To clarify the optimal treatment of peristomal pyoderma gangrenosum(PG),the medical records of fourteen patients with peristomal PG were analyzed in terms of the clinical diagnosis, operative procedures, time until diagnosis of PG,initial dose of prednisolone, period required for healing of PG,local treatment, patients' complaints, and so on. In conclusion, at the onset of peristomal PG,early diagnosis and systemic administration of prednisoline proved to be necessary. In topical management, gentie skin care, topical treatment using cadexomer-iodine and prednisolon ointment, application of hydrocolloid dressing, drainage by cutting of skin barrie, and early change of one-piece stoma bag proved to be effective.

17 years passes after a skin barriers classification table by composition released in 1990 and this is used like a standard in our society.
In the other day,the diversification of the skin barriers moved ahead,an inconvenience in case of using this is felt, because of no proper position for newly developed item.
For solution of this problem, the author create new classification table of main skin barriers on market based on information from product catalogue.New table is expected to be useful for academic and clinical application.

The commercialized skin barrier of stoma appliances is hydrocolloid, can absorb sweat or secretion and attach with no inhibition of physiology function. However ,peristomal skin of the Ostomates has been damaged by removal irritation, repeatable peeled off the stoma appliance faceplate from skin surface.Until now,it has not been designed the hydrocolloid matrix to minimize the skin damages of the Ostomates. To improve barrier function by mixing ceramide constituent of intracellular lipids with skin barrier and enhancing function of stratum corneum,we had conducted patch test with health individuals for one month and with ostomates for one year.
As the result, the TEWL(Transepidermal Water Loss)value of both of health individuals and ostomates were improved by mixing ceramide with skin barrier. From this point, the skin barrier contained cedamide is useful for maintaining and improvement of barrier function.

The intestinal irrigation method by which ostomates can perform irrigation by themselves was developed. This method(a closed soft bag irrigation system)employs a soft plastic bag containing an isotonic sodium chloride solution, and irrigation is performed by pressurizing the soft bag by body weight in a sitting position.lt is possible to irrigation at any times and places of their preference without restraints, and privacy can be protected.Same with usual times such as at home or social life,no special facility for the conduct of irrigation may not be required even during times of disaster.

The purpose of this study is to determine for characteristics of the care that Wound Ostomy and Continence Nurses provide for ostomates in the ostomy outpatient clinic.
The datas were obtained fron observations of the interactions between six pairs of Wound Ostomy and Continence Nurses and ostomates, and analyzed bu qualitative and inductive methods.
As a result, three categories that express Wound Ostomy and Continence Nurse's care were extracted:“Protecting the stoma,”“Deceloping one's self care ability,” and “Supplementing ostomate's energy”
The characteristics of the Wound Ostomy and Continence Nurse's care for ostomates included the skills to treat stoma that acquired as a body sense, the interactiona that emphasize the ostomate's selfs confidence and self-decision making, and the communications that accompany the interchange of deep affection.
