     In the super-aging society, there are many patients whose renal function is deteriorating due to aging and adjustment of an appropriate dose of medicine according to the individual renal function is required. However, it is difficult for pharmacists to adjust and administer the individual dose for all patients due to such as a lack of manpower. In order to prevent medication complications caused by renal deterioration, we developed a medical information system that extracts renal function (eGFR value) and suggests an appropriate dose of medicine for inpatients whose renal function requires attention.

     In addition, we compared four items introduction of the system: the number of patients, the number of prescriptions, the number of prescription questions, the number of prescription changes after prescription questions.

     As a result, improvements were observed in all items. In particular, the number of prescription questions was increased about 5 times, and the number of prescription changes was increased about 3 times.

     This survey objectively showed the usefulness of the system and contributed to the prevention of the occurrence of side effects due to excessive dose of medicine.
