    A questionnaire survey was conducted in order to investigate the methods of bronchofiberscopy (BFS) currently used in Japan and its complications. A total of 286 medical departments of universities and major hospitals throughout Japan responded. Ninety-seven percent of the institutions that replied said that they performed BFS with local anesthesia only. Approximately half said that, as a rule, they conduct tracheal intubation. This is less than the proportion found in a survey four years earlier. With regard to the frequency of the types of BFS procedures, "observation only, ""biopsy of hilar lesions" and "curettage of peripheral lesions" accounted for approximately 30% each, while "TBLB" accounted for the remaining 10%. Hemorrhage due to biopsy as well as pneumothorax (4.1%), hemorrhage and cardiac and respiratory failure due to TBLB were the most frequent complications. Death occurred in 18 cases. The cause of death was hemorrhage in 11 cases, pneumothorax in two, cardiac arrest and myocardial infarction in five. Although BFS is a relatively safe examination method, death can occur in some cases, which suggests that caution should be exercised. The incidence of complications is particularly high with TBLB. Thus, greater care must be taken when this method is employed.

    The type of branching and the relative frequency was evaluated in the normal lower lobes of 300 cases in which the subsegmental bronchi were examined from July 1979 to June 1982. The type of branching was also examined by bronchography in almost all cases. In both right and left B^6, bifurcation to B^6_a and B^6_<b+c> was seen to be the most common type of branching, whereas equal trifurcation to B^6_a, B^6_b and B^6_c was observed in less than 20% of cases. In the left basal bronchus, bifurcation to B^8 and B^9+B^<10> was overwhelmingly the most common, being recognized in almost 2/3 of cases. The absence of B^7 was recognized in 3.7% of cases. In both right and left B^<10> bifurcation to B^<10>_a and B^<10>_<b+c> was recognized in over 90% of cases. In the left lower lobe the segmental bronchi are named turning in a rightward direction from B^6. B^6 and B^<10> show the most central branching of subsegmental bronchi, a, b, c, also turning to the right. B^8_a and B^9_a are in the most distal direction while B^8_b and B^9_b are in a mediastinal direction. Also in the right lower lobe the bronchi are named, turning clockwise to the right from B^6, as B^7_a, B^7_b, B^8, B^9 and B^<10>. The subsegmental bronchi of B^8 and B^9 are named in the same manner as in the left bronchus.

    Bronchial artery embolization (BAE) was performed in nine cases with massive hemoptysis, and in six cases we could use this technique. BAE was effective in five cases except for a case whose lesion was fed by both bronchial and intercostal arteries. Of the five cases three showed a good course for more than a year, but the other two died due to other diseases within a month after BAE. Fever, chest pain and abdominal pain were observed in all cases. One case showed transient elevation of lactic dehydrogenase. BAE is a very useful technique to control massive hemoptysis.

    Transbronchial lung biopsy (TBLB) is used for the diagnosis of pulmonary nodules and diffuse lung disease. Reports of complications have been infrequent, but deaths from hemorrhage have been reported. In the present investigation patients who suffered from hemorrhage induced by TBLB, bronchial mucous membrane biopsy and from local injection of anticancer drugs were treated with thrombin solution. Hemorrhage (30-200ml) occurred in 12 of 122 tumor biopsy cases, 14 of 170 TBLB cases, 2 of 102 local anticancer drug injection procedures and 1 of 20 bronchial mucous membrane biopsy cases. For these 29 cases of hemorrhage, we instilled 5 ml of thrombin solution (100 U/ml). In 27 patients, hemorrhage stopped after instillation of one dose of thrombin, and in the 2 other patients stopped after a second dose within 2 minutes. The avove results suggest that thrombin solution instillation therapy might be a useful method to treat hemorrhage induced by the various fiberoptic procedures.

    104 cases of esophageal diseases which were admitted to our hospital during the last five years were reviewed. Of these, 13 cases were clinically found to have some influence on the airway system and 9 cases of esophagus carcinoma were recognized to be accompanied by various lesions, such as bilateral vocal cord paralysis, invasion to the carina and main bronchus, tracheo-esophageal fistula and bilateral carcinomatous pleural effusion. A case of Boerhaave syndrome with left hydropneumothorax resulting from perforation of the lower esophagus seemed to be one of the most serious esophageal diseases in terms of the severe influence on the airway system. In two cases of achalasia, the megaesophagus compressed the membranous portion of the trachea, causing tracheal stenosis demonstrated by the maximal inspiratory-expiratory flow-volume curve. A case of tracheo-esophageal fistula supposedly caused by mediastinal lymph adenitis suffered from recurrent pneumonia, and esophagography demonstrated the pooling of ingested barium in the right lower lobe. When the symptoms and signs of central airway are recognized, esophageal disease should be suspected as a possible cause of these disorders.

    A 50 year old house wife, who had received total histerectomy followed by postoperative irradiation for uterin cervical cancer three years ago, was admitted complaining of dry cough. The initial chest X-ray was normal except for a minimal irregular density in the right lower lung field. Bronchoscopic examination demonstrated several white nodules located around the right upper lobe bronchus orifice, and a biopsy specimen from the lesion revealed subepithelial squamous cell carcinoma. Under a diagnosis of metastasis of cervical cancer, Bleomycin and Mitomycin were administered. However the endobronchial lesion extended to the right middle and lower bronchus, and subsequent serial chest X-ray showed increasing nodulolinear density compatible with lymphangitis carcinomatosis. It was strongly suggested that an endobronchial lesion could be a manifestation of lymphangitis carcinomatosis in the bronchus.

    A 54-year-old woman with recurrent hemoptysis underwent transcatheteral embolization of her bronchial and intercostal arteries. The materials used for embolization were spongel and an occluding spring embolus (Cook Inc.). After the first embolization by spongel, hemoptysis and hemosputum ceased within a few days. Angiography 3 weeks after the procedure showed recanalization of the occluded vessels and reopening of the intercostal-pulmonary anastomosis, so these vessels were again occluded with spongel. Four months later the patient returned to our hospital after reoccurrence of hemoptysis. A third procedure was needed to control massive bleeding. Occlusion of the arteries with spongel was temporary ; though permanent occlusion of the arteries was needed, the patient was a poor candidate for surgery. A new device, a small stainless steel wire with multiple strands of dacron wool, was used with good results. No complications were encountered with the use of this wire.

    A case of primary racemose hemangioma of the bronchial artery is reported. A 52 year-old woman was admitted because of repeated massive hemoptysis. Bronchoscopic examination revealed an aneurysm with irregularity of the wall of the right basal bronchus, but the nature and location of the couse of hemoptysis was not detected by bronchoscopy or bronchography performed previously at another hospital. Hemoptysis was considered to be due to racemose hemangioma, which was confirmed by extraordinal hypervascular area on bronchial arteriogram and hemangiomatous hyperplasia of submucous vessels, on resected specimen of right basal bronchus. And also the lesion was considered to be primary racemose hemangioma of the bronchial artery, because there were no findings of chronic inflammatory lesions in the histological examination. Bronchoscopy is the most valuable examination for detecting the location and nature of hemoptysis, and bronchial arteriography seems to be another reliable technique for identifying the area and origin of hemoptysis.

    A case of triple bronchogenic carcinoma was reported. The patient had been a chromate worker for 17 years. In 1975 the first malignancy, which was squamous cell carcinoma, was detected in the right lower bronchus, and lower lobectomy was performed. Thereafter for 4 years and 7 months he recieved chemotherapy, and was followed up. In 1980 the second two malignancies were found in the left lingular and the right upper lobe bronchi, by means of fiberoptic bronchoscopy. The former was small cell carcinoma and the latter was squamous cell carcinoma. Resection was not thought to be indicated In spite of chemo-, immuno- and radiation therapies, he died from brain and liver metastases of small cell carcinoma in 1981. This case seemed to present many problemes about the diagnosis and treatment of synchronous and asynchronous multiple lung cancer.
