    We investigated the usefulness of bronchoalveolar lavage fluid(BALF)in the early diagnosis of cytomegalovirus pneumonia(CMVP), pneumocystis carinii pneumonia(PCP)and miliary tuberculosis(MTbc). This study included 41 patients : 4 patients with CMVP, 10 patients with PCP, 7 patients with CMVP and PCP, and 20 patients with MTbc. The early diagnostic rate obtained by analysis of BALF was 72.7% in CMVP, 100% in PCP and 40% in MTbc. BALF PCR assay in CMVP, BALF Grocott's stain and PCR assay in PCP, BALF smear and PCR assay in MTbc were useful for early diagnosis. Analyusis of BALF with PCR assay may be of great value in the early diagnosis of these pulmonary infections with diffuse pulmonary infiltration.

    To determine the clinical utility of a PCR-based test kit(AMPLICOR^<TM> Mycobacterium tuberculosis assay, Roche Molecular Systems)for bronchoalveolar lavage fluid(BALF), we retrospectively evaluated the efficacy of the commercially developed PCR test system comparing it with the results of conventional bacteriological methods. A total of 207 specimens(tuberculosis : 17, non-tuberculous mycobacterium(NTM) : 19, lung cancer : 50, pneumonia : 18, bronchiolitis : 11, metastatic lung tumor : 9, others : 83)from 202 patients were tested. When total tuberculosis, pulmonary tuberculosis and culture results were used as the reference standard, the sensitivity rates for the AMPLICOR test were 12/17(70.6%), 12/12(100%)and 12/13(92.3%), respectively. There were no positive results of AMPLICOR among non-tuberculous mycobacterium(19), lung cancer(50)and the specimens(45)in which acid-fast bacilli were positive because an automated bronchoscope-cleaning machine and the used bronchoscopes were transiently contaminated by NTM. The specificity for the AMPLICOR test was 97.9% using the total tuberculosis as the standard. The PCR test had a false positive rate of 2.1%. Although the test results from AMPLICOR could be obtained more quickly than that from culture, the clinical utility of this PCR-based test is similar but not superior to that of culture for detecting M. tuberculosis in BALF in terms of sensitivity and specificity.

    A 69-year-old woman was admited to our hospital because of cough and low grade fever. Het chest roentgenogram showed infiltration on bilateral upper lung fields. Transbronchial lung biopsy specimens showed marked infiltration of eosinophils in alveolar septa and thickening of alveolar septa. She had taken Pabron-S^[○!R]an over-the-counter cold medicine containing acetaminophen for ten days and naproxen for a month before her admission. She stopped using the two drugs and her symptoms and signs improved spontaneously without any treatment. A drug lymphocyte stimulation test(DLST)was positive for acetaminophen only. To confirm the diagnosis, a drug challenge test for acetaminophen was performed with her informed consent. A total of 2000 mg of acetaminophen was orally ingested during two days, with negative test results. In addition, Pabron-S^[○!R]alone and in combination with naproxen were taken, but the results were also negative. Therefore acetaminophen was ruled out as the cause of eosinophilic pneumonia in this case. It is difficult to diagnose drug-induced pneumonitis by the tesults of DLST alone.

    A 35-year-old previously healthy man suffered from varicella. The patient complained of fever and eruptions, and was admitted. In addition to pneumonia, he had liver dysfunction and thrombocytopenia associated with varicella. Despite a lack of respiratory symptoms he had multiple small nodular shadows, irregular in size, in both lung fields on chest X-ray film. Bronchoscopic examination revealed multiple vesicles on the tracheal mucosa and a moss-like substance in the bronchial mucosa. Examination of the transbronchial lung biopsy specimen revealed VZV DNA using the polymerase chain reaction(PCR)method of DNA amplification, as well as interstitial inflamation. The biopsy specimen of the bronchial lesion also showed VZV DNA by PCR analysis. The patient was treated with antibiotics, acyclovir, and immunogloblin. The skin eruptions and disturbances of gas exchange resolved in about two weeks, but the nodular shadows on chest X-ray films remained for more than eight weeks.

    We report three cases of pulmonary mucormycosis diagnosed by transbronchial lung biopsy. Case 1 is a 53-year-old man. He did not have any previous illness. His chest X-ray film showed and infiltrative shadow with a fungus ball in the cavity. Transbronchial lung biopsy revaaled pulmonary mucormycosis. Antifungal agents improved his symptoms and abnormal X-ray finding. Case 2 is a 55-year-old man who had received treatment for acute myeloblastic leukemia. When his bone marrow was suppresed, his chest X-ray film showed an infiltrative shadow with cavitation. Pulmonary mucormycosis was diagnosed by transbronchial lung biopsy. The oral administration of antifungal agents improved the abnormal shadow on X-ray film. Because the abnormal X-ray findings suggested relapse of pulmonary mucormycosis, left upper lobectomy was performed. Case 3 is a 52-year-old man under treatment for diabetes and non-Hodgikin's lymphoma. Chest X-ray film showed a fungus ball in a preexisting cavity caused by old tuberculosis. Transbronchial lung biopsy revealed pulmonary mucormycosis. We suggested that the host immune system and their underlying diseases resulted in the varied chest X-ray characteristics of pulmonary mucormycosis and their clinical courses.

    A 64-year-old female who had been receiving treatment from her doctor her doctor for hypertension, consulted our hospital because a narrow infiltrative shadow had been detected in the left middle lung field on chest X-ray. She had no respiratory symptoms such as cough, sputum, or stridor. Serum precipitating antibody to aspergillus was positive. Chest tomograph and computed tomograph revealed atelectasis of the left S^4 and calcification in the lingular bronchus. Bronchoscopic finding revealed complete obstruction at the orifice of the left B^<4+5> caused by yellowish-white material. Numerous aspergillar hyphae were detected in the biopsy speicimen, and Aspergillus fumigatus was cultured from washing of the material. The diagnosis was broncholithiasis with saprophytic colonization of Aspergillus fumigatus. Because hemoptysis and obstructive pneumonia were possible in the future, left lingular segmentectomy with bronchoplasty was performed.

    A 65-year-old man was admitted to our hospital with bloody sputum. A chest X-ray film and chest CT scan showed no abnormality. Bronchofiberscopy revealed a rough-surfaced polypoid lesion about 4 mm in diameter in the trachea. Pathological and cytological finding of the biopsied specimen revealed inflammatory granulation tissue. Bronchoscopy after 1 week revealed no change and the biopsied specimen revealed no malignancy. The polypoid wasidentified microscopically as inflammatory polyo of the trachea and antibiotic treatment was started. Bronchoscopy after 2 month showed complete disappearance of the polypoid lesion.

    A 49-year-old woman suffered from tracheal stenosis when in a supine position, apparently due to posttracheotomy cartilage deformation. Stenotic symptoms were improved by implanting a nitinol stent under general anesthesia with ventilation by a laryngeal mask. After 3 months, although granulation tissue projecting out of the mesh was observed at two points, the stent resulted in good patency and she was in excellent condition. Slight difficulty in spitting due to adhesion of mucous sputum to the stent was observed postoperatively, necessitating long term care of the airway and regular observation. The nitinol stent has many advantages : to be inserted without difficulty, to keep functions quite satisfactory, and to result in very few complications. The demerits of this stent were also discussed.

    A 76-year-old man was hospitalized because of progressive dyspnea on exertion. Chest X-ray films on admission showed atelectasis of the right upper lobe. Bronchoscopy revealed a hamartoma obstructing the orifice of the right upper lobe bronchus. Endoscopic Nd-YAG and low wave length KTP laser therapy was performed, because the mass was large and its root could not be identified. He has been well after the treatment, with no evidence of recurrence. Endoscopic therapy using two types of laser may be useful in treating an endobronchial hamartoma in elderly patients with low pulmonary function.

    An abnormal shadow on the chest X-ray of 34-year old man was pointed out, and he was given a diagnosis of cystic bronchiectasis with mucoid impaction. Five years later, March 1999, he was again checked up by the chest X-ray screening, and ne visited our hospital for further examinations on April 26. Chest computed tomographic scan revealed a dendritic solid shadow, 4 cm in size, with a cavity in part of it, expanding from its center in right B^6b along the bronchus. Because an air-fluid level, which is a characteristic radiological finding of cystic bronchictasis, was not found in this cavity, we performed bronchoscopy. A hemorrhagic reddish polypoid tumor occluding the right B^6b lumen was detected, and an atypical carcinoid was diagnosed by transbronchial biopsy. On June 10, right lower lobectomy was performed. Histopathological examination of the resected specimen confirmed that the tumor arose from right B^6 and grew dendritically along the longitudinal axis of the bronchus, and that most of the tumor was located within the bronchial lumen, resulting in the marked dilatation of the bronchus.
