    We compared the results of induction chemotherapy (IC) and radiotherapy (n= 19), and those of radiotherapy (n = 24) in cases of nasopharyngeal carcinoma. We used CDDP and PEP for induction chemotherapy.1) Cases in which clinical effects of IC were poor tended to have poor results.2) IC contributed to improvement in Ti to T3 cases, but did not contribute to improvement in T4 cases.3) IC caused metastatic lymph nodes to become mobile in some cases and thus facilitating neck dissection.4) IC did not prevent distant metastasis.5) The five-year cumulative survival rate of patients receiving IC was 54% in Ti to T3 cases, and 0% in T4 cases.6) IC improved the results of treatment in nasopharyngeal carcinoma except in T4 cases.

    We reviewed the results of 19 cases of advanced carcinoma of the tongue treated by extensive surgery and reconstruction followed by radiotherapy between 1981 and 1990.1) Following a 3-year follow-up, the 3-year control rate of primary lesion was 100% in T2 and T3 and 60% in T4 cases.2)The 3-year control rate of neck metastasis was 100% overall.3) Posttherapeutic speech intelligibility and swallowing problems were related to the degree of resection.4) Following a 3-year followup, the five-year cumulative survival rate was 75% in T2,47% in T3,40% in T4 and 51% overall.

    We reviewed the results of 74 cases of carcinoma of the tongue treated by laser alone or combined laser and radiotherapy between 1980 and 1990. The purpose of this treatment was to minimize post-therapeutic functional disorders without lowering the cure rate. Lesions located only in the mucosa were excised by CO2 laser (Group 1). Lesions involving the muscle but confined to one forth of the transverse dimension of the tongue were treated with laser excision followed by chemoradiotherapy (Group 2).1) Following a 3-year follow-up, of the 17 patients in Group 1, one had died of intercurrent disease, while 16 survived and were free of disease.2) Of the 57 patients in Group 2, one had died of primary lesion,4 died of neck metastasis,3 died of lung metastasis and 11died of intercurrent diseases. Thirty-eight patients survived and were free of disease.3) The fiveyear cumulative survival rate was 94% in Group 1,69% in Group 2,88% in Ti cases and 72% in T2cases.4) Posttherapeutic speech intelligibility was high in the majority of patients.5) There were no swallowing problems following therapy.

    We reviewed the results of 7 cases of advanced carcinoma of the lateral wall of the oropharynx treated by extensive surgery and reconstruction followed by radiotherapy from 1981 to 1990.1)Following the 3-year follow-up, the 3-year control rate of primary lesion was 100% in T2,50% in T3and 75% in T4 cases.2) The 3-year control rate of neck metastasis was 100% overall.3) Posttherapeutic speech intelligibility was related to the degree of resection.4) Regarding posttherapeutic swallowing function, all of the patients ate normal food or gruel with or without occasional aspiration.5) The five-year cumulative survival rate was 43% overall.

    We reviewed the results in 27 cases of carcinoma of the oropharynx (lateral wall) treated by laser alone or combined laser and radiotherapy between 1982 and 1990. The purpose of this treatment was to minimize post-therapeutic functional disorders without lowering the cure rate. Lesions located only in the mucosa were excised by CO2 laser. Lesions involving the muscle were treated with laser excision followed by chemoradiotherapy.1) During a 3-year follow-up, the 3-year control rate of the primary lesion was 100% in Ti and T2 cases, and 66% in T3 and T4 cases.2) Of the 27patients,3 died of primary lesion,2 died of neck metastasis and 7 died of intercurrent diseases. Fifteen patients survived and were free of disease.3) The five-year cumulative survival rate was 100%in Ti,60% in T2,50% in T3 and 33% in T4 cases.4) Posttherapeutic speech intelligibility was high and there were no significant swallowing problems in the majority of the patients.5) Posttherapeutic speech intelligibility decreased and there were some swallowing problems when more than half of the soft palate was resected.

    Pharyngolaryngectomy and esophagotomy with bilateral extended radical neck dissection and upper mediastinal dissection combined with free jejunum transplantation was performed for fortytwo patients with hypopharyngeal carcinoma. The three-year survival rate as determined by the Kaplan-Meier method was 58.9%. Survival rate of patients with neck metastasis was better than that of patients without neck metastasis. Patients who underwent post-operative irradiation showed a better survival rate than patients without irradiation. As for complications,12% of patients showed fistula-formation,12% showed abscess-formation around the tracheostoma,7% died from surgery and complications, and 5% showed total necrosis of the transplanted jejunum.
    Stenosis around the anastomosis tended to decrease over time. These results suggest that this surgical method is effectively increased the survival rate for hypopharyngeal carcinoma, although further improvement is necessary to reduce the rate of distant metastasis and lymph node recurrence.

    Pharyngolaryngectomy with staged pharyngoplasty using local and delto-pectral (DP) flaps was performed in thirty-eight patients with hypopharyngeal carcinoma. The three-year survival rate as determined by Kaplan-Meier method was 32.6%. As for complications, after pharyngolaryngectomy 13% of patients showed advanced necrosis of the neck skin and 26% showed partial necrosis of the neck skin. After pharyngoplasty,45% of patients showed some fistula-formation and seven patients underwent subsequently a second pharyngoplasty using another DP flap. These results suggest that this procedure should be used when jejunum transplantation is impossible or when transplated jejunum becomes necrotic.

    A retrospective review of 184 patients of Ti glottic carcinoma treated by CO2 laser or radiation at Kurume University Hospital between 1971 and 1990 was completed. The following results were obtained:
    1. The 5-year cumulative survival rate calculated by the Kaplan-Meier method was 99%.
    2. Indication for CO2 laser surgery are Tis and Ti cases.
    3. The 3-year primary control rate of carcinoma extended to the anterior commissure was low. In cases involving carcinoma extended to the anterior commissure, CO2 laser surgery should not be indicated.

    A retrospective review of 53 patients with glottic carcinoma treated by hemilaryngectomy at Kurume University Hospital between 1971 and 1990 was completed. The following results were obtained:
    1. The 3-year primary control rate of hemilaryngectomy for glottic carcinoma was 88%.
    2. The 3-and 5-year cumulative survival rates calculated by the Kaplan-Meier method were 98%and 96%, respectively.
    3. In T2 cases,3-year primary control rate was 84% and the 5-year cumulative survival rate was 94%. In T3 cases, an 84% 3-year primary control rate was obtained and the 5-year cumulative survival rate was 100%.
    4. Indication for hemilaryngectomy is T2 glottic carcinoma. Hemilaryngectomy can be performed in some T3 cases which does not extend to the subglottic region beyond 1 cur.

    A retrospective review of 58 patients of supraglottic carcinoma treated by supraglottic horizontal laryngectomy at Kurume University Hospital between 1971 and 1990 was completed. The following results were obtained:
    1. The 3-year primary control rate by supraglottic horizontal laryngectomy was 85%.
    2. There was no difference in the 3-year primary control rate between Ti or T2 and T3 or T4 lesions.
    3. The 3- and 5-year cumulative survival rates calculated by Kaplan-Meier method were 82% and 71%, respectively.
    4. Supraglottic horizontal laryngectomy is indicated for T2 carcinoma, but can be used to treat some T3 or T4 cases.
    5. Controlling of neck metastasis is the most important issue in treating supraglottic carcinoma.

    Ten cases of carcinoma of the external auditory canal are presented. Patient ages ranged from 44 to 84. Six patients were male, and four patients were female. Seven cases involved squamous cell carcinoma,1 was adeno-squamous cell carcinoma,1 was adenoid cystic carcinoma, and 1 was basal cell carcinoma.
    Four cases were treated by en bloc dissection of the external auditory canal alone. Seven cases that extended to the middle ear were treated by surgery and radiation.
    Nine patients have survived due to this treatment.
    Improved results are expected from treating carcinoma of the external auditory canal by this modality.

    We performed anterior skull base surgery in 11 cases of head and neck cancer between 1984 and 1993. 1) Diseases included frontal sinus cancer (n=2), ethmoid sinus cancer (n=3), olfactory neuroblastoma (n=1), nasopharyngeal cancer (n=1) and nasal cavity cancer (n= 4). 2) The 3-year cumulative survival rate was 78% and 5-year cumulative survival rate was 65%.3) It has become possible to perform radical surgery for head and neck cancer invading the anterior skull base.

    We performed skull base surgery in 4 cases of benign head and neck tumors between 1990 and 1992. 1) Diseases included a schwannoma of the ethmoid sinus, a schwannoma of the parapharyngeal space, a chordoma and a papilloma of the ethmoid sinus. 2) Anterior skull base surgery was performed for schwannoma and papilloma of the ethmoid sinus, and middle skull base surgery was performed for chordoma and schwannoma of the parapharyngeal space. 3) It has become possible to perform radical surgery for benign head and neck tumors invading the skull base.

    We reviewed the results of 28 cases of thyroid cancer (papillary carcinoma and follicular carcinoma)invading adjacent organs that were treated between 1981 and 1990.1) 19 cases had invaded the esophagus,13 cases the trachea and 6 the larynx.2) In cases of laryngeal invasion, total laryngectomy was performed in 3 cases and partial resection of the cricoid cartilage was performed in the other 3 cases.3) In cases of tracheal invasion, partial resection and reconstruction of the trachea were performed in 10 cases and tracheal stoma was created in 3 cases following total laryngectomy.4) In cases of esophageal invasion, partial removal of the muscular layer of the esophagus was performed in 19 cases.5) One case died from primary lesion and one died from neck metastasis. Both of these cases involved anaplastic change of the carcinoma.6) Both of the fiveyear and 10-year cumulative survival rates were 70%.

    A retrospective review of 196 patients with carcinoma of the maxillary sinus treated at Kurume University Hospital between 1971 and 1990 was completed. In Kurume University Hospital, the following two treatment methods are used for carcinoma of the maxillary sinus: (A) infusion, radiation, and necrotomy followed by eradicating surgery and (B) infusion, radiation and necrotomy associated with mass reduction surgery. The following results were obtained:
    1. In 196 patients, the 5-year cumulative survival rate calculated by the Kaplan-Meier method was 60%. The 5-year survival rate was 100% in Ti cases,88% in T2 cases,69% in T3 cases and 27%in T4 cases.
    2. There was no difference in cumulative survival rates between method A and method B in Ti, T2, T3 cases, but in T4 cases, the 5-year survival rate was better following method B than method A.
    3. The cause of the death was the primary lesion in 42, neck metastasis in 7, distant metastasis in 11, surgical complication in 6, other complication 6, unknown in 2 and concurrent diseases in 2 5cases. Controlling the primary lesion is the most important issue in treating carcinoma of the maxillary sinus.

    A case of carcinoma of the sphenoid sinus treated by induction chemotherapy and radiotherapy is reported. The patient was a 28-year-old male. He complained of double vision, hearing loss and headache since October 1984. On CT scan, the sphenoid sinus was filled with tumor. Exploratory sphenoidotomy was performed on February 4,1985. Histological examination revealed poorly differentiated squamous cell carcinoma. We administered induction chemotherapy (CDDP 150 mg, PEP 60 mg) twice. Partial remission was achieved after induction chemotherapy, and left abducent nerve paralysis was improved. Radiotherapy (60 Gy) and necrotomy of the tumor in the sphenoid sinus were performed. The patient has survived without any evidence of disease for 9 years after treatment.
