    Eighteen young adults were investigated as to their hearing level before and after exposure to loud sounds at a rock concert, and were also questioned with respect to cochlear symptoms.
    They were divited into 3 groups of 6 subjects each. Group 1 heard the music near the stage, Group 2 in the middle front row, and Group 3 in the middle row.
    Four of six persons in Group 1 showed a threshold shift of more than 20 dB especially in 4 KHz in the left ear,2 of them recovered in 2 hours, and the rest recovered in 12 hours. Significant threshold shifts of 4 KHz in Groups 1 and 3, and of 4 KHz and 8 KHz in Group 2 were observed.
    The cochlear symptoms generally diminished within 30 minutes after exposure to loud sounds. All Group 1 subjects complained of cochlear symptom.
    No correction was seen between those with hearing shift of more than 10 dB or 20 dB and those with cochlear symptoms. Thus, hearing damage induced by loud sounds at rock concerts is mainly a temporary threshold shift around 4 KHz.

    Rock music at a concert was recorded by a data recorder at a point 15 meters from bilateral speakers for 50 minutes. These sound levels were analysed by a sound level recorder and octave bands were analysed by an octave band analyzer. Sound pressure level was 100-110 dB SPL, and dynamic range was about 10 dB. Leq was 104 dB. High energy was observed in lower frequencies such as 62.5 Hz and 125 Hz, while lower energy was observed in higher frequencies such as 4 KHz and 8 KHz. There was evident discrepancy between the frequencies that showed high energy by octave band analysis and the frequencies that showed threshold shift by the hearing test.

    Eye movements induced after unilateral electrical stimulation to the mesencephalic reticular formation, rostral portion of the pontine reticular formation and surrounding area were investigated in thirty adult pigmented rabbits. Under pentobarbital anesthesia, electrical stimulation of pulse trains was applied through a bipolar stainless steel electrode inserted stereotaxically in the brain.
    The majority of the effects were horizontal conjugate deviations, which were usually ipsilateral to the mesencephalic reticular formation, but contralateral to the rostral portion of the pontine recticular formation.
    At the level of the oculomotor nuclei in the mesencephalic reticular formation, horizontal deviations were elicited to either side. It appears that oculomotor decussation for horizontal eye movement occurs at the level of the oculomotor nuclei in the mesencephalic reticular formation in rabbits.

    Inverted nustagmus often occurs after hot air caloric stimuration in an ear with tympanic membrane perforation or moistend ear cavity without disorder of the vestibular system. To this inverson, we calculated the heat value transferred during the air caloric test, estimated the temperatures that yield a heating of +20.5 cal or a cooling of -20.5 cal using dry or full humidified air in dry or wet ear cavity.
    This calculation of heat value and temperature was done under the assumption that the heat transfer coefficient is 100%; thus, the data do not represent the actual amount of heat that is transferred but is only the relative change in temperature.
    When the ear cavity is dry, a commercially available air caloric stimulator provides reliable test data. However, when the cavity is wet, the effect of cooling by evaporation must be taken into consideration.

    The two-dimensional electrophoresis was performed to analyze murine plasma proteins, in mice that were transplanted 20-Methylchoranthrene induced sarcoma in head and neck tissues (tongue, submandibular gland and hypopharyngeal area). There were significant differences in plasma levels of the proteins, compaired to several treated mice one another. In mice with sarcoma transplanted into the salivary gland, transferin, fibrinogen, α1-antitrypsin, haptoglobin, high density glycoprotein, platelet actin levels were higher, whereas low density glycoprotein levels were lower than the levels in the control mice. In mice with sarcoma transplanted into tongue, fibrinogen, hemopexin, haptoglobin and high density glycoprotein levels were higher than the levels in the control mice. In mice with sarcoma transplanted into the hypolaryngeal area, fibrinogen, hemopexin, α1B-glycoprotein, arantitrypsin, haptoglobin, platelet actin and high density glycoprotein levels were higher, but low density glycoprotein levels were lower than the levels in the control mice. New proteins which would be caused by sarcoma growth could not be found out in this study, without understanding which reason, there were completely lack of such substances as tumor associating factor or the volume was undetectable by our used technique.

    The two-dimensional electrophoresis was used to analyze of plasma proteins in patients with head and neck tumor. In patients showing improvement, transferin, α2-HS-glycoprotein and high density glycoprotein showed parallel changes. The levels of these three proteins were increased at the convalescent stage and decreased during relapse. The α2-HS-glycoprotein level seemed to be a useful index of prognosis. In contrast, among patients whose conditions became aggravated, no such proteins's parallel changes in these protein leveles were observed. However, arantitrypsin and haptoglobin showed parallel changes, between plasma protein levels and clinical condition of tumor-bearing patients. Tomor associating factor could be found out in this study, without understunding the reason why such substance is completely lack in plasma proteins of patients with head and neck tumor or undetectable in volume by our used tecknique.

    A 61-year-old female visited our hospital complaining of Vertigo.
    She had a past history of pigmentary degeneration of the reti n a, neurosis and cataract. Vestibular test was perfomed by erectronystagmography. No spontaneous nystagmus was observed when her eyes were open, while periodic alternating nystagmus appeared when her eyes were closed or opened in the dark. The average right-beating duration was 11.0s, left-beating duration 15.9s.
    The horizontal optokinetic nystagmus w as inhibited bilaterally. The eye tracking test showed saccadic pursuit. The visual suppression of caloric nystagmus was normal. In this patient, no diagnosis could be definitely established.

    Symptomatologic and neurological features of two cases of trigeminal neurinoma are reported.
    Case 1. A 22-year-old male, complained of left facial disturbance and hypogeusia. Neurological examination revealed damage of the Vth cranical nerve. Cerebellar symptoms were noted. His functional examination of equilibrium showed Bruns nystagmus, direction changing positional nystagmus, saccadic pattern on ETT, hyporesponse on OKP and OKAN, and positive visual supression test. Enhanced CT scan disclosed a mass in the left cerebellopontine angle region. The cystic mass was removed by the subtemporal approach, and diagnosed as trigeminal neurinoma.
    Case 2. A 36-year-old female had difficulty in walking. Neurolog i cal examination reveraled damage of the Vth cranical nerve. Cerebellar symptoms were noted. Her functional examination of equilibrium showed right spontaneous nystagmus, Bruns nystagmus, direction changing positional nystagmus, saccadic pattern on ETT, hyporesponse on OKP and OKAN, and positive left visual supression test. Enhanced CT scan disclosed a ring enhanced mass in the left cerebellopontine angle region. The cystic mass located at the posterior fossa was removed by the left retromastoid suboccipital approach, and diagnosed as trigeminal neurinoma.

    We re-evaluated 6 patients with ethmoidal cancer in the head and neck region between who had been treated in our clinic 1977 and 1985. Histologically,4 cases were squamous cell carcinoma and the others were anaplastic cancer. At the clinical stage, infiltration to the bone of the basal scalp and orbit was observed in 5 cases. The patients were treated by radiation followed by surgery. Some patients who were suspected of having residual cancer received post-operative radiation. Two patients showed complete remission, but the others died of cancer invasion. The cause of death was considered to be due to insufficient dissection of the basal scalp. We concluded that surgery is required in the treatment of ethmoidal cancer. Thus, it is necessary to approach the lesion from not only the nasal side but also from the scalp when the lesion infiltrates the scalp.

    Between 1975 and 1984,47 patients (males: females =26: 21) were hospitalized for the treatment of airway and esophageal foreign bodies.
    Foreign bodies were found in the airway of 17 (m ales: females = -10: 7) patients and in the esophagus of 30 (males: females =15: 15) patients.
    Thirteen of the patients with airway foreign bodie s, were one-year-old babies, and the oldest patient was a six-year-old male.
    In eight of the 17 cases (47%), peanu t s were found. Radiopaque foreign bodies were found in 16 cases (94%). The obstruction was found in the right bronchi in 12of the 17 patients (71%), in the left bronchi in 3 and in the larynx in 1.
    Among the patients with esophageal foreign bodies, coins were found in ten children 33% under 10 years of age, and dental prosthesis were found in seven adults (23%). The obstruction was found at the first constriction in 21 patients (70%), seven cases at the second constriction in 7 (23%), and at the third constriction and stomach in one patient each. The findings indicate the importance of preventing airway foreign bodies in the infants.

    A statistical review was made of the 726 patients with otolaryngological diseases observed at the Department of Emergency and Critical Care Medicine of St. Marianna University Hospital between July 1,1981 and January 31,1985.
    The Patients'ages showed a wide range, but about 40% were children under ten years with nasal bleeding and acute otitis media.
    Most patients we brought to the depar t m ent on the day after before holidays and at night.
    One hundred fifty-four patients (36.7%) were brought to the department by ambulance most patients had common illness that did not require emergency admission and their symptoms were mild.
    One hundred six pati e nts (14.6%) required emergency admission, because of extensive nasal bleeding, obstruction by a foreign body, vertigo and malignant neoplasma.
    The present findings may be useful in improving the quality of critical care medicine.

    A 65-year-old women was admitted to our hospital with complaints of dizziness, hearing loss and tinnitus.
    The hearing test revealed left sensorineural hearing loss.
    Furthermore, the patient exhibited Bruns-Cushing n y stagmus, canal paresis and directional preponderance of nystagmus caloric response in the left ear, strong inhibitory nystagmus on the optokinetic test, saccadic eye movement on the eye tracking test, but no cranial symptoms, except for impaired balance and nystagmus. These finding indicated the presence of a cerebellopontine angle tumor.
    Stenverse view and temporal polytomogram showed enla r gement of the left jugular foramen without enlargement of the left internal auditory canal.
    Computed Tomography and vertebral angiogram demons t rated a space occupying lesion at the left cerebellopontine angle. Retromastoid craniectomy confirmed a large tumor that covered the left jugular foramen and foramen magunum. The tumor was encapsuled with a thin smooth capsule.
    The histological diagnosis was neurinoma. We suspected that this tumor arose from the XIth cranial nerve.

    We experienced two cases of facial nerve schwannoma. Case 1 was a 55-year-old female who complained of right facial palsy and right otalgia. She was diagnosed as having intratemporal facial nerve schwannoma. Case 2 was a 44 year-old female with mass in the left parotid gland. She was initially diagnosed as having a mixed tumor, but the pathological diagnosis was schwannoma.
    Patients with intratemporal facial nerve schwannoma usually exhibit gradually developing facial palsy and sometimes recurrent facial palsy; these symptoms are similar to those in Bell's palsy, thus, these patients may be initially misdiagnose as having Bell's palsy.

    More than 3000 students were questioned to assess their awareness of danger of auditory damage caused by rock'n roll music.
    Male university students were aware of the danger of auditory damage caused. by headphones, high school students knew about the danger of auditory damage associated with rock concerts and female high school students knew about the danger of auditory damage associated with discos.
    Headphones were most frequentl y used by male highschool students, and the sound level was higher in male students than in female students. A relationship was found. between the sound level and the incidence of cochlear symptoms after use of headphones.
    University students went to rock concerts more frequently than high school students and the more often they went to rock concerts, the higher became the incidence of cochlear symptoms.
    University students, especially female students, went to discos more frequently than others. The often they went to discos, the higher became the incidence of cochlear symptoms, which disappeared in 30 minutes.
    The total amount of time that the students were exposed to loud music was correlated to the extent of cochlear symptoms. Thus, students must be made more aware of danger of auditory damage caused by loud rock'n roll music.

    We experienced a case of malignant fibrous histiocytoma in a 67-year-old man who initially had swelling in the left upper gingival region followed by rapidly extensive growths. He died 3 montsh after the treatment consists of subtotal maxillectomy, postoperative irradiation and chemotherapy with distant metastases to lung, liver and bone marrow. This tumor was difficult to diagnose from the histological findings. Such patients require extensive removal of the tumor, post-operative radiotherapy and combination chemotherapy.

    Twenty-nine laryngeal lesions including 3 hyper-plastic lesions,10 vocal cord polyps,4 dysplastic lesions,6 non-keratinized squamous cell carcinomas, and 6 keratinized squamous cell carcinomas were stained with horseradish peroxidase conjugated lectins, for histochemical studies of the cell membrane.
    Both PSA and DBA failed to stain these tissues. WGA, UEA-1, and RCA-1 were demonstrated in all benign lesions, whereas in dysplastic tissues and squamous cell carcinomas, a loss of cell membrane stining was noted in various degrees, especially in non-keratinized squamous cell carcinomas.
    None of the dysplastic tissues and squamous cell carcinomas were stained by GSII staining. Some of the benign lesions were slightly stained.

    Metastatic carcinoma to the nose and paranasal sinus are relatively rare. We reported a case of hepatocellular carcinoma with metastasis to the ethmoid sinus. A 56-year-old male complained of the left facial pain. Tomography and CT scanning of the paransal sinus showed abnormal shadows in the left ethmoid sinus. The histo- pathological examination of the tumor revealed hepatocellular carcinoma. The patient was treated with chemotherapy (MMC,5-FU) and immunotherapy (OK-432), but died about 9 months later.
    The possibility of metastatic carcinoma should be considered when diagnosing head and neck cancer.

    We reported two patients with vertigo of central origin. Case 1 was a 68-yearold male who complained of right shoulder pain, bilateral tinnitus and right facial palsy. He was initially suspected of having an acoustic tumor, but was later found to have a tumor at the cerebello-pontine angle by neurotological examination. Case 2was a 54-year-old male who had a traffic accident 18 years earlier. He complained of headache, dizziness, cochlear symptoms and transient unconsciousness. This case was diagnosed as the vertibulo-basilar insufficiency with post-traumatic lesions on the basis of neurotological examination, including tests of nystagmus and OKN.
    These two cases indicate the importance of neurotological examination in patients with vertigo of central origin.

    Studies on the determination of the serum complement CHSO was determined in 54 patients with nasal allergy. Thirty-eight subjects with severe symptoms of more than five years' duration had a mean value of 37.08±1.698 standard deviation (p<0.05).
    This was significantly higher than that (30.18±3.43) in eighteen patients with mild symptoms of under five years' duration.
    There was a high correlation for background factors of nasal allergy, as assessed from information obtained on their first visit to the hospital.

    Two patients complained of hearing impairment, high fever and diarrhea after returning from abroad, and they were diagnosed as having severe typhoid fever based on bacteriological examination. Audiometry performed before treatment reveated sensori-neural deafness of both ears.
    After treatment with Chloram p henicol and Amino Benzil Penicillin, the patients hearing level returned to normal.

Four patients are presented to illustrate tho usefulln. ess of ultrasound in assessing masses in the neck. Ultrasound is capable of differentiating cystic from solid lesions but it does not permit differentiation between malignant and benign masses.

    A 74-year-old man became aware of a growing mass in the left parotid region in October 1980. The tumor (30×36 mm), which appeared in the left parotid area was elastic and mobile. On October 13,1980, the mass was removed surgically under general anesthesia. Histopathological examination revealed other type of monomorphic adenoma.

    Two cases of esophageal carcinoma with hoarseness as the earliest symptom, are reported.
    Case 1 was a 57-year-old man who complained of hoarseness, a right cervical mass, and pain in the right arm. Case 2 was a 68-year-old man with hoarseness only. Both patients showed right recurrent laryngeal nerve paralysis. Radiographic and endoscopic examination revealed esophageal carcinoma of the thoracic portion.
    These cases suggest that patients with recurrent laryngeal nerve paral y sis should underdo examination of the esophagus.

    Distant metastases were observed in 19 cases (11.6%) in a series of 164 patients with head and neck cancer and treated between 1974 and 1983.
    The incidence of distant metastases was high in patients w ith advanced primary tumors, i. e. T3 and T4.
    Pathological type s of distant metastases that were observed at a high incidence were MFH, lymphoepithelioma and undifferentiated carcinoma. The most common organs of distant metastases were the lung, bone and liver.
    Distant metastases of head and neck cancer were c o nsidered to be rare but according to the studies of pathological autopsies, it was higher than our clinical findings.
    Therefore, it was necessary to follow-up the head and neck cancer patients very carefully for a long period.

    We reviewed 8 cases of the paranasal sinuses that were treated at our department between 1977 and 1985, with respect to clinical picture, predisposing factors, diagnosis.
    The 8 patients (2 males and 6 females), ranged in age from 25 to 59 yea r s. The most common chief complaint was cheek pain followed by toothache, nasal obstruction, and rhinorrhea.
    In all c ases, mycosis was confined to an unilateral maxillary sinus. In one case, we found candida and in the remaining cases aspergillus. In two cases, bony erosion was noted in the medial wall of the maxillary sinus by X-ray examination.
    Mycotic culture of the fluid, which was taken by antralirr igation, and the necrotic debris did not reveal mycosis, but its cytological analysis leaded to the definitive diagnosis of mycosis prior to the operation in 3 cases.
    However, the remaining 5 cases were diagnosed by pathological examination of mucosa taken during operation. In 6 cases, surgical procedures were performed and in 2 cases, only antral irrigation were performed, which resulted in cure without systemic nor local antifungal therapy. We considered systemic, local and host-parasite factors as etiological factors in these cases.

    A 29-year-old male complained of a painless right neck tumor. He had undergone surgery for chronic otitis media 8 years ago. One year after this operation, he noted right facial nerve palsy.
    He was examined by computed tomography (CT), carotid angiography and biopsy. With contrast enhancement on CT, a hypervascular tumor shadow (5×4×9 cm) was detected in his right deep neck. Carotid angiogram demonstrated dilatation and deviation of the external carotid artery, and tumor staining in the carotid bifurcation. The biopsy specimen revealed venous hemangioma.
    In Jan.1984, he was operated on tumor resection under general anesthesia. However, the tumor could not be removed completely, because it was so large that it involved the parapharyngeal space. Adhesion was found between the tumor and carotid bifurcation.
    This tumor was diagnosed as paraganglioma by pathological examination.

    A 29-year-old man, who had complained of hoarsness and anterior neck mass, was diagnosed as having metastatic seminoma. He was treated by radiotherapy and chemotherapy, which were very effective, and there was no recurrence for 2 years and 2 months after therapy.

    A 51-year-old diabetic male patient who developed lifethreatening Ludwig's angina from a dental infection is presented. In spite of initial treatment consisting of antibiotics and surgical decompression, the infection extended to the abdominal cavity. Therefore, trachostomy and drainage of bilateral thorax, mediastimum and pericardium were performed. The decreased defense system of the diabetic patient was considered to have prolonged the infection.

    We reported a case of rhinogenic brain abscess with a severe course. The patient was a 48-year-old male, who was admitted because of sudden consciousness disturbance. Despite surgical treatment and administration of broadspectrum antibiotics, he died of brain abscess, which ruptured into the ventricular system, and other various complications, such as hydrocephalus, liver and renal dysfunction, and pneumonia.
    This case suggests that careful diagnosis and treatment are needed to avoid such an unfortunate outcome.

    A case of adnoid cystic carcinoma originating in the root of tongue is reported. A 59-year-old man complained of bilateral submaxillary tumor and pain in the mouth. He is progressing favorably after a successful operation and doing well. However, the possibility of metastasis or relapse of adenoid cystic carcinoma requires careful observation.

    A 14-year-old boy, who complained of a painful swelling of the palatine tonsil on the right side, was diagnosed as having malignant lymphoma. Biopsy of the tonsil was performed. The histological examination showed non-Hodgkin's lymphoma of diffuse, medium sized cell type and the clinical stage was classified as II. Local irradiation and chemotherapy for malignant lymphoma were performed. The tumor was initially responded to treatment, and the patient showed continuous remission until he died after about 4 months of treatment. The clinical course and schedule of chemotheraphy of this patient are reported.

    Three patients with orbital phlegmone occuring from paranasal sinusitis, who complained of unilateral exophthalmus, are reported. One of these patients developed meningitis dispite the intensive antibiotic chemotherapy. This case suggests that the best initial therapy for such cases is to administer a sufficient amount of appropriate antibiotics as soon as possible. In additionary, surgical procedures must be performed after initial treatment.

    We report a case of neuroblastoma arising from the nasopharynx. A 15-year-old girl presented with dysphagia, neck pain, and a large mass in the right side of her neck. Clinical involvement of cranial nerves (IX, XI and XII) was noted on that side. Because her clinical conditions became rapidly worse within about two weeks of admission and she was drowsy and had cramps, emergency irradiation (total 4350 rads) was performed immediately followed combination chemotherapy of vincristine and endoxan, After these treatments, she showed transient improvement, but died six months after the onset of the disease. Results of autopsy showed the generalized metastasis in the bone marrow, skull, liver, lung, diaphragm and lymph nodes of the paratracheal and neck regions. Microscopically, the diagnosis of immature neuroblastoma was confirmed. We considered that this case was a very rare case of primary nasopharyngeal neuroblastoma, which might have originated in relation to the cervical sympathetic chain of the prevertebral region.
