.9%) to isoeugenol. Four out of the 5 ingredients were included at the worst 20. Thus, it was investigated whether the perfumes mixture is available for screening of hypersensitivity to the 5 isolate fragrances or not.
The incidence of positvie reactions to the perfumes mixture is 111/1, 102 (10.1%) up to now, and it was ranked as the worst first in 1980 and 1981. Twenty out of 749 patients, who revealed no reactions to it, showed positive allergic reactions to either one or more of the 5 ingredients. Thus, the frequency of false negative reactions is counted 20/749 (2.7%). Eight of 63 patients, who revealed positive reactions to the perfumes mixture, showed no reactions to anyone of the 5 ingredients. Thus, the frequency of false positive reactions is counted as 8/63 (12.7%).
Therefore, it was estimated that the perfumes mixture was useful as one of standard allergens for screening the hypersensitivity to fragrance materials.

Cefroxadine (CXD, Oraspor
®capsule), a cephalosporin derivative antibiotic was used in the various moderate and above skin infections and pustular acne, and its clinical efficacy and antibacterial potency against isolates from the foci were examined.
Twenty four patients (15 males and 9 females between 13 and 68 years of age) were subjected to the trials and their diagnostic breakdown showed, 11 cases of multiple folliculitis, 1 case of furuncle, 2 cases of furunculosis, 1 case of carbuncle, 1 case of phlegmon, 3 cases of pustular acne, and 5 cases of secondary-infected trauma.
The daily dosage of 1000mg was divided into 2 fractions and was taken after breakfast and after supper.
Its efficacy was evaluated in the 24 cases and was markedly effective in 7 cases, moderately effective in 11 cases, slightly effective in 5 cases, and in-effective in 1 case, with the effective rate being 75%.
The clinical isolates from the foci were mostly
S. epidermidis and
S. aureus, and the antibacterial potency of cefroxadine against these bacteria was somewhat superior to that of cefalexin. Side effects i. e., elevation of GOT and GPT values were recognized in 1 case.

The results of clinical trial of 21 cases using a single intravenous injection of Restamine calcium KOWA confirmed that the treatment is more effective to urticaria.
Remission in 61.9%, marked improvement in 28.6% and moderate improvement in 9.5% were observed. no adrverse reaction was observed.
