In esophageal endoscopic ultrasound-guided fine-needle aspiration cylology (biopsy) is cells or tissues are obtained by a needle from the esophagus or gastric cavity. A curved linear array echoendoscope is used. The results of published data were sensitivity 75-100%, specificity 83-100%, overall-accuracy 83-100% , average sensitivity 89.6%, specificity 98.5%, overall accuracy 92.4%. Complications of EUS-FNA was 1-20%.

Purpose. We described 7 cases of benign diseases in the peripheral lung, diagnosed in 2003 by transbronchial biopsy using endobronchial ultrasonography with a guide sheath (EBUS-GS) . Patients aud Methods. A 20 MHz radial type probe (XUM-S20-17R or 20R, Olympus, Tokyo) with a special guide sheath was inserted via the instrumentation channel of the bronchoscope. After the EBUS image of peripheral pulmonary lesion was obtained to confirm the proper position for biopsy, only the probe was removed and biopsy or brushing was subsequently performed through the guide sheath. Results. One lesion was located in the right upper lobe, 2 were in the right middle lobe, 2 were in the right lower lobe, I was in the left upper lobe, and I was in the left lower lobe.

Background. It is often difficult to distinguish pulmonary actinomycosis from lung cancer, tuberculosis and fungal infection. Case. A 62-year-old man with diabetes mellitus and bronchiectasis was admitted to our hospital because of persistent hemoptysis. Chest computed tomography revealed a nodular lesion in the right S^2-S^3. Serum tumor markers and plasma β-D-glucan levels were normal. Sputum cytology and culture were negative. Bronchoscopy showed a white mass in the right B^3 orifice and bleeding was seen in the distal portion of the bronchus. The mass was removed with forceps. The diagnosis was confirmed by detection of sulfur granules in the removed materials. After ampicillin treatment, symptoms disappeared and radiological abnormalities markedly improved. Conclusion. We reported a case of pulmonary actinomycosis with hemoptysis. Bacterial clots were directly observed by bronchoscopy. Pulmonary actinomycosis is a rare disease. In this case, the bronchoscopic procedure was useful to diagnosis and treatment.

Background. We report a case of congenital tracheal diverticulum with pneumonia. Case. A 27year-old woman was admitted with chief complaints of cough, sputum, chills, and dyspnea. Chest computed tomography revealed bilateral consolidation and a cystic lesion with air-density on the right side of the trachea. Multilobar pneumonia was diagnosed and she was treated with antibiotics. Flexible bronchoscopy examination revealed a tracheal diverticulum filled with mucus at the right posterolateral wall, 9 x 6 mm in size 30 mm above the carina. Congenital tracheal diverticulum was diagnosed. Conclusion. Tracheal diverticulum is a rare disease, and this anomaly may act as a reservoir for secretions, leading to respiratory tract infection. Clinicians should be aware of this anomaly when respiratory tract infection recurs.

Background. It is occasionally difficult to visibly detect pulmonary peripheral small nodules in video-assisted thoracic surgery (VATS), and pre-operative markings are useful in confirming the lesions and resection margins. The conventional way of percutaneous marking has a high risk of causing complications such as pneumothorax or bleeding. In addition, percutaneous punctures are sometimes contraindicated by the location of neighboring organs. Cases. We performed pre-operative barium marking using an ultrathin bronchoscope under virtual bronchoscopic navigation in two cases with pulmonary peripherai small nodules. Results. Five points were marked for the two lesions. During VATS, the markings were clearly detectable under fluoroscopy and the one near the pleura was found visibly. Pathological diagnoses were adenocarcinoma in both cases. Conclusion. This procedure was of benefit in detecting lesions and deciding on the resection margins in VATS for pulmonary peripheral small nodules.
