1997年6月26日，長野県下水内郡信濃町大井のコナラ林で，ちょうど総苞を開げた花序を一面につけているヤマボウシに出会った．観察していたところ，球状の頭花からドーム状にふくらんでいる開花直前の小花のつぼみに指で触れると，突然花弁が開き，おしべが展開することを増井がみつけた．同じ状態のつぼみについては，同じ反応があることを知った．ヤマボウシの開花行動はこれまで記録されていないと思うので報告する．

富士吉田市のクレソン水田ではほぼ半数の田にイケノミズハコベが発生しており，増殖が旺盛で，植え付けたクレソンの隙間に侵入して生育を阻害し，収穫の際に混入して問題になっている．クレソン栽培では通常年間を通して10～15cmの水深で，湧水を掛け流す．この条件下で，イケノミズハコベ以外には，セリ，コカナダモ，エビモ，バイカモ，カワヂシャなどの水生植物が発生しており，また，コウキクサや東海地方で知られていた帰化種のイボウキクサ（Lemna gibba: 長田1976）も多く発生していた．すなわち，年間を通して水温の低い流水が保たれる条件下で帰化植物を含む特異な植生が成立したものと考えられた．

57歳，男性．7年ほど前より右下腿に有痛性の皮下腫瘤が出現した．初診時，右下腿内側に10×10 mm大の境界明瞭な弾性軟の皮下腫瘤を認め，揉むと硬くなり疼痛が出現した．超音波所見では，結節はlow echoic lesionで内部エコーはほぼ均一であった．カラードプラでは血流が豊富で，揉むと腫瘍は硬くなって血流シグナルが消失し，温めると再び血流シグナルが出現した．病理組織学的所見では，血管と不規則に交錯する紡錘形細胞から構成されており，血管平滑筋腫と診断した．血管平滑筋腫は発作性に疼痛が出現することが知られるが，疼痛出現前後の超音波所見の変化についての報告はいまだない．本稿では，自験例の超音波所見を記載するとともに文献的考察を加えた．

The clinical effects of tizanidine hydrochloride were evaluated in 58 patients with tinnitus. The subjective improvement rate was 60.0%. A better effect was obtained when the loudness of the tinnitus was more than 10 dB and when treatment was continued for over 4 weeks. Treatment with tizanidine hydrochloride alone was as effective as combined treatment with other drugs.
Shoulder stiffness improved in 47.5% of the patients and in these patients tinnitus responded better. Side effects were observed in 15.5%, but none were serious.

モルモット1匹あたり10mg/kgのメタンフェタミンを腹腔内注射し, 同剤が聴覚系に及ぼす影響をプライエル反射域値測定器やABRを用いて測定した。この結果, プライエル反射域値は薬剤投与後30分, 60分で有意に低下した。一方, ABRについては, 薬剤投与後ABRのI, II, IV波潜時は240分まで, III波潜時は180分までに有意に短縮し, 波間潜時についてもほぼ同様の傾向を示した。しかし, 振幅や域値については, 薬剤投与前後で変化がみられなかった。以上より, メタンフェタミンは聴覚伝導路全搬において, 潜時を短縮させる作用をもつことが判明した。

ミリ波を放送波として利用する場合、サービスエリア内でどの程度の受信者(世帯)が受信可能になるかという受信場所率を検討する必要がある。本稿では多数の小電力送信局を設置した場合について、建物高さや幅データを持つ3次元の住宅地図データを利用して、受信場所率のシミュレーションを行った。典型的な住宅地区である世田谷砧地区の例では、1km^2に送信基地局が20局の時、固定受信で「世帯場所率」約90%、移動受信では「道路場所率」約70%が得られた。

Since the discovery of auditory brainstem response (ABR), considerable interests have been generated in the practical application of ABR for experimental and clinical studies on auditory systems. The present work was carried out to investigate the basis of rat ABR and the implication of noradrenergic(NA)systems on brain stem auditory transmission.
The rat ABR, appearing within a latency of 10msec, is composed of 6 to 7 waves, the 2nd wave having clearly separated small two peaks (IIA and IIB). Electrical lesions of the dorsal cochlear nucleus resulted in the disappearance of the IIB peak, remaining the IIA intact. This result suggests that a specific part of the cochlear nucleus may be involved in the generations of each peak. Amphetamine as well as phenylephrine (NA-α1 agonist) shortened the latency of each wave, these changes being antagonized by prazosin, aNA-α1 antagonist.
Amplitudes of the IIB peak and after decreased following injections of clonidine, a NA-α2 agonist. Similar changes were detected after electrical stimulations of the locus coeruleus. Yohimbine, a NA-α2 antagonist, obviously antagonized clonidine and also locus coeruleus stimulation effects.
These results indicate that there are at least two NA systems influencing on the activities of acoustic transmission in the brain stem. One is a NA-α1 system which causes a diffused activation of the central nervous system and then causes the shortening of ABR latencies. The other is a NA-α2 system which is probably related to activities of efferent neurons from the locus coeruleus and serves as an inhibitory function on acoustic transmission.

The clinical effects of tizanidine hydrochloride on tinnitus were investigated in 41 patients (60 ears).
1. “Slight improvement, ” “moderate improvement, ” or “great improvement” was reported for 75% of the 60 ears.
2. The agent was more effective in females than in males. The type of tinnitus most effectively treated was that associated with sensorineural hearing loss of unknown etiology.
3. Patients with a tinnitus history off under 5 years, with pure tone tinnitus, or high pitch tinnitus responded best.
4. Tinnitus with hearing loss of less than 40dB was more responsive to the agent than that with hearing loss of more than 40dB.
5. Patients were given 1mg, 2mg, or 3mg per day. The 3mg per day group showed the best improvement.
6. The agent proved effective in a majority of patients who did not respond to eperisone hydrochloride treatment.
7. Shoulder stiffness was reduced in 50% of the patients and headache or head dullness in 75% with these complaints.
8. Side effects were noted in 3 patients (7.3%).

The present study was designed to investigate how ABRs of the rats are influenced by experimental manipulations of central noradrenergic (NA) activities. Experiments were conducted on Wistar strain male rats anesthetized with α-chloralose. ABR waveform patterns had dependencies on electrode positions. A clear pattern with a distinct separation of each peak could be recorded when an electrode was placed on the midline of the skull close to the lambda. A reference electrode was placed on the tail or pinna. Clonidine, an NA α2-agonist, depressed the amplitudes of ABR at intravenous doses of 0.05-0.1mg/kg; larger changes in peaks IIb and IV were noted. These effects were antagonized by yohimbine, an NA α2-antagonist. Electrical stimulation of the locus coeruleus also depressed peak II and beyond in a current-dependent manner. The maximum inhibitory effect was observed at the conditioning-test stimulus interval of about 50msec. Yohimbine suppressed these changes, too. These results suggested that an NA system of α2-type, originating from the locus coeruleus, was involved in the efferent modulatory function on the auditory afferent system.

移動通信における瞬時受信レベル周波数相関係数について, 遅延プロファイルの到来波振幅が指数分布する広帯域伝搬モデルに基づきコンピュータシミュレーションを行った.その結果, 相関係数は受信帯域幅と遅延プロファイルの傾きを表す平均通路長差に依存することがわかった.更に, 到来波振幅が一様分布する場合と比較を行い, 到来波振幅分布の周波数相関係数に与える影響を明らかにした.

近年,超音波パルス・ドプラー法によって弁膜疾患の逆流度評価を非侵襲的に行う試みがなされている.今回,我々は大動脈弁閉鎖不全症においてカラー・ドプラー法にて認められた弁上部吸い込み流れ血流(suction signal)を検索し他の重症度評価との対応について検討した.Suctionsignalはカラー・ドプラー法にて大動脈弁逆流シグナルの認められた47例中20例に検出された.S秘ctionsigna1の検出によって,III/IV以上の大動脈弁逆流を診断するsensitivityは100%, speciflcityは64%, predictive accuracyは67%であった.また,逆流点に向かうV字状のsuctionsignalは,逆流口の同定に有用と考えられた.長軸像で計測されたsuctionsignalの面積は,左室内逆流血流シグナルの面積と有意な相関を示し,大動脈造影およびパルス・ドプラー法での腹部大動脈血流パターンによる重症度に準じて大となる傾向を認めた.suctionsignalの面積が40mm²以上でIII/IV以上の高度逆流を診断するsensitivity88%, specificity91%, predictiveaccuracy88%であった.すなわち,suctionsignalの広がりは大動脈弁閉鎖不全症重症度評価の一指標になり得ると考えられた.

近年グラム陰性桿菌感染症の増加に伴い, 小児科領域においても他剤に抵抗を示す重症感染症に遭遇することが多く, アミノ配糖体系抗生剤を使用せざるを得ない場合もしばしばある。しかも, これらの薬剤は筋肉内投与だけが許可されているが, 小児科領域では筋拘縮等の局所の毒性の問題があり, 主に静脈内投与が行われているのが現状である1)。
アミノ配糖体系抗生剤を一定時間かけて点滴静注した場合の血清中濃度推移は, 筋注時とほぼ同じであるとされており1), 我々が行つたTobramycin (TOB) に関する検討2, 3) においても30～60分点滴静注が筋注時と同様のバターンを示すことが確認されている。
今回, Gentamicin (GM) を小鼎こ点滴静注にて投与し, その薬動力学的検討を行つたので報告する。

耳鳴の薬物療法

Patients with tinnitus were treated with Ternelin® (tizanidine hydrochloride) alone or in combination with Hydergine® (dihydroergotoxine mesylate). The following results were obtained.
1) When overall improvement was assessed at the end of the dosing period, 36% of the 136 patients who received Ternelin and Hydergine® (the combined therapy group) showed moderate or marked improvement, while 61.8% of the patients in this group showed slight, moderate or marked improvement. The combined therapy was rated as moderately or highly useful in 35.3% and slightly to highly useful in 61% of all cases.
2) Of the 24 patients who received Ternelin alone (the solo therapy group), 25% showed moderate or marked improvement, and 66.7% showed slight, moderate or marked improvement. Solo therapy was rated as moderately or highly useful in 25% and slightly to highly useful in 66.7%.
3) of the 153 patients who received combined therapy (including 17 patients who were only included in the safety assessment), 9 developed adverse reactions (3 cases of drowsiness, and one each of orthostatic syncope, hypotonia, dizziness, gastric pain, constipation and oral thirst). None of the patients given Ternelin® alone showed any adverse reactions.
4) The percentage of cases showing marked or moderate improvement was higher in the combined therapy group than in the solo therapy group, although this difference was not significant. In the ombined therapy group, the reduction in tinnitus peaked 2 to 4 weeks afterr the start of treatment. In the solo therapy group, the number of patients responding to the drug increased gradually with time, and more time was needed to obtain maximal effects than that in the combined therapy group.

We used tizanidine hydrochloride, a muscle relaxant, and dihydroergotoxine mesilate, an activator of central nervous system function, as basic drugs in the treatment of 48 patients with tinnitus (25 males and 23 females, aged 25 to 82 years).
Usual amounts of the two drugs were administered daily alone or in combination with other drugs for 2 to 31 weeks, and the clinical effect was evaluated by analyzing subjective tinnitus and accompanying symptoms before and after treatment.
Loudness, continuousness and distress of tinnitus, and headache and shoulder stiffness as accompanying symptoms were reduced significantly after treatment (p<0.05 with Wilcoxon test). The therapy was markedly effective in 2 (4.2%) of the patients, moderately effective in 15 (31.2%), and slightly effective in 13 (27.1%).
Side effects of sleepiness or digestive symptoms were reported by 4 (7.4%) out of 54 patients. These and two others stopped treatment within 2 weeks.
Comparing the results of treatment of tinnitus with a muscle relaxant only, we con-clude that the present combination of the two drugs is more effective in relieving both tinnitus and accompanying symptoms.

Although the Japanese government is promoting the use of generic drugs in order to reduce medical costs, providing information to physicians and pharmacists on the quality, safety or utility of generic drugs compared to the original drugs is insufficient. Carboplatin is a platinum-containing antineoplastic agent that is used for the treatment of several types of cancer. Carboplatin Intravenous Infusion “NK” (GE-CBDCA) is a generic drug of Paraplatin^® Injection (OR-CBDCA). In this study, we investigated the quality and safety of OR-CBDCA and GE-CBDCA. Purity tests conducted using high performance liquid chromatography showed that there were no clear differences between the two drugs with respect to contaminant or related substances. As a result of susceptibility testing performed in vitro by using five types of tumor cells, both drug products showed similar receptivity. For comparison of adverse drug reactions, the incidence of adverse events was surveyed in 72 patients who underwent chemotherapy switched from OR-CBDCA to GE-CBDCA. No significant differences were found in seven of eight parameters. Hematological tests of hemoglobin and white blood cell counts tended to show a higher incidence of more serious adverse reactions in the group treated with GE-CBDCA, and the GE-CBDCA-treated group also showed significant higher neutrophil counts (P = 0.029). However, it was supposed that the total dosage and the administering period of the Carboplatin were factors of the adverse reaction in the GE-CBDCA-treated group, therefore, the quality and safety of the two drug products were considered to be similar.
